About this file

This is a Statement of Work (SOW) for the on-site relocation of a skid-mounted mobile unit at Thomas E. Creek VA Medical Center in Amarillo, Texas. The contract is firm-fixed price for services and requires the contractor to safely move an existing modular office/clinical support unit from its current location to a new designated pad within VA property, without damage to the unit, utilities, pavement, landscaping, or VA facilities, and with minimal disruption to patient care and traffic operations.

The contractor shall furnish all labor, supervision, tools, rigging, equipment, materials, permits, and traffic control necessary to complete the work. Key tasks include conducting a site assessment and developing a detailed Method Statement/Lift & Move Plan documenting equipment selection, rigging configuration, load weight and center of gravity, travel path, ground bearing capacity, traffic and pedestrian controls, and weather thresholds. All lifting and rigging must comply with ASME B30 standards and OSHA material handling requirements. If the move crosses public roads or rights-of-way, the contractor must obtain Texas DMV oversize/overweight permits and comply with escort requirements. The work encompasses pre-move assessment, temporary utility disconnection coordination with VA, execution of the lift and placement, re-leveling and anchoring, post-move inspection, and restoration of disturbed areas. Deliverables include a pre-move condition report, approved method statement and lift plan, permit documentation, and a post-move report with as-moved drawings and incident-free certification. The base period allows 30 calendar days from Notice to Proceed for completion of one move event including planning, execution, and closeout, with optional CLINs available for additional intra-site moves. The contractor must carry appropriate insurance for work on a Government installation and provide a minimum 12-month workmanship warranty. Government acceptance is contingent upon written approval by the Contracting Officer's Representative (COR) following successful placement, restoration, and completion of any punch-list items.

Statement of Work

Relocation of a Mobile Unit on Skids On-Site Move (504-26-995)

Agency/Location: Department of Veterans Affairs, Thomas E. Creek VA Medical Center, 6010 Amarillo Boulevard West, Amarillo, TX 79106-1991.

Contract Type: Firm-fixed price, service.

1. Requirement Title

On-site relocation of an existing mobile unit mounted on skids from its current location to a new designated location within VA property at the Thomas E. Creek VA Medical Center.

2. Background

The Medical Center requires the safe, controlled movement of a skid-mounted mobile unit (e.g., modular office/clinical support unit) across the campus to a different prepared pad/location.

Work must be completed without damage to the unit, utilities, pavement/landscaping, or VA facilities, and with minimal disruption to patient care and traffic. (Structure and flow modeled on the attached VA mobile trailer SOW.)

3. Scope

The Contractor shall furnish all labor, supervision, tools, rigging, equipment, materials, permits, traffic control, and coordination required to:

• inspect and plan the move;

• temporarily disconnect/secure attachments and utilities (where directed by VA);

• safely lift, transport, and place the skid-mounted unit at the new location;

• re-level, re-anchor/tie-down (if applicable), and verify alignment;

• restore disturbed areas and provide closeout documentation.

Standards & Compliance: All lifting and rigging shall comply with applicable ASME B30 volumes (e.g., B30.5 mobile cranes, B30.9 slings, B30.10 hooks, B30.26 rigging hardware) and OSHA material handling requirements (29 CFR 1910.184; 1926.251; Subpart CC).

Note on public roadway permits: This SOW covers an on-property move. If any portion of the movement uses public roads or crosses a right-of-way, the Contractor shall obtain TxDMV oversize/overweight permits (e.g., House Move / Portable Building single-trip) and comply with designated routing/escort requirements.

4. Definitions

• Mobile unit on skids: A relocatable facility or modular unit set on skid beams intended for movement and re-placement on prepared surfaces. (Relocatable facility concept referenced).

• Lift plan: A stamped or qualified-person plan showing rigging configuration, load weight/center of gravity, crane selection (if used), travel path, ground bearing, and controls.

5. Specific Tasks

A. Pre-Move Assessment and Plan

1. Conduct a site walk to document the current unit condition, dimensions, estimated weight, skid configuration, lifting points, and attachments (utilities, ramps, steps, awnings).

2. Provide a Method Statement / Lift & Move Plan, including: equipment list (e.g., dollies, hydraulic jacks, slide-systems, crane/toter), rigging drawings, ground bearing/soil checks, travel path, traffic/pedestrian controls, weather thresholds, and contingency measures. Standards cited above apply.

3. Identify permit needs (if roadway crossing), escorts, and schedule.

4. Coordinate with VA for utility disconnects/reconnects (VA trades or Contractor if authorized).

B. Preparations

1. Provide temporary protection (pavement, curbs, landscaping, adjacent structures).

2. Install signage and barricades; set a work zone and traffic plan coordinated with facility.

3. Pre-rig the unit (blocking, bracing, spreader bars, shackles, slings, hoist rings) per ASME

B30 and manufacturer guidance.

C. Move Execution

1. Lift/translate the unit using appropriate techniques (jack/roll, skid-slide, crane pick, toter), keeping clearances and load control per the approved plan.

2. Maintain continuous spotter/rigging supervision; halt if conditions exceed plan limits (wind, bearing failure, obstructions).

3. Place the unit at the new location; level, plumb, and anchor/tie-down (as specified). If foundation work is needed, it shall be within scope only if specified by VA; otherwise VA will prepare the pad.

4. Perform post-move inspection (structure, doors/windows, MEP stubs, finishes) and correct minor alignment issues.

D. Restoration and Closeout

1. Remove temporary protections and restore disturbed areas.

2. Deliver as-moved documentation: annotated photos, final placement diagram

(dimensioned), level/anchor verification, and incident-free certification signed by the contractor’s qualified person.

3. Provide warranty for workmanship (minimum 12 months unless otherwise specified).

6. Deliverables & Schedule

• D1: Pre-Move Condition Report (photos, measurements).

• D2: Method Statement / Lift & Move Plan (incl. traffic & safety plan).

• D3: Permits/escorts (if applicable).

• D4: Post-Move Report & As-Moved Drawing.

• Target schedule: Planning submittals within 10 business days of award; move windows to be coordinated with clinical operations (modeled after delivery cadence used in your CT trailer lease SOW).

7. Safety & Security

• Comply with VA campus safety, badging, access control, and any IT/privacy rules if unit contains equipment/data; follow VA directions for safeguarding VA property during work.

(Use the structure and approach in the attached SOW’s security section as a guide.)

• Rigging lift team qualifications and daily equipment inspection logs per ASME/OSHA.

8. Insurance & Liability

• Contractor shall carry insurance suitable for work on a Government installation (e.g., FAR 52.228-5), and customary minimums similar to those routinely specified by VA (workers’ comp, general liability, auto liability). Use the attached VA award’s supplemental insurance minimums as a baseline reference (final values at CO’s discretion).

9. Quality Assurance & Acceptance

Quality Assurance Plan – Government will verify:

• Approved plan on file; qualified personnel present.

• No damage to unit or VA property; proper leveling/plumb and anchorage; clearances met.

• Documentation complete; site restored.

Acceptance: Written acceptance by the COR after successful placement and restoration, with punch-list (if any) closed.

10. Period of Performance

Base: One (1) move event, inclusive of planning, execution, and closeout, completed within 30 calendar days of NTP (exact move dates coordinated with VA). Optional CLINs may be included for additional intra-site moves.
