STATEMENT OF WORK

To Perform Major, Rebuild of Two (2) Powerex Air Compressors Located at Building 010B

Statement of Work for the major rebuilding of Powerex Air compressors 1 and 2

1.0 Introduction: Requirements are to rebuild two (2) Powerex air compressors to within factory standards.

2.0 Background: There are two Powerex air compressors that are in the basement mechanical room in building 010B. The two air compressors are utilized to supply air to critical scientific research equipment and need major overhauls to be brought back to factory standards.

3.0 The Scope: The contractor shall provide all labor and material to bring two (2) Powerex air compressors Model number SE0400740AJ and Serial number H04/15/14-5626794-294 to within factory standards.

The contractor shall provide all labor and material to: Lock and tag out the electrical power source to Powerex air compressor number 1 and verify the absence of power. The contractor shall replace one (1) Powerex model – SLAE05E Scroll pump. The contractor shall perform a complete preventive maintenance on the remaining seven (7) pumps to include replacing (7) Powerex (P/N 92832070) tip seal kits, seven (7) (P/N Powerex IP602000AV) heat insulation pipe, sixteen (16) Powerex (P/N BT012900AV) belts, two (2) Powerex (P/N ST073907AV) air elements, and only utilize Powerex grease three (3) (P/N IP600000AV).

The contractor shall perform all safety checks, remove lock and tag out, and start air compressor number 1 and record all voltage readings. The contractor shall ensure air compressor number 1 is completely online prior to turning off air compressor number 2 to perform the maintenance and repairs. The contractor shall lock and tag out the electrical power source to Powerex air compressor number 2 and verify the absence of power. The contractor shall replace five (5) Powerex model (SLAE05E) Scroll pumps. The contractor shall replace one defective contactor (P/N PE000101AV) that serves pump number 8. The contractor shall perform a complete preventive maintenance on pumps 2,4, and 8 to include replacing three (3) Powerex (P/N 92832070) tip seal kits, three (3) Powerex (P/N IP602000AV) heat insulation pipe, (16) Powerex belts (P/N BT012900AV), (2) Powerex air elements (P/N ST073907AV), and utilize only Powerex grease (1) (P/N IP600000AV).

The contractor shall perform safety checks prior to starting air compressor number 2. The contractor shall start air compressor number 2 and record all voltage readings.

4.0 Codes and Standards: The contractor shall:

A. Follow the National Electrical Code (NEC) for electrical components

B. ASME standards for pressure vessels

C. OSHA regulations regarding compressed air safety

5.0 Pre-start-up Documentation and Qualifications: The contractor shall provide all labor and material to:

B. The contractor shall provide a cost breakdown of all materials and labor listed separately

6.0 DELIVERABLES

The contractor shall provide the following deliverables in the performance of this project task order.

A. Construction Project Schedule:
1. The Contractor will submit a construction project schedule to the Contracting Officer for approval within 5 days after receiving the Notice to Proceed. The Contractor will not begin actual work until the construction project schedule has been approved by the Contracting Officer.

2. The construction project schedule shall clearly show estimated construction time as well as actual construction time as the work progresses. A Gantt-type chart software shall be used.

3. The Contractor shall submit an electronic copy of the updated construction project schedule with each progress payment request.

4. The Contractor shall complete the project within 60 days of receiving the Notice to Proceed.

B. Contract Inspector’s Daily Report:

1. Contractor shall submit to the Contracting Officer a daily report listing the names of all those present on the job site each day, along with their Job Classification, hours of work and a brief description of the work performed by each on that day. The Contract Inspector’s Daily Report is to be submitted at the beginning of work each day for the previous day’s work and shall be maintained throughout the term of the contract.

C. Contractor’s Contact Log (Form SF-1413):

1. The contractor shall provide the Contracting Officer a list of all sub-contractors in a table format listing at least the following (which apply): Company Name, Company Address, Name of Contact person(s), Telephone Numbers, Fax Numbers, and E-mail address. The Contractor shall update the list immediately when subcontractors change or are added.

D. Formal Meetings:

1. Any meeting (deemed formal by the contractor, the COR, or the Contracting Officer) will have the minutes of said meeting prepared by the contractor (and a copy provided to the Contracting Officer). The minutes will include Date, Name of Attendees (with contact info), Old Business, and New Business subjects (or narratives). Regardless of who requests a meeting (the COR or the Contractor or the Contracting Officer) the contractor may be required to provided hard copy pictures of the issues to be discussed. Each issue will be linked to a contract specification section number and/or subject area by page number. Issues discussed will also include an action/completion date.

7.0 SAFETY SUBMITTALS

A. Contractor shall submit a "site specific" Safety Plan to the Contracting Officer that describes how the contractor is to comply with all applicable OSHA standards and NFPA codes pertaining to construction work to include, but not limited to: General Safety and Health Provisions, Occupational Health and Environmental Controls, Personal Protective and Life Saving Equipment, Hand and Power Tools, Welding and Cutting, Electrical, Scaffolds, Fall Protection, Excavations, Demolition, Stairways and Ladders, Toxic and Hazardous Substances, Confined Space Entry, Lockout/Tagout, Respiratory Protection, Hearing Protection. Safety Plan shall identify Contractor and sub-contractor employees who are authorized/qualified to perform electrical work. No work shall proceed until written approval of the Safety Plan by the Contracting Officer. Safety Plan shall describe how the Contractor and/or sub-contractors are to secure equipment, supplies, tools, and chemical products. Contractor and/or sub-contractor equipment, supplies, tools, and chemical products are to be under the control of the Contractor always, equipment, supplies tools, and chemical products found not to be under the control of the Contractor or sub-contractor may be confiscated and delivered to the Contracting Officer.

B. The Contractor shall inspect the entire construction site daily, including days without construction work if there are NFPA 101 Life Safety Code deficiencies existing while construction work is shut down, the results of that inspection shall be documented on the attachment “DAILY INSPECTION FORM”, and the form is to be signed and submitted to the Contracting Officer Representative.
C. Contractor shall provide continuous monitoring for noise, dust, chemical vapor generation, volatile organic compounds, and vibration. At no time, shall noise exposure to any ARS staff exceed 75 db. Contractor shall provide to the Contracting Officer a written weekly update of the test results and for proposed work for the subsequent two weeks (a two week “look-ahead”) involving any construction activity that will produce loud noises, heavy vibrations, strong smells, or significant dust inside ARS staff occupied space.

D. Contractor and sub-contractors shall notify in writing the Contracting Officer Representative at least one week prior to executing high risk work (this may be included in the two week “look-ahead” update). High risk work includes, but is not limited to, tasks requiring Lockout/Tag Out, live electrical work, hot work, work at heights, trenching/ shoring, crane operations, and confined space entry. Contractor’s Competent Person shall provide the Contracting Officer Representative written weekly documented safety inspection results.

E. Contractor shall provide to the Contracting Officer Representative for approval, copies of Material Safety Data Sheets (MSDS) for EVERY CHEMICAL WITHOUT EXCEPTION prior to bringing such chemical on the work site.

8.0 CONSTRUCTION SECURITY REQUIREMENTS

A. Security Plan:

1.The security plan defines both physical and administrative security procedures that will remain effective for the entire duration of the project.
2.The General Contractor is responsible for assuring that all sub-contractors working on the project and their employees also comply with these regulations.

B. Contractor Personnel

1.Upon receipt of the Notice to Proceed, the Contractor shall provide information about every employee including that of all sub-contractors who will need access to construction site.
2.Each employee shall be furnished with a badge by the Government for access to construction site. This badge must be worn to be clearly visible always while on the work site.

C. Security Procedures:

1.Contractor’s employees shall not enter the project site without appropriate badges. The badges shall be obtained from building 426A prior to reporting to the job site. Each vehicle onsite shall have a vehicle tag for the rear-view mirror during construction.
2.For working outside the “regular hours” as defined in the contract, The Contractor shall give a ten (10) day notice to the Contracting Officer so that security arrangements can be provided for the employees. This notice is separate from any notices required for utility shutdown described later in this section.

3. No photography of ARS premises is allowed without written permission of the Contracting Officer.

4. ARS reserves the right to close or shut down the project site and order the Contractor’s employees off the premises in the event of a national emergency or Government shutdown. The Contractor may return to the site only with the written approval of the Contracting Officer.

D. Key Control:

1. The Contractor shall provide duplicate keys and lock combinations to the Contracting Officer for security inspections of every area of project including toolboxes and parked machines and take any emergency action.

E. Document Control:

1. The Contractor is responsible for safekeeping of all drawings, project manuals and other project information. This information shall be shared only with those with a specific need to accomplish the project.

2. These documents shall not be removed or transmitted from the project site without the written approval of Contracting Officer.

3. All paper waste or electronic media such as CD’s and diskettes shall be shredded and destroyed in a manner acceptable to the ARS.

F. Motor Vehicle Restrictions

1. Vehicle authorization request shall be required for any vehicle entering the site and such request shall be submitted 24 hours before the date and time of access. Access shall be restricted to picking up and dropping off materials and supplies and Contractor must be on site to receive all deliveries.

2. Separate permits shall be issued for Contractor and its employees for parking in designated areas only. The badging shall be obtained at building 426A.

9.0 FIRE SAFETY

B. Safety Plan: Establish and maintain a fire protection program in accordance with 29 CFR 1926. Prior to start of work, prepare a plan detailing project-specific fire safety measures, including periodic status reports, and submit to the Contracting Officer for review for compliance with contract requirements. Prior to any worker for the contractor or subcontractors beginning work, they shall undergo a safety briefing provided by the contractor’s competent person per OSHA requirements. This briefing shall include information on the construction limits, ARS safety guidelines, means of egress, break areas, work hours, locations of restrooms, use of ARS equipment, etc. Documentation shall be provided to the Contracting Officer that individuals have undergone contractor’s safety briefing.

C. Site and Building Access: Maintain free and unobstructed access to facility emergency services and for fire, police and other emergency response forces in accordance with NFPA 241.

D. Separate temporary facilities, such as trailers, storage sheds, and dumpsters, from existing buildings and new construction by distances in accordance with NFPA 241. For small facilities with less than 20 feet exposing overall length, separate by 10 feet.

E. Temporary Heating and Electrical: Install, use and maintain installations in accordance with 29 CFR 1926, NFPA 241 and NFPA 70.

F. Means of Egress: Do not block exiting for occupied buildings, including paths from exits to roads. Minimize disruptions and coordinate with Contracting Officer.

G.Egress Routes for Construction Workers: Maintain free and unobstructed egress. Inspect daily. Report findings and corrective actions weekly to the Contracting Officer.
H.Fire Extinguishers: Provide and maintain extinguishers in construction areas and temporary storage areas in accordance with 29 CFR 1926, NFPA 241 and NFPA 10.

I. Flammable and Combustible Liquids: Store, dispense and use liquids in accordance with 29 CFR 1926, NFPA 241 and NFPA 30.

J. Existing Fire Protection: Do not impair automatic sprinklers, smoke and heat detection, and fire alarm systems, except for portions immediately under construction, and temporarily for connections. Provide fire watch for impairments more than 4 hours in a 24-hour period. Request interruptions in accordance with Article, OPERATIONS AND STORAGE AREAS, and coordinate with the Contracting Officer. All existing or temporary fire protection systems (fire alarms, sprinklers) located in construction areas shall be tested as coordinated with the Contracting Officer. Parameters for the testing and results of any tests performed shall be recorded by the General Contractor and copies provided to the Contracting Officer.

K. Hot Work: Perform and safeguard hot work operations in accordance with NFPA 241 and NFPA 51B. Coordinate with the Contracting Officer. Designate contractor's responsible project-site fire prevention program manager to coordinate permit hot work.

L. Fire Hazard Prevention and Safety Inspections: Inspect entire construction area weekly. Coordinate with, and report findings and corrective actions weekly to the Contracting Officer.

M. Smoking: Smoking is prohibited in and adjacent to construction areas inside existing buildings and additions under construction. In separate and detached buildings under construction, smoking is prohibited except in designated smoking rest areas.

N. Dispose of waste and debris in accordance with NFPA 241. Remove from site daily.

O. Perform other construction, alteration, and demolition operations in accordance with 29 CFR 1926.

10.0 OPERATIONS AND STORAGE AREAS

A. The Contractor shall confine all operations (including storage of materials) on Government premises to areas authorized or approved by the Contracting Officer. The Contractor shall hold and save the Government, its officers, and agents, free and harmless from liability of any nature occasioned by the Contractor's performance.

B. Temporary buildings (e.g., storage sheds, shops, offices) and utilities may be erected by the Contractor only with the approval of the Contracting Officer and shall be built with labor and materials furnished by the Contractor without expense to the Government. The temporary buildings and utilities shall remain the property of the Contractor and shall be removed by the Contractor at its expense upon completion of the work. With the written consent of the Contracting Officer, the buildings and utilities may be abandoned and need not be removed.

C. The Contractor shall, under regulations prescribed by the Contracting Officer, use only established roadways, or use temporary roadways constructed by the Contractor when and as authorized by the Contracting Officer. When materials are transported in prosecuting the work, vehicles shall not be loaded beyond the loading capacity recommended by the manufacturer of the vehicle or prescribed by any Federal, State, or local law or regulation. When it is necessary to cross curbs or sidewalks, the Contractor shall protect them from damage. The Contractor shall repair or pay for the repair of any damaged curbs, sidewalks, or roads.

D. Working space and space available for storing materials shall be as determined by the Contracting Officer.

E. Workers are subject to rules of the Location applicable to their conduct which shall be provided by the Contracting Officer.

F. Execute work to interfere as little as possible with normal functioning of Location including operations of utility services, fire protection systems and any existing equipment, and with work being done by others. Use of equipment and tools that transmit vibrations and noises through the building structure, are not permitted in buildings that are occupied during construction except as permitted by the Contracting Officer where required by limited working space.

1. Do not store materials and equipment in other than assigned areas.

2. Schedule delivery of materials and equipment to immediate construction working areas within buildings in use by the ARS in quantities sufficient for not more than two workdays.

3. Where access by ARS personnel to vacated portions of buildings is not required, storage of Contractor's materials and equipment may be permitted subject to fire and safety requirements.

G. Contractor shall take all measures and provide all material necessary for protecting existing equipment and property in affected areas of construction against dust and debris, so that equipment and affected areas to be used in the ARS operations will not be hindered. Contractor shall permit access to ARS personnel through construction areas which serve as routes of access to such affected areas and equipment. Coordinate alteration work in areas occupied by ARS so that operations will continue during the construction period.

H. Utilities Services: Maintain existing utility services for ARS always. Provide temporary facilities, labor, materials, equipment, connections, and utilities to assure uninterrupted services. Where necessary to cut existing water, steam, gases, sewer or air pipes, or conduits, wires, cables, etc. of utility services or of fire protection systems and communications systems (including telephone), they shall be cut and capped at suitable places where shown; or, in absence of such indication, where directed by the Contracting Officer.

1. No utility service such as water, gas, steam, sewers or electricity, or fire protection systems and communications systems may be interrupted without prior written approval of the Contracting Officer. Electrical work shall be accomplished with all affected circuits or equipment de-energized. When an electrical outage cannot be accomplished, work on any energized circuits or equipment shall not commence without the Contracting Officer’s prior knowledge and written approval.

2. Contractor shall submit a request to interrupt any such services to the Contracting Officer, in writing, Two weeks advanced notice and again 48 hours in advance of proposed interruption. Request shall state reason, date, exact time of, and approximate duration of such interruption.

3. Contractor will be advised (in writing) of approval of request, or of which other date and/or time such interruption will cause least inconvenience to operations of ARS. Interruption time approved by ARS may occur at other than Contractor's normal working hours.

4. Major interruptions of any system must be requested, in writing, at least 21 calendar days prior to the desired time and shall be performed as directed by the Contracting Officer. It is the Contractor's responsibility to develop a work plan and schedule detailing, at a minimum, the procedures to be employed, the equipment and materials to be used, the mitigating life safety measures to be used during the work, and a schedule defining the duration of the work with milestone subtasks.

5. In case of a contract construction emergency, service will be interrupted on approval of the Contracting Officer. Such approval will be confirmed in writing as soon as practical.

6. Whenever it is required that a connection fee be paid to a public utility provider for new permanent service to the construction project, for such items as water, sewer, electricity, gas or steam, payment of such fee shall be the responsibility of the Government and not the Contractor.

I. To minimize interference of construction activities with flow of ARS traffic, comply with the following:

1. Keep roads, walks and entrances to grounds, to parking and to occupied areas of buildings clear of construction materials, debris and standing construction equipment and vehicles.

2. Method and scheduling of required cutting, altering and removal of existing roads; walks and entrances must be approved by the Contracting Officer.

J. Coordinate the work for this contract with other construction operations as directed by Contracting Officer. This includes the scheduling of traffic and the use of roadways, as specified in Article, USE OF ROADWAYS.

11.0 ALTERATIONS

A. Survey: Before any work is started, the Contractor shall make a thorough survey with the Contracting Officer Representative, of areas of the building in which alterations occur and areas which are anticipated routes of access, and furnish a report, signed, to the COR.

1. Shall note any discrepancies between drawings and existing conditions at site.

2. Shall designate areas for working space, materials storage, and routes of access to areas within buildings where alterations occur, and which have been agreed upon by Contractor and Contracting Officer.

B. Any items required by drawings to be either reused or relocated or both, found during this survey to be nonexistent, or in opinion of COR, to be in such condition that their use is impossible or impractical, shall be furnished and/or replaced by Contractor with new items in accordance with specifications which will be furnished by Government. Provided the contract work is changed because of this subparagraph B, the contract will be modified accordingly, under provisions of clause entitled "DIFFERING SITE CONDITIONS" (FAR 52.2362).

C. Re-Survey: Thirty days before expected partial or final inspection date, the Contractor and COR together shall make a thorough resurvey of the areas of building.

1. Resurvey report shall list any damage caused by Contractor and will form basis for determining extent of repair work required of Contractor to restore damage caused by Contractor's workers in executing work of this contract.

D. Protection: Provide the following protective measures:

1. Temporary protection against damage for portions of existing structures and grounds where work is to be done, materials handled, and equipment moved and/or relocated.

E. Final Cleanup:

1. Upon completion of project, or as work progresses, remove all construction debris from the site.

12.0 PROTECTION OF EXISTING VEGETATION, STRUCTURES, EQUIPMENT, UTILITIES, AND IMPROVEMENTS A. THE CONTRACTOR SHALL PRESERVE AND PROTECT ALL STRUCTURES, EQUIPMENT, AND VEGETATION (SUCH AS TREES, SHRUBS, AND GRASS) ON OR ADJACENT TO THE WORK SITE, WHICH IS NOT TO BE REMOVED AND WHICH DOES NOT UNREASONABLY INTERFERE WITH THE WORK REQUIRED UNDER THIS CONTRACT. THE CONTRACTOR SHALL ONLY REMOVE TREES WHEN SPECIFICALLY AUTHORIZED TO DO SO AND SHALL AVOID DAMAGING VEGETATION THAT WILL REMAIN IN PLACE. IF ANY LIMBS OR BRANCHES OF TREES ARE BROKEN DURING CONTRACT PERFORMANCE, OR BY THE CARELESS OPERATION OF EQUIPMENT, OR BY WORKERS, THE CONTRACTOR SHALL TRIM THOSE LIMBS OR BRANCHES WITH A CLEAN CUT AND PAINT THE CUT WITH A TREEPRUNING COMPOUND AS DIRECTED BY THE CONTRACTING OFFICER.

B. The Contractor shall protect from damage all existing improvements and utilities at or near the work site and on adjacent property of a third party, the locations of which are made known to or should be known by the Contractor. The Contractor shall repair any damage to those facilities, including those that are the property of a third party, resulting from failure to comply with the requirements of this contract or failure to exercise reasonable care in performing the work. If the Contractor fails or refuses to repair the damage promptly, the Contracting Officer may have the necessary work performed and charge the cost to the Contractor.

13.0 RESTORATION

A. Remove, cut, alter, replace, patch and repair existing work as necessary to install new work. Except as otherwise shown or specified, do not cut, alter or remove any structural work, and do not disturb any ducts, plumbing, steam, gas, or electric work without approval of the Contracting Officer. Existing work to be altered or extended and that is found to be defective in any way, shall be reported to the Contracting Officer before it is disturbed. Materials and workmanship used in restoring work shall conform in type and quality to that of original existing construction, except as otherwise shown or specified.

B. Upon completion of contract, deliver work complete and undamaged. Existing work (mechanical and electrical work, lawns, paving, roads, walks, etc.) disturbed or removed because of performing required new work, shall be patched, repaired, reinstalled, or replaced with new work, and refinished and left in as good condition as existed before commencing work.

C. At Contractor's own expense, Contractor shall immediately restore to service and repair any damage caused by Contractor's workers to existing piping and conduits, wires, cables, etc., of utility services or of fire protection systems and communications systems (including telephone) which are indicated on drawings, and which are not scheduled for discontinuance or abandonment.

D. Expense of repairs to such utilities and systems not shown on drawings or locations of which are unknown will be covered by adjustment to contract time and price in accordance with clause entitled "CHANGES" (FAR 52.2434) and "DIFFERING SITE CONDITIONS" (FAR 52.2362).

14.0 LAYOUT OF WORK

A. The Contractor shall lay out the work from Government established base lines and benchmarks, indicated by the contract documents, and shall be responsible for all measurements in connection with the layout. The Contractor shall furnish, at Contractor's own expense, all stakes, templates, platforms, equipment, tools, materials, and labor required to lay out any part of the work. The Contractor shall be responsible for executing the work to the lines and grades that may be established or indicated by the Contracting Officer. The Contractor shall also be responsible for maintaining and preserving all stakes and other marks established by the Contracting Officer until authorized to remove them. If such marks are destroyed by the Contractor or through Contractor's negligence before their removal is authorized, the Contracting Officer may replace them and deduct the expense of the replacement from any amounts due or to become due to the Contractor.

B. Establish and plainly mark lines and grades that are reasonably necessary to properly assure that location, orientation, and elevations established are in accordance with lines and elevations shown on contract drawings.

15.0 USE OF ROADWAYS

A. FOR HAULING, USE ONLY ESTABLISHED PUBLIC ROADS AND ROADS ON ARS PROPERTY AND, WHEN AUTHORIZED BY THE CONTRACTING OFFICER, SUCH TEMPORARY ROADS WHICH ARE NECESSARY IN THE PERFORMANCE OF CONTRACT WORK. TEMPORARY ROADS SHALL BE CONSTRUCTED BY THE CONTRACTOR AT CONTRACTOR'S EXPENSE. WHEN NECESSARY TO CROSS CURBING, SIDEWALKS, OR SIMILAR CONSTRUCTION, THEY MUST BE PROTECTED BY WELL CONSTRUCTED BRIDGES.

16.0 AVAILABILITY AND USE OF UTILITY SERVICES

A. The Government shall make all reasonably required amounts of utilities available to the Contractor from existing outlets and supplies, as specified in the contract. Should existing services be inadequate for the Contractor’s purposes without interfering with ARS operations, the Contractor shall provide and pay for their own services. The amount to be paid by the Contractor for chargeable electrical services shall be the prevailing rates charged to the Government. The Contractor shall carefully conserve any utilities furnished without charge.

B. The Contractor, at Contractor's expense and in a workmanlike manner satisfactory to the Contracting Officer, shall install and maintain all necessary temporary connections and distribution lines, and all meters required to measure the amount of electricity used for the purpose of determining charges. Before final acceptance of the work by the Government, the Contractor shall remove all the temporary connections, distribution lines, meters, and associated paraphernalia.

C. Contractor shall install meters at Contractor's expense and furnish the ARS a monthly record of the Contractor's usage of electricity as hereinafter specified.

D. Heat: Furnish temporary heat necessary to prevent injury to work and materials through dampness and cold. Use of open salamanders or any temporary heating devices which may be fire hazards or may smoke and damage finished work, is prohibited. Maintain minimum temperatures as specified for various materials and equipment as recommended by the manufacturer of the materials and equipment.

E. Electricity (for Construction and Testing): Furnish all temporary electric services.

1.Obtain electricity by connecting to the ARS electrical distribution system. The Contractor shall meter and pay for electricity required for electric cranes and hoisting devices, electrical welding devices and any electrical heating devices providing temporary heat. Electricity for all other uses is available at no cost to the Contractor.

F. Water (for Construction and Testing): Furnish temporary water service.

1. Obtain water by connecting to the ARS water distribution system. Provide reduced pressure backflow preventer at each connection. Water is available at no cost to the contractor.

2. Maintain connections, pipe, fittings and fixtures and conserve wateruse so none is wasted. Failure to stop leakage or other wastes will be cause for revocation (at Contracting Officer’s discretion) of use of water from ARS system.

17.0 HISTORIC PRESERVATION

A. Where the Contractor or any of the Contractor's employees, prior to, or during the construction work, are advised of, or discover any possible archeological, historical and/or cultural resources, the Contractor shall immediately notify the Contracting Officer verbally, and then with a written follow up.

INSTRUCTIONS: This form is to be utilized when hazards are posed by NFPA 101 deficiencies or construction activities are in progress. Remediation must be implemented upon project start and continuously enforced through project completion to provide a level of life safety comparable to that described in the Life Safety Code. Submit completed forms to the CO & COR weekly.

29. Are utility services (e.g., electrical, steam, water, waste, gas) properly secured at the end of each day?

30. If there is any hot work (welding, soldering, cutting) being performed within the construction site, have additional fire safety precautions been taken and necessary equipment provided?

31. If there is any hot work (welding, soldering, cutting) being performed on the construction site, has Contracting Officer been notified? Has the Hot Work Permit been approved?
