STATEMENT OF WORK (SOW)

PUBLIC HEALTH NURSING Curriculum Development, Training, and Program Technical Assistance

(PHNCDTA)

Mission Statement The mission of the Indian Health Service is to raise the physical, mental, social, and spiritual health of American Indian and Alaska Natives (AI/AN) to the highest level. Public Health Nursing (PHN) within the Division of Nursing Services (DNS), Indian Health Service Headquarters (IHS/HQ) addresses improving the health of populations and individuals through prevention and health promotion to promote the healthcare systems focus on quality care, treatment, and promotion of life, while respecting the wellness and resilience of AI/AN people.

A. Background While the Indian Health Service (IHS) was previously the principal health care delivery system for AI/AN, there is now a less centralized and more diverse network of care provided by federal, Tribal, and Urban Indian health programs. The Indian health system denotes this larger network of programs and care delivery throughout Indian Country. The IHS/HQ PHN program is committed to providing training resources and technical assistance to ensure that health care providers are prepared to meet the health care needs of AI/AN patients and address the well-being of entire populations by supporting communities to make healthy choices. IHS/HQ PHN program is seeking a contractor to provide public health nursing in-person and web-based training; curriculum development; and technical assistance to support program sustainability, maintenance and capacity building.

The PHN program provides direct patient care services and manages community health initiatives for the AI/AN population, from developing population-based nursing interventions to preparing for and responding to public health disasters. The PHN Program provides health care services in the community which improve access to health care and expand service options. PHNs are licensed, professional nursing staff available to improve transitions of care by providing patients with support to promote knowledge and self-management of their condition as they transition from the hospital to home. PHN expertise in communicable disease assessment, outreach, investigation, and surveillance help to manage and prevent the spread of communicable diseases. PHNs conduct nurse home visiting services via referral.

PHNs perform a community assessment to identify high-risk populations and implement evidenced based interventions to address identified areas of need. This activity targets fragmentation in patient care services and improve care continuums. Interventions are monitored with data collection and evaluated for outcome.

Practical local access to comprehensive health care services is vital for the AI/AN population. AI/AN people born today have a life expectancy that is 5.5 years less than the US all races population (73 years to 78.5 years, respectively). They continue to die at higher rates than other Americans from diseases such as chronic liver disease and cirrhosis (some cases are result of chronic alcoholism, obesity, and exposure to Hepatitis B and C viruses), diabetes mellitus, unintentional injuries, assault/homicide, intentional self-harm/suicide, and chronic lower respiratory diseases. Health care services, accessible when and where they are needed, are essential to reduce and eliminate persistent health disparities that burden AI/AN people. Given the high rates of health disparities and related health consequences places critical importance on training PHNs to provide health care and preventive services, offer appropriate resources, and collaborate in a coordinated community response team regarding public and community health services.

B. Purpose
