About this file

This is a vendor questionnaire spreadsheet accompanying Sources Sought Notice #HHS-NIH-NCI-SS-CSSB-0007 from the National Cancer Institute (NCI) seeking information about maintenance and repair services for Illumina scientific equipment. The questionnaire requests company details including POC information, UEI number, socioeconomic status, and current government contract experience.

The questionnaire focuses on vendors' capabilities regarding equipment maintenance, including: experience with scientific/medical equipment maintenance, pricing catalogs, reagent replacement policies, OEM authorization status, access to OEM parts and procedures, response time guarantees, technician qualifications, remote service capabilities, and ability to service multiple Illumina instrument types (including iScan, NextSeq, ISeq, MiniSeq, MiSeq, and NovaSeq systems). It also inquires about volume discount availability for multiple instrument maintenance contracts. The NAICS code is 811210 with a $34.0M size standard, and responses will help determine if a small business set-aside is appropriate.
