INDIAN HEALTH SERVICE

PHOENIX INDIAN MEDICAL CENTER

Statement of Work

Background The pharmacy cabinetry in the Primary Care Medicine Clinic, Specialty, and Main Hospital pharmacies needs replacement.

Objective The objective of this contract is to furnish, deliver, remove existing cabinetry, and install new modular pharmacy cabinetry systems at the Phoenix Indian Medical Center in accordance with the specifications contained herein.

System Requirements A. Attached drawings may be used as concept and specifications as the basis of design.

B. The following performance and design criteria are project requirements and must be met. Any deviations from these specifications must be noted, item by item, by the bidder and a clear justification for the deviation must be provided with the bid.

1. Modular System: The product required for this project shall incorporate a recognized modular design from which system components may be switched out, added or deleted as necessary with minimal disruption to the work environment.

2. Factory Assembled Cabinet Assemblies: All cabinet boxes shall be factory assembled with appropriate joinery techniques to ensure that a square and solid cabinet box is obtained.

Sharing of cabinet sides and/or backs is not acceptable as it limits the products modularity.

3. Radiused Corners in Picking Station: The unit dose picking station shall include modular radiused corner modules that promote ergonomics and workflow efficiency.

4. USP-795/797 / USP 800 Compliant: The final design provided by the vender needs to be compliant with USP-795/797/800 for the compounding of sterile preparations and hazardous drugs at the appropriate risk level as determined by pharmacy management. The contractor shall demonstrate documented experience in designing and installing USP <795>, USP<797>, and USP <800> compliant pharmacy environments. The contractor shall provide written certification that the final installation complies with applicable USP standards at the risk level designated by pharmacy leadership.

5. Integrated Modular Electrical System: Provide, attach and connect together an integrated electrical system comprised of 20-amp hospital grade duplex receptacles that are modular in design allowing for the use of up to 4 circuits with 4-line connectors by simply switching the receptacles in the system blocks. One circuit shall be isolated and dedicated and serviced by its own line conductor, neutral and isolated ground. This system must lock into the modular casework specified herein through the use of factory machined and installed mounting plates to insure secure attachment. The electrical system shall be routed under the countertop and accessible to power consuming equipment above countertop level by the use of wire grommets. This system must be UL and CSA recognized.

An electrician provided by either the facility or general contractor shall perform final attachment of the electrical to facility power. Adequate installation instructions shall be provided to the electrician prior to final connection is completed in order to guarantee a successful installation of the electrical product specified.

6. AL3300 Raceway: Provide and install dual channel aluminum raceway and devise cover plates. Standard cover plates fit standard duplex receptacles for electrical or data devices. Additional cover plates are available per the customer requested requirements. Internal devices, wiring and/or final connections are the responsibility of the facility.

7. The contractor shall furnish all labor, materials, equipment, and supervision necessary to cut a new wall opening and install one (1) counter roll-up security door and one (1) pharmacy transaction window in the Specialty Pharmacy at the Phoenix Indian Medical Center.
