About this file
This document is a Request for Sole Source Authorization (SSA) submitted by the Idaho Department of Health and Welfare (IDHW) to the Idaho Division of Purchasing. IDHW is seeking to renew a contract for the Parenting with Love and Limits (PLL) System of Care Enterprise License with Savannah Family Institute, Inc. (SFI), the sole provider and owner of the PLL evidence-based treatment model. The PLL program provides group and family therapy, training, materials, and outcome data dashboards for youth aged 10-18 exhibiting emotional or behavioral problems. IDHW has utilized PLL services for 15 years and now seeks to expand the program statewide as part of its new Idaho Behavioral Health Plan, potentially doubling or tripling the number of families served over the next 2-3 years.
The SSA outlines the unique benefits of the PLL program, including higher family engagement, flexible and effective training methods, outcome data tracking, and shorter service durations. IDHW has documented significant cost savings compared to other mental health and rehabilitative services, as well as improvements in youth emotional/behavioral issues and family functioning. The total cost of the 5-year enterprise license is $1,500,000. IDHW has determined that SFI is the sole provider of the PLL model and that there are no other suppliers that can meet the agency's business needs.

NEW REQUESTS (complete #1 5)
1. Describe the Product or Service.
Savannah Family Institute (SFI) is the sole provider, program developer, and owner of the “Parenting with Love and Limits (PLL)” evidence based treatment model. The PLL model is an evidence based system of care that includes certification training and licensure of providers/staff, allowing them to utilize the model and receive ongoing model fidelity consultation and support from PLL and the Division of Behavioral Health (DBH) PLL Center of Excellence (CoE) team. The PLL system of care includes training agencies and clinicians to achieve better outcomes with children and families in areas of trauma, mental illness, and delinquency through the evidence based model. PLL treatment integrates group therapy and family therapy, within an intensive and brief outpatient structure. Also included in PLL’s system of care structure are updated books and training materials, a PLL dashboard system for clinicians and

Inclusionary criteria for the program include youth aged ten (10) to eighteen (18), exhibiting emotional or behavioral problems, probation violators and repeat offenders, and/or victims of abuse or neglect.
2. Describe your Agency’s Business Need for the requested product or service.
SFI (dba PLL) has previously been granted an SSA, which has been renewed several times since 2008 when DBH first began contracting with SFI for use of PLL services. With the current changes in DBH and restructuring of our mental health delivery system, we are now seeking a new contract which includes a new implementation model with SFI.
Over the past fifteen (15) years, we have been able to collaborate and build a positive working relationship with PLL.
Because of this partnership, we have worked with the model developer and program director to create an innovative new structure of an enterprise license which will allow us to build a larger provider network in Idaho, allowing many more families to receive PLL services in the future. We also now have an internal team in Idaho, our PLL Center of Excellence (CoE) team, who will assist provider agencies with program implementation, provide trainings and consultation, and in essence build an Idaho PLL Program in collaboration and partnership with SFI/PLL.
Historically, IDHW employees have provided the PLL treatment directly to families in our communities statewide.
PLL has provided the trainings, materials, and support. In our new structure, we will be entering into an enterprise license model wherein our DHW PLL CoE staff will provide the trainings and ongoing consultation support to provider agencies under the new Idaho Behavioral Health Plan. The newly structured enterprise license and contract will cover the following areas: the training of PLL CoE program specialists to become PLL certified trainers and consultants, therapist licenses for up to thirty (30) therapists, all books and training materials for providers and families served in the program, utilization of the PLL dashboard for all providers and PLL CoE staff, and annual research reports for each provider agency.
IDHW’s Children’s Mental Health (CMH) program serves children/youth that have been diagnosed with a severe emotional disturbance (SED). DBH has implemented a system of care for Idaho’s children and youth through the Youth Empowerment Services (YES). In YES, three (3) key principles of care are to 1) provide services that are family centered and increase family engagement; 2) ensure that an array of community based interventions are available and provided in the least restrictive setting; and 3) provide services that are outcome based to ensure that we achieve the intended goals and outcomes. The PLL program specifically achieves our goals in these three (3) areas.
PLL has been offered through our CMH program for fifteen (15) years and has proven to be one of the most effective treatment programs that we offer. Under a new SSA and new contract with PLL, we plan to expand PLL services as a part of our new Idaho Behavioral Health Plan (IBHP) with the contracted Managed Care Organization (MCO).
Through this model enterprise license and contract, we will be able to assist provider agencies so that they have access to the PLL system of care, including access to therapist licenses, ongoing training, support, and materials. We will be able to double and triple the number of families served with PLL as projected in the next two to three ( 2 3) years as the program grows.
To demonstrate the effectiveness of PLL, the program has yielded fourteen (14) outcome studies that evidence the benefit to consumers and IDHW.
The PLL Research Evaluation – 2021 2022 Outcomes included the following summary:
This report presents an evaluation of the fourteenth year of the Parenting with Love and Limits® (PLL) program implementation delivered through the Idaho Department of Health and Welfare (DHW), Children’s Mental Health (CMH) Division in Regions 1 7.
In its fourteenth year of services in Idaho, PLL served 95 new youth and families. (This number is lower than our average annual number, likely due to continued barriers caused by the Covid 19 pandemic. Our numbers were very typical to other PLL sites across the country for the 21 22 year). The total number of families served, since the beginning of the PLL program in 2008, in Idaho is 2198. Key findings include:
Request for Sole Source Authorization_updated 7/2015 Page 3
Family Engagement: PLL continued the engagement of parents/caregivers in their treatment of youth. 85% of the youth and families who started PLL, completed the program this year.
Length of Stay: In comparison to other CMH and Community Based Rehabilitative Services (CBRS) that typically last 12 months and 24 months on average respectively, clients served through PLL programming completed treatment in 90 days on average.
Cost of Care: PLL services have been found to be substantially less than other CMH and CBRS interventions. A comparison of the costs of care for the 188 new youth and families served by PLL in 2016 17 reveals a cost savings of $225,224 compared to other CMH services and a cost savings of $481,280 compared to CBRS programming. 2016 2017 was the latest year that these numbers were analyzed in this way. Because of the change in landscape in our Idaho Behavioral Health system over the past several years, we are looking at new ways to evaluate these cost comparisons.
Emotional/Behavioral Problems: PLL significantly reduced severe emotional and behavioral problems among youth served as measured by the Child Behavior Checklist (CBCL). Statistically significant improvement is seen in youth mental and behavioral problems with a medium effect size, meaning the improvements are readily noticeable.
Family Functioning: Improvements are seen on seven of the eight scales, with five being statistically significant:
Balanced Flexibility, Disengaged Scale, Chaotic Scale, Family Communication, and Family Satisfaction. In addition, the Youth assessment showed a statistically significant increase in family satisfaction, in addition to increases in the disengaged and family communications scale. Given the challenges with engaging youth, these results are excellent.
3. Justification: Check each box that applies and attach required documentation.
X Sole source provider of a licensed or patented good or service (attach provider’s letter) Sole manufacturer AND distributor (attach manufacturer’s letter) Sole provider of factory authorized/warranty service (attach provider’s letter) Sole provider of items that are compatible with existing equipment, systems, programs, etc.
Other (attach description and justification)
4. Describe what makes this Product or Service unique.
The PLL system of care is unique in that it is the only treatment model found to include all of the following elements:
group therapy, family therapy based on Structural and Strategic family therapy theory (with two [2] options for treatment – family systems stabilization, and family systems trauma, or a hybrid combination of both), treatment within an intensive and brief outpatient structure, a PLL dashboard system which includes outcome data in real time, on going model fidelity training and support, updated copyrighted books and training materials, online access to materials, support documents, etc., and annual research and evaluation reports. Four (4) key benefits of the program that we have found to be important and effective in our fifteen (15) year partnership with PLL include 1) higher parent/family engagement and program completion rates; 2) a manualized model with flexible and effective training and delivery methods that assist the clinician in providing effective treatment; 3) dashboard and fidelity assurance measures; and 4) smaller caseloads and shorter service duration.
5. Provide a list of other suppliers contacted and describe the method for determining that there are no other possible suppliers.
As stated previously, Savannah Family Institute is the sole provider, program developer, and owner of the “Parenting with Love and Limits” evidence based treatment model. “Parenting with Love and Limits” is a registered service mark and trademark of SFI. The PLL materials are proprietary to, and copyrighted by, SFI. SFI is the only entity authorized to issue PLL licenses and to provide the requisite PLL certification training, consultation, and materials to its partners. (See attached letter from the provider.)
There are no other suppliers of this product. There are different treatment interventions that exist. However, PLL is the only treatment model that uses this comprehensive system of care, as identified above and they are the only supplier. We have developed a unique partnership which has allowed us to create an innovate structure for program delivery in Idaho, and substantially increasing services to Idaho families.
