About this file

This document is a Request for Sole Source Authorization (SSA) from the Idaho Department of Health and Welfare (DHW) to the Idaho Division of Purchasing (DOP) to obtain Crisis Prevention Institute (CPI) Nonviolent Crisis Intervention, Trainer Certification Training. The training is required for the seven adult Idaho Crisis Centers and will be required for mobile crisis response teams as part of the Division of Behavioral Health's Center of Excellence. The training focuses on prevention, de-escalation techniques, and alternatives to the use of restraint. The desired term for the sole source contract is from 5/1/2024 to 4/30/2029, with a cost estimate of $500,000. The agency has determined that CPI is the sole provider of this proprietary, evidence-based, and nationally recognized best practice training.

The agency has conducted market research and determined that no other suppliers offer a comparable person-centered crisis intervention program with a focus on de-escalation and minimizing physical intervention. The training will be provided to all three state hospitals, as well as be offered to the provider network through Optum, ensuring continuity across the state's behavioral health services.

1. Describe the Product or Service.

Proprietary Nonviolent Crisis Intervention Training from the Crisis Prevention Institute, Inc. (CPI) provides staff with the resources to respond to crisis. Training includes de-escalation communication, proactive verbal de-escalation strategies, and safety intervention and disengagement skills. Staff are taught improved decision making, how to minimize physical intervention, confidence in recognizing risk behavior, how to respond to defensive behaviors, how to better understand behavior, and how to address the needs of the individual. The CPI proprietary framework includes a four (4) step process to turn crisis into an opportunity for positive outcomes. CPI utilizes a train-the-trainer method allowing certified trainers to train additional staff. CPI Nonviolent Crisis Intervention includes both online learning and in-classroom training designed to enable staff with the skill-set necessary to recognize risk level and apply the appropriate interventions for de-escalation and alternatives to the use of restraints.

2. Describe your Agency’s Business Need for the requested product or service.

Nonviolent Crisis Intervention Training from the Crisis Prevention Institute, Inc. (CPI) is the required training for the seven (7) adult Idaho Crisis Centers and will be the required training for mobile crisis response teams as a core function of the Division of Behavioral Health's Center of Excellence. IDAPA 16.03.14 relating to facility standards for hospitals requires training in de-escalation techniques, use of least restrictive interventions, application of safe restraints, monitoring patients in restraint or seclusion, and providing care for a patient in restraint or seclusion. All three (3) State Hospitals (West, South, and North) have staff who are currently certified CPI trainers and would like to maintain current CPI certifications and in addition to certifying additional staff trainers. Each State Hospital has a history of success in utilizing CPI training. In addition, Optum has been offering CPI Nonviolent Crisis Intervention training to the provider network for many years, providing continuity between the provider network and Department supported services.

3. Justification: Check each box that applies and attach required documentation.

X Sole source provider of a licensed or patented good or service (attach provider’s letter) Sole manufacturer AND distributor (attach manufacturer’s letter) Sole provider of factory-authorized/warranty service (attach provider’s letter) Sole provider of items that are compatible with existing equipment, systems, programs, etc.

Other (attach description and justification)

4. Describe what makes this Product or Service unique.

CPI Nonviolent Crisis Intervention training is an evidence-based training that includes person-centered training and is considered a best practice in crisis intervention. According to SAMHSA Best Practice Toolkit (2020), CPI Nonviolent Crisis Intervention is a nationally recognized best practice in crisis intervention and highly effective in minimizing the need for physical interventions. CPI is unique in the fact that "the training focuses on prevention and de-escalation techniques and other alternatives to the use of restraint, as well as information on evaluating risk of harm;

monitoring for signs of distress; documenting incidents; and safe, less-restrictive physical interventions to be used only as a last resort". CPI has received both copyrights and trademarks for their proprietary training materials.

5. Provide a list of other suppliers contacted and describe the method for determining that there are no other possible suppliers.

Initial research generated the following list of four possible suppliers in addition to Crisis Prevention Institute.

These possible suppliers were evaluated based on three criteria:

1. Utilizing a person-centered approach;

2. Focus on de-escalation techniques; and

3. Minimizing the need for physical intervention.

Crisis Intervention Team (CIT) International: Focuses on providing community-wide and state-wide solutions to reduce stigma and further involvement with the criminal justice system for individuals with mental health conditions or substance use disorders. CIT focus is too broad to suit the needs of the State Hospitals and Crisis Teams. CIT does not offer a person-centered approach, de-escalation training, or training to minimize the use of restraints.

MOAB Training International: Focuses on teaching participants how to protect themselves from injury along with controlling and managing aggressive behavior. Teaches the use of a wide variety of restraints. MOAB does not offer a person-centered approach, de-escalation training, or training to minimize the use of restraints.

The Mandt System: Focuses on de-escalation techniques and crisis prevention specifically in the workplace or a school setting. The de-escalation training and crisis prevention training are two distinct programs. The State Hospitals and Crisis Teams will routinely be interacting with new individuals and will need de-escalation and crisis prevention techniques meant for individuals with whom a familiar relationship may not have been established. The Mandt System does not offer a person-centered approach, nor training to minimize the use of restraints or physical intervention.

Right Response: Focuses on de-escalation techniques and conflict prevention. Right Response does not focus on minimizing the need for physical intervention and focuses mainly on the "impact of self" in conflict and crisis response.

Using the criteria outline above, none of the four potential suppliers are adequate suppliers.

After researching the above possible suppliers, a secondary search was done to determine if any other suppliers not already researched offer person-centered crisis intervention programs. Search results returned only the Crisis Prevention Institute. None of the above listed four suppliers are an adequate supplier.

7. Justification: Check each box that applies and attach required documentation.
