The Quality Assurance Surveillance Plan establishes performance standards requiring 100% compliance that repaired equipment functions as intended, with the contractor providing assessment reports to the Government Services Office (GSO). The contractor must maintain close liaison with the Government, addressing any problems or questions within 24 hours (one business day), and provide a final condition report upon completion. The contractor must comply with FAR Clause 52.222-50 regarding Combat Trafficking in Persons with 100% compliance and immediate notification of violations. Additionally, the contractor shall report all contractor and subcontractor labor hours via a secure data collection website (https://doncmra.nmci.navy.mil), with all data reported no later than October 31 of each calendar year for the applicable Government fiscal year (October 1 through September 30).

9. OTHER CONSIDERATIONS:
9.1. Technical Clarifications: All technical clarifications shall be coordinated through the Engineering Rep and Technical Subject Matter Experts.
9.2. Lay-down and Utilities Requirements: Vendor to coordinate with the COR and Project Subject Engineer for delivery(s) of parts to designated lay-down area.
9.3. POC Clarification: Government Technical Point of Contact is the primary point of contact is charge of reporting equipment status and receipt inspection.
QUALITY ASSURANCE SURVEILLANCE PLAN
Purpose: To ensure that the Government has an effective and systematic method of surveillance for the services in the SOW. The QASP will be used primarily as a tool to verify that the contractor is performing all services and delivery/installation of replacement parts required by the SOW in a timely, accurate and complete fashion.
Performance Measurement: Performance will be measured in accordance with the following table:

"The contractor shall report ALL contractor labor hours (including subcontractor labor hours) required for performance of services provided under this contract for the Fiber Blower Machine Rental via a secure data collection site. The contractor is required to completely fill in all required data fields using the following web address https://doncmra.nmci.navy.mil.
Reporting inputs will be for the labor executed during the period of performance during each Governmental fiscal year (FY), which runs October 1 through September 30. While inputs may be reported any time during the FY, all data shall be reported no later than Oct 31 of each calendar year, beginning with 2014. Contractors may direct questions to the help desk at:

AQL = Acceptable Quality Level
If performance is within acceptable levels, it will be considered to be satisfactory. If not, overall performance may be considered unsatisfactory.
