I. THE PURPOSE OF THIS AMENDMENT IS TO UPDATE THE WAGE DETERMINATION AND ANSWER SUBMITTED

QUESTIONS.

A. The DoL Wage Determination (1996-0223) listed on page 3 of Attachment 2 has been updated to “Revision 45, dated 4/2/2019.”

B. The following answers are provided in response to submitted questions:

1. HOW OFTEN DOES EACH FACILITY REQUIRE A PICKUP? Varies depending on the each installation. The COR will try to schedule the pickups when it is convenient to the Generator and Contractor. Per 7.0 b., “Standard removal and performance timeframe for all sites is 21 calendar days from the date of order issuance unless otherwise specified.”

2. FOR EACH PICK UP, CAN WE HAVE A MINIMUM DOLLAR INVOICE AMOUNT? See 13.1.1, FAR 52.216-19 Order Limitations, paragraph (a). The minimum order amount is $5,000.00.

3. FOR THE LAB PACK ITEMS, WILL THERE BE AN INVENTORY SUPPLIED PRIOR TO OR AT THE TIME OF ORDER? Yes, the generator will provide the inventory through the COR.

4. IS THERE A LIST OF MILITARY ITEMS THAT ARE INCLUDED IN FL99? There is not an exhaustive list; all items will fit into CLINs on the schedule.

5. ARE ALL LOCATIONS ACCESSIBLE BY SEMI-TRUCK? If a location is not accessible by semi-truck, the generator will move the waste to an area at ground level so the Contractor can load it onto a semi-truck.

6. IS THERE AN APPROVED FACILITY AND TRANSPORTER LIST AVAILABLE FOR THIS CONTRACT? See 5.0 a. The QFL and QTL is located on the DLA Disposition Services Website at: http://www.dla.mil/DispositionServices/Offers/Disposal/HazardousWaste/ HazWasteDisposal.aspx

7. FOR THE ITEMS SHIPPING BY THE POUND, IS THERE A PLACE FOR MINIMUM INVOICE PER CONTAINER? There is not a per container minimum; the only minimum is in the Order Limitations clause (see Q&A 2.).

II. NO OTHER CHANGES ARE AUTHORIZED.
