Statement of Work (SOW) Annual Service For Tissue Processor, Excelsior AS

GENERAL INFORMATION

1.0 Scope of Work: Contractor will provide factory trained engineers to perform one

(1) preventive maintenance on site service visit and unlimited emergency visits on the Excelsior AS Tissue Processor located at 1920 Dayton Ave. in Ames, IA.

Covered components will include:

1) Excelsior AS Tissue Processor w conditioner – SN: ET70212009EP

2) All OEM replacement parts

3) All labor and travel expenses

4) Annual Preventative Maintenance

Coverage: Base (12 months) coverage: approx. 02/01/2022-01/31/2023 Option Year 1: approx. 02/01/2023-01/31/2024 Option Year 2: approx. 02/01/2024-01/31/2025 Option Year 3: approx. 02/01/2025-01/31/2026 Option Year 4: approx. 02/01/2026-01/31/2027

To be paid quarterly in arrears.

2.0 Background: This piece of equipment was purchased new in 2020 and will go off warranty on December 8, 2021. Preventive maintenance ensures continued & reliable operation in support of the NADC research mission.

CONTRACTOR REQUIREMENTS

3.0 Technical Requirements/Tasks:

Service tasks include, but are not limited to:

1) Check seals for cracks and replace when necessary

2) Check bottle strapping and replace when necessary

3) Check bearings and replace when necessary

4) Check and replace batteries annually

5) Check and clean reagent bottles and replace when necessary

6) Perform engineering tests

7) Labor, travel and replacement parts are included in service

8) Unlimited service visits, Monday-Friday, excluding holidays

9) Unlimited telephone support for instrument, reagents and applications

10) On-Site repairs by certified original equipment manufacturer trained personnel

11) All repairs done with OEM parts

12) Maintenance done in accordance with OEM specifications.

13) 48 hour guaranteed response to a request for emergency on-site service

Contractor will provide telephone support and unlimited emergency visits to the customer location.

Repair parts are to be included unless otherwise stated in this statement of work.

4.0 Government Furnished: Government-furnished support is limited to security access to the equipment in the NCAUR facility, replacement parts not otherwise covered under this agreement, AC power (110VAC) and standard office lighting.

6.0 Travel: One (1) preventive maintenance service visit will be provided by the contractor. All service visits will be coordinated with the Processor Operator at

NADC.

7.0 Contractor’s Key Personnel: Only factory trained engineers are to perform any service procedure on the Excelsior AS Tissue Processor.

8.0 Security Requirements: A copy of an official government photo ID (State-issued driver’s license or ID card, passport, green card, etc.) must be provided no later than one week before arrival of contract service technician(s) except in the case of emergency service, were only the name of the technician provided via email or telephone will be necessary. Security access to the NADC facility will be provided based on this identification requirement.

9.0 Place of Performance: USDA, 1920 Dayton Avenue, Ames, IA 50010
