About this file

This document is a Sole Source Justification Form submitted by the Illinois Department of Public Health (IDPH) for a suicide assessment training program. IDPH is seeking to provide Assessing and Managing Suicide Risk (AMSR) trainings online to health and behavioral healthcare professionals in Illinois. The trainings will cover core competencies for outpatient, inpatient, and substance use disorder treatment settings to help providers assess, manage, and treat suicide risk. The total value of the initial term is $173,908, with the contract running from January 14, 2022 to February 28, 2022. IDPH is sole sourcing this training to Education Development Center, the owner of the AMSR curriculum, as it meets the specific training needs outlined in IDPH's federal grant requirements and is not available from any other provider.

The pricing for the trainings was determined to be fair and reasonable, as offering them virtually allows IDPH to reach more participants at the same cost while reducing travel expenses. This sole source purchase is funded through federal supplemental funds from the U.S. Centers for Disease Control and Prevention to address COVID-19 injury and violence prevention activities. Not approving this sole source would jeopardize IDPH's ability to fulfill the federal grant requirements and put future funding at risk.

Section V - Purchase History

Has the Agency or University Purchased these supplies or services in the past? Yes No

Section VI - Business Rationale

1. Provide a detailed explanation of the need for the supplies or services:

IDPH Violence and Injury Prevention Section received federal supplemental funding from U.S. Centers for Disease Control and Prevention to initiate, expand, or enhance approved activities to address COVID-19 injury and violence prevention activities. There is a great concern that the lack of mental health and risk of suicide will be one of the negative impacts of COVI-19. Health and behavioral health providers play a crucial role in preventing suicides. Studies have shown that a substantial proportion of people who have died by suicide had either been in treatment or had some recent contact with a mental health professional. Yet many providers report that they feel inadequately trained to assess, treat, and manage suicidal clients.

It will be imperative those at risk are identified and referred for professional help. Most behavioral health professionals have not been trained in assessing or treating a person at risk of suicide. If efforts are made to refer individuals, then it is imperative the professional they are referred to are trained to respond appropriately. For this reason, it is essential to train professionals in the core competencies to provide the safer care needed.

2. Provide a list and describe in detail the specifications required to satisfy the need:

The approved budget included purchasing the following Assessing and Measuring Suicide Risk (AMSR) trainings to be offered on-line to clinicians in Illinois:

a. AMSR-Outpatient - Assessing and Managing Suicide Risk: Core Competencies for Health and Behavioral Health Professionals Working in Outpatient Settings. The curriculum develops skills in the recognition, assessment, and management of suicide risk and the delivery of effective suicide-specific interventions. The audience is behavioral health clinicians working in outpatient settings who conduct suicide risk assessments, primarily those with a master’s degree or above. Outpatient providers can help patients with elevated suicide risk remain in their community setting and recover, by using the suicide risk identification, assessment, and management competencies taught in the AMSR-Outpatient training.

b. AMSR-Inpatient - Assessing and Managing Suicide Risk: Core Competencies for Behavioral Health Professionals Working in Inpatient Settings. The curriculum develops skills in the recognition, assessment, and management of suicide risk and the delivery of effective suicide-specific interventions. Behavioral health care professionals working in inpatient settings can help patients with elevated suicide risk by using the suicide risk identification, assessment, and management competencies taught in the AMSR-Inpatient training.

Delivering best practices in suicide care to patients with elevated suicide risk during inpatient treatment can offset the high-risk period they may experience post-discharge. The audience are behavioral health clinicians working in inpatient settings who conduct suicide risk and nursing assessments, primarily those with a master’s degree or above, including medical providers and registered nurses.

c. AMSR-Substance Use Disorder - Assessing and Managing Suicide Risk for Substance Use Disorder Treatment Professionals. The curriculum develops skills in the recognition, assessment, and management of suicide risk and the delivery of effective suicide-specific interventions. The training aims to provide participants with the background knowledge and practical skills that they need to address suicidal risk and behaviors in clients in care for substance use disorder treatment. Participants will have the opportunity to increase their knowledge and apply practical skills. The audience are professionals who offer any level of substance use treatment to individuals at risk for suicide. A Train the Trainer session for up to 25 professionals also will be offered.

3. Provide detail explaining the justification selected in Section IV to explain why the requested supplies or services are the only ones available that can satisfy the agency or university requirements?

Assessing and Managing Suicide Risk (AMSR) fills the training gap by teaching the core competencies that meet suicide care standards established by national organizations including The Joint Commission, the National Action Alliance for Suicide Prevention, the Substance Abuse and Mental Health Services Administration, the Veterans Administration, and others.

AMRS is owned by the Education Development Center (EDC). The curricula were developed under EDC's direction by a panel of researcher/practitioners in public health, psychology, and suicide prevention. As owner of the trainings, EDC has the rights to engage sponsor organizations and hire trainers to conduct the trainings based upon the curriculum.

Requesting Agency/University Illinois Department of Public Health (IDPH)

4. What are the unique features of the supplies or services that are not available in any other product or by any other vendor? Provide specific quantifiable factors/qualifications:

AMSR meets providers’ need for research-informed, skills-based training by presenting five of the most common dilemmas faced by providers and the best practices for addressing them. We provide a framework to determine patients’ risk and how to make relevant and meaningful treatment decisions. Teaching and skill-building methods include:

» Expert teaching » Video demonstrations » Group discussion » Documentation practice » Reflection

5. Has the Agency or University considered alternative supplies or services to satisfy their need? Yes No

5a. If no, why weren't alternatives evaluated?

The approved budget and work plan designated the purchasing of the AMSR trainings to be offered on-line to clinicians in Illinois. The funding required the trainings by offered in an on line format.

6. Are there resellers or distributors?

Yes No N/A

7. What efforts were made to get the best possible price (i.e. did the agency/university negotiate) and how did you determine the price for this purchase is considered fair and reasonable?

By offering the trainings virtually, the number of participants was increased leading to more clinicians being trained for the same prices.

Virtual trainings also cut travel costs.

8. Will this purchase obligate the State to this vendor for future purchases such as maintenance, licensing or continuing need?

Yes No

9. What will be the financial or other impact to the State if this sole source is not approved and a competitive bid is required?

The impact of not approving this sole source would mean the program does not fulfill the requirement in their federal grant agreement and there will be a loss of funding. It also puts the program in jeopardy of being eligible for future funding.

10. Is there any additional information you would like to add to justify this sole source?
