STATEMENT OF WORK

LEIPERS FORK DISTRICT OFFICE

HVAC REPAIR AND INSTALLATION

DECEMBER 2023

HVAC SERVICES

(TN)

NATCHEZ TRACE PARKWAY

GENERAL: The Natchez Trace Parkway needs several HVAC related components replaced at the Leipers Fork District Office. Several components have failed or have exceed their life expectancy.

1. Scope of Work: The Natchez Trace Parkway has an inoperable HVAC system located at the Leipers Fork District Office. An A&E Design Firm has completed and evaluation of the system. This contract will follow the design recommendations, specifications, and more importantly, the Summary of Work (see additional attachments from the completion of the final design documents) which are supplemental to this Statement of Work.

Observed 2024 Federal Holidays are:

June 19th,2024 – Juneteenth Independence Day July 4th, 2024 - Independence Day September 2nd, 2024 – Labor Day October 14th, 2024 – Columbus Day November 11th, 2024 – Veterans Day November 28th, 2024 – Thanksgiving Day December 25th, 2024 – Christmas Day January 1st, 2024 – New Year’s Day January 15th, 2024 – Martin Luther King Jr Day February 19th, 2024 – Washington’s Birthday May 27th, 2024 – Memorial Day

2. Location: Leipers Fork District Office, (Mile Post 428, Natchez Trace Parkway) 5430 Pinewood Road, Franklin, Tennessee 37064.

Latitude - 35°53'39.32"N Longitude - 87° 1'29.89"W.

3. Work Schedules: All work must be completed between the hours of 7:00 AM and 4:30 PM, Monday thru Friday, excluding Holidays, except when otherwise indicated. A proposed schedule of work for the project construction shall be submitted to the COR for approval 5 days prior to starting construction. The work must be planned and executed to minimize conflict with the office daily activities.

4. Supplies, Materials, and Equipment: None provided by the National Park Service, Natchez Trace Parkway. The Contractor may store materials and equipment at the work site at their own risk, in a predetermined area. All materials must be kept in these designated areas determined by the COR and must remain neat and orderly.

5. Conduct of Contractor's Employees: The contractor and all of contractor's employees shall treat all visitors to the area and NPS employees with courtesy and respect. Any violation of this provision will be cause for immediate removal from the Parkway and possible termination of this contract.

6. Key Control: National Park Service personnel will be on site and access is only available when National Park Service personnel are present. The ground and building are controlled and access to the facility or grounds are only available during hours of operation specified under Work Schedule.

7. Safety: The contractor shall provide a Safety Plan prepared and submitted to the COR before work can begin. The Safety Plan should be a written document that describes the process for identifying the physical and health hazards that could harm workers, procedures to prevent accidents, and steps to take when accidents occur. The written safety plan is your blueprint for keeping workers safe. The plan needs to compile the list of activities and what hazards exist and what safety equipment will be used to prevent accidents from occurring. The plan needs to include 24-hour emergency contact information.

A. All reportable accidents, which are those resulting in death, occupational disease, traumatic injury to employees or the public, fires, and property damage by accident more than $100, shall be reported to the Contracting Officer and COR immediately. Within 7 days of a reportable accident, the contractor shall fill out and forward to the Contracting. Officer an Accident/Property Damage Report.

B. If the Government determines that the Contractor did not use reasonable care for Government property and caused damage to or loss of said property, the Contractor shall repair the damage or replace the property at no cost to the Government. If the Contractor fails or refuses to make such repairs or replacement, the Contractor shall be liable for the cost, which shall be deducted from the contract payment.

C. The interruption of mechanical and electrical services to other parts of the building shall be coordinated with the designated COR.

D. Nonsmoking Building/Tobacco Use/Vaping: Smoking and all tobacco use is not permitted within the building or within 25 feet of entrances, operable windows, or external air intakes.

E. No firearms are allowed in government buildings except for on duty law enforcement officials.

8. Contractor License and Other Requirements: Contractor must be licensed in the State of Tennessee in areas where licensing is a requirement or for any required or necessary work to be completed for or at the job site.

9. Contractor Performance Evaluation:

a. All services performed under this contract shall be subject to inspection and approval by the Contracting Officer or their designated representative.

b. Inspection is governed by the contract clause "Inspection of Services." "Services," as used in this clause, includes services performed, workmanship, and material furnished or utilized in the performance of services.

c. The Government has the right to inspect and test all services called for by the contract, to the extent practicable always and places during the term of the contract. The Government shall perform inspections and tests in a manner that will not unduly delay the work.

d. When all required tasks have been accomplished in a complete and skillful manner, then the work will be considered acceptable.

However, should one or more tasks required not be completed or completed in a manner which is unacceptable to the Contracting Officer or his designated representative, then the services will be considered unacceptable. The decision of acceptability rests with the Contracting Officer, whose decision is final.

e. If any of the services do not conform to contract requirements, the Government may require the Contractor to perform the services again in conformity with contract requirements, at no increase in contract amount. If required, the contractor shall correct deficiencies within twenty-four hours of notification of defect. When the defects in services cannot be corrected by reperformance, the Government may (1) require the Contractor to take necessary action to ensure that future performance conforms to contract requirements and (2) reduce the contract price to reflect the reduced value of the services performed.

f. If the Contractor fails to promptly perform the services again or to take the necessary action to ensure future performance in conformity with contract requirements, the Government may (1) by contract or otherwise, perform the services and charge to the Contractor any cost incurred by the Government that is directly related to the performance of such service or (2) terminate the contract for default.

10. Payments: The government will make milestone payments as agreed upon with the Contracting Officer, followed by final payment for the services under this contract once all aspects of the required work have been completed.
