Question RFP Page RFP Para Government Response
1 Paragraph 5 indicates there are approximately 1500 employees on medical surveillance but Attachment 5 indicates the amount of Annual Medical Surveillance Exams is 1000.
Please clarify.
1 5 Attachment 5 is only an estimated amount based on historical data.
Approximately 1000 medical examinations have been conducted each year. Para 5.1.3.A “As part of medical surveillance, a medical examination shall be performed every 1 to 3 years as medically indicated. . .” Historically, medical surveillance examinations, pre-employment/job change medical examinations and deployment medical examinations were roughly 1000 per year.
2 If there are exams for another DoD agency, does the number of pre-placement exams on Attachment 5 include those examinations?
1 5 Yes. As stated above, Attachment 5 estimates are based on historical data. Therefore, any and all exams conducted over the years were included in those estimates. .
3 If Corky Hull Medical Associates, Inc. (CHMA) provides a “means of emergency diagnosis and treatment” of an occupational injury/illness at a facility located within a 25 mile driving distance during non-operating hours and the employee chooses this “contracted medical provider” for treatment – It doesn’t make sense that the follow-up appointments would be conducted at the DLA Distribution San Joaquin clinic.
For example: An employee is seen at an Urgent Care center (which is a means of diagnosis and treatment) and they choose to treat with the Urgent Care center – then the Urgent Care center would be their treating provider, not CHMA. Please clarify
22 1 It makes sense when trying to cut ‘lost time or lost production’ for an employee to utilize the on depot clinic. Federal Employees Compensation Act (FECA) provides that medical care for an occupational injury/illness shall be provided by a US Government medical facility, or a duly qualified physician contracted by the Government. DLA San Joaquin is a 24 hour operation. Instead of requiring a contractor to staff a 24 hour medical facility on depot, the contractor may use a medical affiliate within 25 miles of the depot to conduct the initial evaluation of an injured employee. However, in an effort to keep the lost time/lost production to a minimum and if the employee chooses the contracted facility, the follow-up appointments shall be at the depot clinic. This saves time and results in significant cost avoidance for the Government. Besides, if an employee chooses a non-affiliated urgent care to treat their injury, they can’t seek follow up treatment at the clinic.
4 If CHMA clinic staff calls *911 for an ambulance and the employee subsequently declines transport, will there be any “dry run” fees charged to CHMA?
23 2 No. By dialing *911 contact is made with the depot dispatchers – not outside emergency responders. The Government – not the contract medical staff - is contacting emergency response. This protocol has nothing to do with the contracted medical facility. Any and all emergency services are between the patient and emergency responders.
5 The contractor is to provide the optometric examinations, but who is responsible for procuring/purchasing prescription safety glasses?
23 5 The Government. If the contractor was responsible for providing prescription eyewear, it would clearly delineated as a requirement in the
SOW.
6 Historically, how many Fitness For Duty exams are there per year? 24 3 The number of FFD exams is highly unpredictable and impossible to estimate with any certainty/accuracy. For example, in one five (5) year period, no FFD exams were conducted. In the following two-year period, we conducted three (3) FFD exams. However, not all three resulted in a requirement for a psychological evaluation.
7 Historically, how many motor vehicle operator exams are there per year and is there any expectation that there will be a significant increase in this number?
25 2 There are currently 36 motor vehicle operator employees at San Joaquin. However, it is important to note that not all of them maintain commercial licenses. There is no anticipated change at this time.
8 How many Firefighters are currently employed at DLA Distribution San Joaquin and is there any expectation that there will be a significant increase in this number?
25 4 There are currently 40 firefighters. There is no anticipated increase at this time. In fact, the closing of the Sharpe facility in September 2013 may result in a yet-to-be-determined reduction in the number of firefighters.
9 How many Security Force Officers are currently employed at DLA Distribution San Joaquin and is there any expectation that there will be a significant increase in this number?
25 5 There are currently 78 security force officers. There is no anticipated increase at this time. In fact, the closing of the Sharpe facility in September 2013 may result in a yet-to-be-determined reduction in the number of security force officers.
10 The SOW indicates there are currently no government personnel on the lead monitoring program. Does the government plan to add any personnel to that program? If so, approximately how many would there be?
25 4 There are no current plans to add personnel to the program at this time.
If there were, the intent would have been clearly stated in the SOW.
11 The SOW indicates there are currently no government personnel on the pesticide monitoring program. Does the government plan to add any personnel to that program? If so, approximately how many would there be?
25 5 There are no current plans to add personnel to the program at this time.
If there were, the intent would have been clearly stated in the SOW.
12 The SOW indicates there are currently no government personnel on the asbestos removal program. Does the government plan to add any personnel to that program? If so, approximately how many would there be?
26 2 There are no current plans to add personnel to the program at this time.
If there were, the intent would have been clearly stated in the SOW.
13 Regarding audiological evaluation in the case of a STS:
Is the audiological evaluation to be covered under the Firm Fixed Price contract? If so, approximately how many audiological evaluations are anticipated per year?
28 1 Zero.
14 Is the Bloodborne Pathogens Exposure Control Plan IAW 29 CFR
1910.1030 in regards to evaluating and managing potential exposures/exposures of only government employees? Or is the government requesting a copy of CHMA’s internal Exposure Control Plan also?
28 2 The requirement is to write and provide an Exposure Control Plan specifically for the Tracy Depot Clinic, to include personnel functions, procedures in administering medical care, and procedures for handling potential and actual exposures to bloodborne pathogens for Government personnel even during non-routine clinic hours. In other words, the contractor must detail how they will evaluate and treat our employees in the event of a potential or actual exposure to bloodborne pathogen, and be in compliance with 29 CFR 1910.1030.
15 The SOW states “The contractor shall provide a written report to the COR within 24 hours of the medical evaluation.”
According to 29 CFR 1910.1030, the clinician has 15 days to provide a written report. Although it probably won’t be an issue to provide a
28 2 No, written reports are required within 24 hours as stated in the SOW.
written report within 24 hours, can this be changed to 15 days to be in alignment with 29 CFR 1910.1030?
16 If an employee has a verified exposure and post-exposure prophylaxis is indicated, would the cost of the recommended drug regimen(s) need to be covered under the firm-fixed price proposed for this contract?
28 2 No. If the source individual agrees to be tested and is positive/known to be infected with a bloodborne pathogen, the exposed employee is covered under the Office of Worker’s Compensation Programs. The evaluation/testing is NOT compensable under OWCP, and therefore must be considered in the firm-fixed price of the contracted medical services.
17 Regarding: “…immunization program for government personnel at DLA Distribution San Joaquin on a voluntary basis, exposed to a risk of disease.”
Please clarify that this applies to personnel that are ‘occupationally’ exposed?
28 3 No, because that would be inaccurate. Specifically, not all immunizations for depot employees are ‘occupational’, per se. For instance, as stated in paragraph 5.1.5.C., the influenza vaccine is offered and administered in substantial numbers to government employees every year.
18 “Hearing Protection will be provided by the Government.”
However, Attachment 4, Line 19 lists Ear Plugs as an example of contractor supplied items. Please clarify.
28 1 The Government provides all fitted ear plugs and ear plugs for the employees throughout the depot for everyday use. However, as the industrial hygienist conducts their evaluations, training classes, briefings etc. they often provide on-the-spot hearing protection on a one-time basis to employees/managers. They also provide examples of recommended hearing protection for managers to purchase. They use the hearing protection almost daily in their work. Additionally, IH provide disposable hearing protection for isolated events such unscheduled noise sources. Hence, the estimated amount of hearing protection historically provided by a contractor for use by the industrial hygienist in attachment 4. That amount doesn’t come close to the amount required for 1500 plus employees’ day-to-day use provided by the Government.
19 Varicella is a 2-dose vaccination – so does the annual amount listed include 20 employees (at 2 doses each), or 20 total doses?
Attachment
Line 13 20 Employees or 40 total doses.
20 Is labor category synonymous with labor positions? If not, please explain what is meant by “labor category”.
