Defense Distribution Depot Susquehanna PA (DDSP), New Cumberland PA 17070.
Statement of Work:
Raymond Hybrid Model 89 and Model 589 Inspection
The Contractor shall inspect thirteen (13) Model 859 and fourteen (14) Raymond Model 589 Hybrid Lift’s masts for cracks and other weld deficiencies. The entire inspection procedure shall be supervised and directed by an experienced professional engineer. The inspections shall be performed by an ASNT (The American Society for Nondestructive Testing) certified inspector(s) by means of nondestructive tests. The prospective contractor shall be responsible for any and all equipment and materials to be used in performing the inspections. The prospective contractor shall submit four (4) copies of full documentation concerning the inspections one-hundred-twenty (120) days of the start date. The award winning contractor shall commence inspections no latter than thirty (30) days from the award date. The Government reserves the right to make the report public for any potential competitive solicitation to perform repairs.

1. An experienced Professional Engineer to supervise and direct inspection procedures.

3. All personal protective equipment, safety equipment, and licenses required by the government.
4. High lift and fuel to perform the inspections.
5. Administrative personnel to document the inspections.
Technical Capability
Contractors are required to submit technical and past performance information with their price quote that clearly indicates the contractor is qualified and technically capable to perform the services required in accordance with the statement of work.
The technical information shall explain how the Contractor intends to accomplish the services required, and contain resumes and certifications for the Professional Engineer and ANST Inspector to be assigned to the project.
The Past Performance Information shall include:
i) Proof the contractor has performed work of the same or similar scope.
ii) Proof the contractor has a record of confirming to specifications and the standards of good workmanship, and has demonstrated adherence to contract
RFQ SP310008T0050 Page 4 of 10 schedules, including the administrative aspects of performance of work as described in this contract.
iii) Proof the contractor has established a reputation for reasonable and cooperative behavior, commitment to customer satisfaction and general business like concern for the interests of the customers.
v) Give contract numbers, dates and duration of contracts.
vi) List customer’s company name, address, phone number and point of contact for each contract.
vii) Identify the number and names of subcontractors utilized on each contract.
viii) Provide approximate dollar value of each contract.
Evaluation
Award will be made to the lowest price technically acceptable contractor.
Contractors determined to have passed the technical capability requirements will then be evaluated for acceptable past performance history. Any contractor who has submitted technically acceptable information and who has been found to have a satisfactory record of past performance will then have its quote evaluated for price. Technical and Past Performance information shall be evaluated for acceptability (only) on a pass/fail basis and will not be rated.

To receive awards resulting from solicitations issued after May 31, 1998, your company must be registered in the DoD Central Contractor Registration (CCR) Database.

“Women-owned small business concern,” means a small business concern --
(1) That is at least 51 percent owned by one or more women; or, in the case of any publicly owned business, at least 51 percent of the stock of which is owned by one or more women; and
