Statement of Work Summary: Audio Booth Relocation, Fort Eustis, Virginia

This Statement of Work (SOW) details the relocation of two existing audiometric booths from Building 664 (Room 144) to Building 576 (Room 278) at McDonald Army Health Center, Fort Eustis, Virginia. The Contractor must provide all labor, materials, certifications, and licensed supervision for complete decommissioning, transport via freight elevator, and reassembly within five consecutive business days. Key technical requirements include conducting pre-move condition assessments and joint inventory with the Government; safely disassembling and tracking all hardware (fasteners, brackets, structural components); navigating freight elevators with dimensions of 92" height x 66" width x 87" depth and door opening of 80" height x 50" width; and performing structural reassembly in strict accordance with original manufacturer guidelines. The Contractor bears liability for any damage to panels or facilities during transit and must ensure zero interference with active patient care and clinical operations.

Quality assurance and compliance requirements are substantial. Following reassembly, the Contractor must verify all electrical circuits, lighting, ventilation/exhaust systems, and audio pathways; perform octave-band ambient noise surveys confirming compliance with ANSI/ASA S3.1 and OSHA 29 CFR 1910.95 Appendix D standards for audiometric testing environments; submit formal test results within three business days; and ensure booths are perfectly level with properly aligned doors and continuous airtight acoustic seals. All work must occur during standard duty hours (Monday-Friday, 07:30 AM to 05:30 PM) with no weekend or after-hours work without 48-hour advance approval. The Contractor must maintain compliance with military base regulations, obtain short-term visitor passes for personnel, restrict movement to designated work sites, and participate in joint post-installation walkthroughs with the Government Acceptor. Final invoice authorization requires submission of a completed Inspection/Acceptance Checklist signed by the Government Acceptor, confirming resolution of all punch list items and acoustic certifications. Project Managers are Mr. Hunter Linker and Mr. William Goins; Contracting Office representatives are Mr. Chester Terrill and Ms. Wendy MacDonald.

Prior to the commencement of any disassembly or decommissioning work, the Contractor shall conduct a comprehensive visual and functional inspection of both audiometric booths.

The Contractor shall document all pre-existing wear, physical damage, or operational deficiencies

The Contractor shall provide all labor, packing materials, rigging equipment, and logistics management to relocate the existing booths from B664 (Room 144) to B576 (Room 278) (as depicted in Figures 2 and 3).

All fasteners, brackets, and structural hardware removed during disassembly shall be and tracked to ensure no loss of original components.

The Contractor shall utilize two (2) designated freight elevators in B576 for vertical transit. The Contractor must execute all logistics and staging to ensure zero interference with active patient care, clinical staff, or emergency pathways.

: The Contractor is solely responsible for verifying and navigating the physical constraints of B576’s freight elevators, as detailed below:

e. The Contractor shall maneuver all structural components to ensure they safely clear the elevator and hallway

The Contractor shall reassemble both audiometric booths in B576 (Room 278) in strict accordance with the original manufacturer’s installation guidelines.

The Contractor shall ensure both booths are perfectly level.

All doors must align properly with the frame. Magnetic and compression acoustic seals must form a continuous, airtight, and acoustically insulated seal when the doors are closed.

The Contractor shall successfully reconnect all internal electrical circuits, lighting fixtures, and integrated ventilation/exhaust systems, verifying proper air exchange rates and power distribution inside the reassembles units.

Immediately following reassembly, the Contractor shall perform an end-to-end functional check of all electrical feeds, light switches, ventilation fans, and pass-through audio jack pathways.

The Contractor shall perform an ambient noise level survey inside each reassembled booth to verify that interior noise levels comply strictly with ANSI/ASA S3.1 and OSHA permissible ambient noise standards for audiometric testing environments.

The Contractor shall compile all testing data and submit a formal written Compliance to the Project Manager within three (3) business days of successful test completion.

a. : The entire relocation project that encompasses disassembly, transit, reassembly, and acoustic testing, shall be completed in no more than f ( ) consecutive business days from the designated start day.

b. Scope Adjustments: Any deviation from this schedule must be submitted in writing and pre-approved by the Contracting Officer’s Representative (COR).

: complete decommissioning, disassembly, and hardware of both audiometric booths located in B664, room 144 ll panels be prepped, protected, and staged for transit, with all hardware safely .

T ( ) business days total are allocated for the physical transit, rigging, and structural assembly of the booths in B576, room 278.

for relocation and structural reassembly , ensuring the physical footprint is established and leveled by Day .

: One (1) business day is allocated for final systems integration and acoustic of both booths.

ctave-band ambient noise surveys shall take no more than four (4) hours per audio both, ensuring all physical site work is completed by the close of business on Day .

The Contractor shall make every effort to execute all phases of work quickly, safely, and efficiently as possible.

The Contractor must phase all noisy or high-traffic operations (such as panel rigging and heavy transport) to ensure minimal disruption to building occupants, staff workflow, and patient care pathways.

All on-site contractor operations, deliveries, and testing shall be conducted strictly during normal duty hours: Monday through Friday from 07:30am to 0 :30pm.

No weekend, holiday, or after-hours work is authorized unless explicitly requested and approved 48 hours in advance by the project manager.

The site of the work is on a military reservation, and all rules and regulations issued by the Commanding Officer, Fort Eustis covering safety and sanitary requirements, shall be observed by the contractor.

: For short-duration access (including site visits, meetings, and equipment/material deliveries) the Contractor shall request a short-term visitor pass at the Visitor Control Center (VCC) for the specific duration required (not to exceed 30 days).

: When requesting access, the Contractor shall designate the Project Manager as their official Government Sponsor, providing their name, office, and telephone number.

: The 733d Security Forces Squadron (733 SFS/S5/VCC) personnel will conduct a background and criminal history check on all personnel to evaluate and determine fitness for base access. If approved, 733 SFS will issue a short-term base pass.

: A criminal history check will be conducted by security personnel each time a contractor requests a new or renewed pass. This screening process is expected to take approximately 3 to 5 minutes per individual, excluding VCC queue wait times.

: Contractor personnel shall restrict their movement solely to the immediate work sites (B664 Room 144, B576 Room 278, and the direct transportation routes between them). Under no circumstances shall personnel enter any other buildings, offices, or facilities not explicitly involved in the execution of this contract.

: All employees of the Contractor and its Subcontractors shall remain subject to all rules, laws, and military regulations of Joint Base Langley-Eustis.
