About this file
This is a Mercury Affidavit form for the State of Maryland, specifically for procurement SP270014 related to a contract opportunity for 13" and 14" MacBook Air computers and accessories. The affidavit is effective as of July 2024 and must be submitted with all bid and proposal packages. Bidders are required to complete the form by identifying an authorized representative with legal authority to execute the affidavit on behalf of their business and provide a declaration regarding mercury content in the products being offered.
The affidavit requires vendors to certify whether their offered products contain mercury or do not contain mercury. If mercury is present in any product or product component, vendors must provide detailed information in an attachment that describes the specific product or component containing mercury and specifies the amount of mercury present with the corresponding unit of measure. Vendors acknowledge that the affidavit will be shared with the State of Maryland, its subdivisions, other states, and federal government entities, and that the submission is subject to applicable U.S. and Maryland law. False statements in the affidavit constitute perjury, and the certification is binding on the vendor throughout the contract term with respect to all mercury-related representations and obligations.
