About this file
This is a Justification and Approval (J&A) document for a sole source contract modification for the Defense Health Agency (DHA). The document supports a new one-year contract with SwordHealth, Inc. for a Virtual Physical Therapy (PT) Pilot program, valued at an undisclosed amount and funded through Operation and Maintenance (O&M) funds. The contract period runs from 30 September 2025 to 29 September 2026.
DHA requires a telehealth physical therapy solution that can be delivered through the Amwell platform, with specific requirements including compatibility with DHA and Military Health System (MHS) Genesis systems, and the ability to operate in deployed or low-communication environments. Market research identified seven potential virtual PT solution providers, but only SwordHealth meets all integration and operational requirements. The contract aims to validate the efficacy of virtual PT services and address gaps in high-demand readiness-related PT services for servicemembers. A notice was published on the Government Point of Entry (GPE) for 15 days, with no responses received.

Without these services, DHA will experience an inability to validate the efficacy of Virtual PT services for future transformation and stabilization of Military Health Systems (MHS) operations, and overall, there will be an inability to close the gap in current availability of high-demand readiness- related PT services for servicemembers.
6. Efforts to Obtain Competition: The government conducted extensive Market Research in which they reviewed the capabilities offered by several virtual PT solutions to obtain competition. However, due to the Information Technology delivery mechanism, only one potential source offers a virtual PT solution that can be delivered through DHA’s telehealth platform. Additionally, a notice was publicized on the Government Point of Entry (GPE) on 20 August 2025 for a total of 15 days as required by FAR Subpart 5.2.
7. Actions to Increase Competition/Remove or Overcome any Barriers to Competition: The Contracting Office will continue to assess the market to determine whether other virtual PT solutions can meet the specifications required to integrate with DHA’s telehealth platform, Amwell.
8. Market Research: Market Research was conducted from 01 January 2025 to 15 August 2025 in accordance with FAR Part 10. The Requiring Activity reviewed mandatory sources, reviewed catalogs and product literature, and contractors’ pricelist under the General Services Administration site to foster competition amongst all potential sources.
Market Research indicated there are seven potential sources who offers a virtual PT solution in the commercial sector. Six of the potential sources are large businesses while one potential source is listed as a small business. Of the seven potential source, only one contractor, Sword, offers out-of-the-box integration and is compatible with the DHA systems Amwell, and the MHS Genesis. Sword meets the required specifications required for use in operational military healthcare delivery scenarios of interest to the virtual PT pilot program including but not limited to fully offline capability for use in deployed/low communication environments such as deployed carriers and austere, downrange environments.
