U.S. AIR FORCE (USAF)/NAME METAL NAME TAGS
Statement of Work
21 Jan 2021
1) SCOPE OF WORK: The Government requires finished metal USAF name tags in order to meet all of the agency’s requirements, In Accordance With (IAW) The Institute of Heraldry (TIOH) specifications for approximately 38,000 trainees or prior service personnel per year at JBSA Lackland, TX. The metal name tags are designed for wear on the Air Force Service Dress Uniform, and each tag shall state the last name of the trainee or prior service personnel. Each trainee or prior service personnel will receive one (1) each United States Air Force (USAF) Service Coat Metal Name Tag. All orders shall be delivered IAW delivery specifications in paragraph 4 of the Statement of Work (SOW). The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items necessary to make metal USAF name tags. Name tags shall be made to fully comply with the standards set forth by TIOH and IAW the description denoted in Paragraph two (2) of this document. All orders shall be placed by fax, email or telephone and delivered to:

2) DESCRIPTION: The name tag will be made of Brass or Nickel Silver (Name Plate) with Plastic (Attaching Insert). The Dimensions will be Height (Overall) ¾ inch. Width (Overall) 3 inches, Thickness (Name Plate) 0.025 inch (+/- 0.005 inch) and Thickness (Overall) 0.105 inch (+/- 0.005 inch). The Finish shall be Nickel Plated, Silver Plated, and Matte Finish with a brushed silver finish.

Plastic (Attaching Insert) DIMENSIONS: Height (Overall): 3/4-inch
Width (Overall): 3 inches
Thickness (Name Plate): 0.025 inch plus or minus 0.005 inch
Thickness (Overall): 0.105 inch plus or minus 0.005 inch FINISH: Nickel plated, silver plated, matte with a brushed silver finish
NOTES:
1. Attaching device shall be prong and clutch type. Location shall be as follows:
a. Two prongs shall be located on the horizontal center line 9/16 inch from each end of the plastic edge and be 3/8 inch in length.
2. Lettering shall be standard block or SL513 Portuguese font engraved in all capital letters and is ¼ inch in height. When a lower case letter is specified in the order, the letter shall be 1/8 inch in height. The lines forming the letter shall use no less than 0.030 inch or more than 0.040 cutting head.
a. An epoxy-based paint shall be applied to the engraved letters. The paint shall match Pantone Color 2747C.
3. The name tag is to be packed in a protective plastic tube encased in a poly-bag.
a. The poly-bag shall have a minimum wall thickness of 2 mils.
b. The protective tube shall be clear plastic, not less than 0.015 inch in thickness, and not less than ¾ inch in diameter.
4. The finished name tag shall be lacquered using a pale, clear, synthetic lacquer. The use of a pigmented lacquer shall not be permitted.
3) PERFORMANCE: The Government will provide rosters with trainee or prior service personnel’s last name to the contractor weekly as needed via fax, e-mail and/or telephone. The regular AF roster will contain approximately 15 – 70 names per roster/flight and there will be an average of 12 - 20 flights per week. The prior service personnel roster will contain 1 – 50 names.
4) DELIVERY: All orders shall be placed by fax, email or telephone and delivered to 502 LRS/LGRM, 1550 Wurstmith Avenue, Suite 4, Bldg. 5725, JBSA Lackland, TX 78236-5250. Delivery for regular Air Force (AF) basic trainees shall be within 14 calendar days of receipt of order to the address listed in paragraph one (1). Delivery of the prior service personnel metal name tag will be within three (3) business days of receipt of order. The contractor shall clearly mark each bundle of Metal Name Tags with the appropriate roster/flight number as provided by the order. Re-orders/corrections shall be delivered to the government within three (3) business days from notification.
5) REJECTED NAME TAGS: Errors/defects attributed to the contractor will be corrected and delivered to the government within 3 business days at no cost to the government.
Defective or misspelled name tags will not be returned to the contractor unless the contractor request they are returned, and pay for the cost of returning them. Defective or misspelled name tags will be re-accomplished and delivered to the government within three (3) business days from notification of the defect/misspelling.
Incorrect name tags resulting from government-caused errors shall be corrected by the contractor and delivered to the government within three (3) business days following notification of the error.
6) DEFINITIONS:
DEFECTS: A commercial defect of this product is defined as those defects promoting an adverse appearance of the product. Material defects include, but are not limited to, any nicks, cuts, blemishes, spots, stains, fraying, or work defects.
PRIOR SERVICE PERSONNEL: Individuals who are returning or recalled to active duty.
FLIGHT: A group of basic trainees or prior service personnel consisting of 15 to 70 males and/or females.
