About this file
This is a Performance Work Statement (PWS) and Quality Assurance Surveillance Plan (QASP) for an outcome-based reagent supply contract supporting the Abbott i-STAT analyzer at Pine Ridge Indian Hospital Laboratory (PRSU).
The contract objective is to ensure continuous, uninterrupted availability of fully compliant reagent supplies required to operate the Abbott i-STAT analyzer in support of 24/7 clinical diagnostic services. The contractor must achieve mission-critical outcomes including: continuous supply availability with zero disruption to testing capability; 100% product usability upon receipt with full compliance to cold-chain, expiration, and manufacturer requirements; delivery performance aligned with laboratory demand without overstocking or shortages; and regulatory and manufacturer compliance. The scope is non-personal services, with the contractor responsible for sourcing, quality assurance, cold-chain management, and delivery of all required reagents, calibrators, and controls. Estimated annual usage includes approximately 450 Troponin tests, 1,400 ABG tests, and 400 Chem8 tests. Deliverables include test reagents, calibrators, and multi-level quality control materials specified by catalog number. The contract period is 12 months from award, with delivery to Pine Ridge Indian Hospital Laboratory in Pine Ridge, South Dakota.
Performance metrics establish zero tolerance for stockout incidents and temperature excursions, ≥95% on-time delivery, ≥98% product acceptance rate, 100% shelf-life compliance, and 100% lot consistency. The Government will conduct outcome-based surveillance through inspection at receipt, inventory tracking, deficiency reporting, and trend analysis. Non-performance remedies include mandatory replacement at contractor expense, shipment rejection, negative CPARS ratings, increased surveillance, and potential termination for cause. Critical failures requiring immediate action include any stockout impacting patient care, cold-chain breaches, and repeated delivery failures. Technical evaluation uses a Lowest-Priced Technically Acceptable (LPTA) methodology across three factors: technical capability (ability to obtain Abbott i-STAT supplies, supply chain capability, cold-chain management, product quality, and delivery performance); past performance on similar contracts; and price (unit pricing, extended pricing, and shipping costs). Quotations must be technically acceptable to receive consideration, with no tradeoffs conducted under this LPTA evaluation.
