About this file

This is a Statement of Work (SOW) for the disposal of solid waste from Camp James A. Garfield Joint Military Training Center (CJAG) in Portage County, Ohio, prepared by the Ohio Army National Guard Environmental Office and dated August 18, 2026.

The OHARNG estimates approximately 1,980 tons of non-hazardous solid waste, consisting of vegetation, soil, brick, metal, terracotta tile, glass, and wood, requires disposal at a licensed and permitted facility. The OHARNG will load and transport the waste via dump truck. Key requirements include: the licensed disposal facility must be within 125 miles via public road of 8451 State Route 5, Ravenna, Ohio 44266; the contractor or facility must provide support in completing waste acceptance profiles since OHARNG may lack website/portal access; copies of manifests must be provided prior to waste transportation; and the final bid must include all costs and fees for disposing the 1,980-ton baseline volume, with a per-ton rate for any overage. The contractor must notify OHARNG immediately when the 1,980-ton threshold is met. Within 30 days of completion, the contractor must supply signed manifests and scale receipts for each load. Tipping fees per Ohio Revised Code 3734.024-3734.026 apply since waste originates from Portage County. The SOW will not be considered complete until all required forms and data are submitted and accepted by OHARNG. The point of contact is Mr. Bradley Kline (614-336-4918 or bradley.w.kline2.nfg@army.mil). Analytical data confirming non-hazardous waste classification is attached, along with asbestos testing results showing minimal chrysotile asbestos content across samples.

SOW for the Disposal of Solid Waste from CJAG Page 2

1.0 Introduction

This statement of work (SOW) describes the scope of services requested to accept and properly dispose of solid waste from Camp James A. Garfield Joint Military Training Center (CJAG) located in Portage County Ohio. The services provided by the winning contractor or licensed disposal facility must comply with Federal, State and local regulations including but not limited to Ohio Administrative Code 3745 and Ohio Revised Code 3734.

2.0 Location

The Ohio Army National Guard (OHARNG) will be transporting the waste from the below listed address:

Camp James A. Garfield Joint Military Training Center 8451 State Route 5 Ravenna, Ohio 44266

Generator name and mailing address:

Camp James A. Garfield Joint Military Training Center 1438 State Route 534 SW Newton Falls, Ohio 44444

Camp James A. Garfield Joint Military Training Center is in both Trumbull and Portage Counties.

Please note the waste generated for this SOW is from Portage County. Therefore, tipping fees in accordance with ORC 3734.024-3734.026 do apply for this waste stream.

3.0 Qualifications

Waste generated from this activity must be disposed of at a properly licensed facility in accordance with OAC 3745-37-01. The OHARNG reserves the right to request proof of knowledge or permits and to disqualify any contractor who cannot demonstrate this to the satisfaction of the OHARNG.

4.0 Scope of Work

The OHARNG estimates that approximately 1,980 tons of solid waste is located at CJAG and will be disposed of at a licensed and permitted facility. Visual inspections show the solid waste consists of vegetation, soil, brick, metal, terracotta tile, glass and wood. Analytical data confirms that the material is non-hazardous solid waste, see Attachment A. Please note that the OHARNG will be loading and transporting the waste via dump truck to the licensed disposal facility.

Upon an approved profile the licensed disposal facility provides the generator, OHARNG, access to enter the facility to offload the accepted solid waste. The licensed disposal facility has the right to refuse or reject any load that does not match the approved profile.

SOW to Transport and Dispose of Construction and Demolition Debris at CRJMTC Page 3

5.0 Requirements

• The licensed disposal facility must be within 125 miles via public road of 8451 State Route 5 Ravenna, Ohio 44266.

• The OHARNG may not be able to access the licensed disposal facility website, software, portal, etc and therefore the contractor or licensed disposal facility will provide support in completing the profile for acceptance.

• The contractor or licensed disposal facility must provide copies of manifests to the OHARNG prior to the solid waste being transported.

• The final bid is to include all costs and fees associated with the disposal of 1,980 tons of solid waste. The licensed disposal facility will be required to notify the OHARNG immediately if/when the 1,980 ton threshold is met. If the total amount disposed is greater than 1,980 tons the contractor can be paid on a per ton basis for the amount over the 1,980 ton threshold. Contractor must include a per ton rate within their bid for any amount over 1,980 tons.

• In accordance with ORC 3734.024-3734.-026 tipping fees based on the waste being generated within Portage County apply.

• Within 30 days of all waste being transported by the OHARNG to the licensed disposal facility contractor will supply copies of the signed manifest and scale receipts for each load.

• The scope of work will not be considered completed until all required forms and data are submitted and accepted by the OHARNG.

Case Narrative

Project: Waste Sampling

Job ID: 240-256376-1 Eurofins Cleveland Job Narrative 240-256376-1

The analytical test results presented in this report meet all requirements of the associated regulatory program listed on the Accreditation/Certification Summary Page, unless otherwise noted. Data qualifiers and/or narrative comments are included to explain any exceptions, if applicable. Regulated compliance samples (e.g. SDWA, NPDES) must comply with associated agency requirements/permits.

Matrix-specific batch OC (e.g., MS, MSD, SD) may not be reported when insufficient sample volume is available or when site-specific QC samples are not submitted. In such cases, a Laboratory Control Sample Duplicate (LCSD) may be analyzed to provide precision data for the batch.

For samples analyzed using surrogate and/or isotope dilution analytes, any recoveries falling outside of established acceptance criteria are re-prepared and/or re-analyzed to confirm results, unless the deviation is due to sample dilution or otherwise explained in the case narrative.

Receipt The samples were received on 7/23/2026 4:18 PM. Unless otherwise noted below, the samples arrived in good condition, and, where required, properly preserved and on ice. The temperature of the cooler at receipt time was 6.3°C.

Receipt Exceptions The followinq samples were received at the laboratory outside the required temperature criteria: - .. -. ._. -

26-007 EAST PILE A (240-256376-2), 26-008 EAST PILE B (240-256376-3), 26-009 SOUTH PILE A (240-256376-4) and 26-Oil 0 SOUTH PILE B (240-256376-5). The samples are considered acceptable since it was collected and submitted to the laboratory on the same day and there is evidence that the chilling process has begun.

The test report shall not be reproduced except in full, without written approval of the laboratory. The report must not be used by the client to claim product certification, approval, or endorsement by NVLAP, NIST, or any agency of the federal government. The Company reserves the right to dispose of all samples after a period of thirty (30) days, according to all state and federal guidelines, unless otherwise specified.

All components not quantified as asbestos content and non-asbestos content are considered to be non-fibrous matrix components. Matrix components may include, but are not limited to, gypsum, paint, silicate minerals, vinyl, binder, calcium carbonate, tar, and foam.

Inhomogeneous samples are separated into homogeneous subsamples and analyzed individually. ND means no fibers were detected. When detected, the minimum detection and reporting limit is less than 1% unless point counting is performed. Floor tile samples may contain large amounts of interference material and it is recommended that the sample be analyzed by gravimetric point Count analysis to lower the detection limit and to aid in asbestos identification.

t A "Version" indicated by -"x" after the Lab ID# with a value greater than 1 indicates a sample with amended data. The revision number is reflected by the value of "x".
