STATEMENT OF WORK

(SOW)

Kardex Megamat 125

1.0 Background

The Department of Veteran Affairs VISN 1 requires the procurement of a Kardex Megamat 125 filing system and removal of the existing filing system located at the VAMC (Veteran Affairs

Medical Center) West Haven campus.

1.1 Places of Performance is VAMC West Haven located at 950 Campbell Avenue, West

Haven, CT 06516.

1.2 The Period of Performance is from date of contract award through 25 weeks thereafter.

2.0 Scope

The Contractor shall provide the required Kardex Megamat 125 filing system, in accordance with the required specifications included in Attachment 1 of the solicitation. The Contract shall remove the existing filing system located at VAMC West Haven.

3.0 Technical Requirements

3.1 The Contractor shall remove the existing filing system from VAMC West Haven.

Details of the existing equipment as well as dimensions of the facility room are included in Attachment 2 of the solicitation.

3.1.1 The Contractor shall break down the existing filing system equipment and remove it from the premises. The Contractor shall complete the disassembly in a controlled manner.

3.1.2 The Contractor shall remove any and all waste due to the removal of the filing system and discard of the equipment. The Contractor is responsible for transporting the waste and equipment from VAMC West Haven.

3.2 The Contract shall provide all supervision, labor, materials, and equipment to install the required Kardex Megamat 125 filing system in accordance with the Attachment 1 specifications sheet.

3.2.1 Training - Contractor shall provide initial training to government system administrator, upon setup and training materials.

3.2.2 Documentation – The contractor shall provide complete documentation, all manuals, drawings, and schematics of installed equipment. The contractor shall provide all necessary information and software documentation, so that the government can program and repair the system independent of the contractor, after the government inspects, accepts, and takes ownership of the equipment.

3.2.3 The contractor is responsible for cleaning up the area and returning it to the condition it was found. Any damage to the facility will be repaired by the contractor at their expense.
