SECTION 1
DESCRIPTION OF SERVICES
1. Description of Services. Contractor shall furnish all labor, equipment, supervision, management, supplies, linen, bulk delivery carts, transportation, and facilities (except as identified elsewhere in this solicitation) necessary to perform contractor owned, contractor operated bulk laundry and linen services and tasks required under this contract for the 366th Healthcare Operations Squadron, 90 Hope Drive, Mountain Home AFB, ID and MHAFB Veterinary Services, 455 Phantom Ave, Bldg 2616, Mountain Home AFB, ID. All services are to be performed in accordance with standard industry practices and quality assurance measures for hospital linen services. Services shall include pick-up and delivery of items by contractor personnel. These services are in support of patient care programs and base veterinary services.
1.1. Requirements.
1.1.1. Pick-up. The contractor shall pick up soiled laundry Monday through Friday between 9AM and 3:30PM, except holidays, at buildings 2616 and 6000.
1.1.2. Delivery. Contractor shall deliver clean, finished, packaged laundry no later than 48 hours after initial pick up of the soiled articles for Monday through Wednesday pick ups. For Thursday and Friday pick ups, deliveries will be made the first duty day after a weekend or holiday. Deliveries must be made between 9AM and 3:30PM. Delivery carts shall be clean and in sanitary condition.
1.1.3. Industry Regulation and Codes. The Contractor shall comply with applicable Occupational Safety and Health Administration (OSHA), federal, state, and local regulations and codes. The Contractor shall keep abreast of and comply with changes in all regulations and codes applicable to the contract.
1.1.4. Specifications. Contractor furnished wash formula, chemicals, materials, equipment and tools used shall be standard products of manufacturers regularly engaged in the production of such items. All items must be approved by the 366th MTF Infection Control Function prior to being used. All supplies and equipment used or changes in previously approved supplies or equipment shall require literature and may require samples. All materials must comply with all applicable environmental laws.
1.1.5. Packaging. The packaging will be as indicated in Appendix 2.
1.1.5.1. All packaged laundry shall be wrapped in plastic film two layers thick unless otherwise specified in the contract.
1.1.5.2. Physician’s coats shall be hung individually on hangers of a type that does not stain garments and individually covered with plastic film bags.
1.1.5.3. Color-Coded Items. All items marked with color-coded strips or tags (normally located in/on collar, neckband, waistband, etc.) shall be folded and bundled in a manner which makes the color-code easily visible without having to open the bundle. These items may include scrubs, gowns, jackets, robes, pajamas, etc.
1.1.5.4. Sized and Colored Items. All items carried by size (small, medium, large, etc.)
shall be bundled by size. Items coming in varied color and size shall be bundled by color and size respectively. EXAMPLE: Small cranberry scrub pants would be bundled together; small purple scrub pants would be bundled together, not mixed with small cranberry scrub pants.
1.1.6. Maximum Shrinkage Factor. The total number items delivered on any given day shall be no less than 92 percent of the total number of soiled items picked up 48 hours previous to the delivery. Further, the assortment of clean laundry delivered shall be comparable to that of the soiled laundry picked up. Should the required percentage or mix not be attained, the contractor shall be afforded 24 hours to rectify.
1.1.7. Contaminated Laundry. All soiled laundry originating from the Military Treatment Facility (MTF) is categorized as contaminated. The contractor is responsible for handling contaminated laundry in accordance with Health Care Infection Control Practices Advisory Committee (HICPAC). (Draft Guidelines for Environmental Infection Control in Health Care Facilities, 2001 or most current.)
1.1.8. Reperformance of Laundering. In accordance with the provisions of this contract covering inspection, acceptance, and payment, items found not to meet the requirements of the contract terms shall be recleaned at no additional cost to the Government. Such items (found by the MTF personnel as packages are opened) shall be identified by the QAP to the contractor's representative at the next scheduled pick up after the unsatisfactory condition is discovered. After they are recleaned, the contractor shall pack these items separately and mark each bundle "Attention Quality Assurance Personnel (QAP)." If reperformance of linen is required, the contractor must assure adequate quantities remain in-house to meet functional requirements. When government owned items cannot be reperformed due to damage caused by the contractor, the contractor is responsible for obtaining the replacement.

SECTION 3
GOVERNMENT FURNISHED PROPERTY AND SERVICES
3. Government-Furnished Property and Services. The Government shall provide the items listed below:
3.1. Telephone. Contractor personnel shall be allowed the use of a Government telephone for local calls upon request.
3.2. Commercial Grade Scales. Scales shall be of a type capable of weights up to 2,000 pounds. Scale is located in room P1-10 of Building 6000. They shall be calibrated for accuracy at least semiannually by government.
SECTION 4
GENERAL INFORMATION
4. General Information..
4.1. Contract Management:
4.1.1. Contractor Responsibilities:
4.1.1.1. The contractor shall designate, in writing, one program manager (PM) who shall be responsible and accountable for the overall supervision and control of all contractor services and personnel required to satisfy the requirements of this PWS and shall also function as a technical representative. If applicable, the PM shall identify, in writing, a project manager to the contracting officer. The PM is to be reachable during normal business hours, which are 0700 a.m. to 3:30 p.m. Monday through Friday, excluding holidays. The PM will also provide emergency contact numbers to be used in emergency situations.
4.1.1.2. The contractor shall have a Quality Assurance Program in place that covers all requirements listed in the Service Delivery Summary (see Section 2.0). The program shall consist of methods used for identifying and preventing defects in the quality of service performed.
4.1.1.3. While working on government installations, contractor personnel shall present a neat appearance and be easily recognized. This may be accomplished by wearing distinctive clothing bearing the name of the company and employee name or by wearing appropriate badges bearing the name of the company and employee name. Employees are expected to comply with reasonable dress and grooming standards based on comfort, productivity, health and type of position occupied. Employee attire will be in good repair, and should not be considered offensive, disruptive or unsafe.
4.1.1.4. All contractor commercial vehicles shall be easily recognizable by either painting the side of the vehicle with the company name and/or logo or using magnetic signs containing the company name and/or logo. The contractor shall obtain identification cards and base passes for all employees and vehicles. The application for passes shall be coordinated through the Contracting Officer or his/her representative.
Upon termination of employment, the contractor shall return identification cards and base passes to the 366th Security Forces Squadron within 24 hours of the release of the employee.
4.1.1.5. Contractor personnel must be able to read, write, speak, and understand the English language.
4.1.1.6. The contractor shall not employ any person who is an employee of the United States government if the employment of that person would create a conflict of interest, nor shall the contractor employ any person who is an employee of the Department of the
Air Force, either military or civilian, unless such person seeks and receives approval in accordance with DOD 5500.7 and the Joint Ethics Regulation.
4.1.1.7. The contractor shall, without any additional expense to the government, be responsible for obtaining any necessary licenses and permits, and for complying with any applicable Federal, State, and municipal laws, codes, and regulations in connection with the performance of the work as specified in this PWS. The contractor shall be liable for all personnel injuries and equipment damage that result from their negligence or failure to follow safety/health guidelines. The contractor shall take proper safety and health precautions to protect work, workers, the public, and the property of others, to include all appropriate insurance.
4.1.1.8. The contractor’s PM shall implement procedures to encourage early problem identification and expeditious resolutions and/or remedies.
4.1.1.9. The contractor shall be responsible for complying with the security requirements as established by the Base Commander.
4.1.1.10. The contractor shall verify the items and countersign the ticket to indicate agreement with those items. Contractor personnel shall be responsible for physically transferring soiled laundry into the contractor’s vehicle.
4.1.1.11. Several departments within the MTF (Medical Treatment Facility) have articles that, by their nature, cannot be processed along with the bulk-soiled linen. These items include, but are not limited to: privacy curtains, window curtains, restraint sets, VIP linens, thermal or oversized scrubs, surgilift pads, oven pads and mittens, etc. The appropriate method for cleaning (laundering, dry cleaning, etc.) and packaging (on hangers, bundling, etc.) these articles shall be at the professional discretion of the contractor. Except for privacy/window curtains shall be dry cleaned.
4.1.1.12. The contractor shall launder and return all unserviceable and non-repairable articles within 48 hours of identification in separate bundles clearly marked
“UNSERVICEABLE.”
4.1.1.13. All workmanship shall be in accordance with practices established by the National Association of Institutional Linen Management (NAILM), International Fabricare Institute (IFI), Textile Rental Services Association (TRSA) and accepted industry standards. It is intended that services shall include all processes necessary for the laundering of the articles even though every step involved is not specifically mentioned. In no case shall clean and soiled linen share the same physical space. *Plants in which the work is done shall be open to inspection of sanitary conditions by Government representatives during contracts normal business hours. After processing, laundered articles shall be in serviceable and sanitary condition, meeting established quality and performance requirement standards as shown in the Service Delivery Summary.
4.1.1.14. Contractor must comply with MHAFB Instruction 31-204 (Latest Version). In addition contractor must comply with The Joint Commission Laundry Practices Infection Control Assessment Checklist (Appex 3) and the Air Force Inspection of Laundry Services (Appex 4).
4.1.2. Physical Examination: Contractor’s personnel who acquire a communicable illness shall not perform service under the contract and must be free of illness before returning to work. The Contractor shall be responsible for all physical examinations.
4.2.Government Responsibilities.
4.2.1. Government shall count soiled laundry and shall record the count on a contractor supplied laundry ticket prior to pick up by the contractor. The Government shall retain a copy of each completed ticket.
4.2.2. Scales shall be of a type capable of weights up to 2,000 pounds. Scale is located in room P1-10 of Building 6000. They shall be calibrated for accuracy at least semiannually by government.
4.2.3. Facility Inspection. Government representative(s) shall have the right to access and inspect the bidder's laundry plant to assure ability to perform and/or Contractor's performance and compliance prior to award and during the full term of the contract.
4.3. Quality Performance Plan. (QPP) The contractor shall develop and maintain a quality program to ensure laundry services are performed in accordance with commonly accepted commercial practices, manufactures’ recommendations, and this contract and all federal, state and local laws and regulations. As a minimum, the contractor shall develop quality assurance procedures addressing the areas identified in the Service Delivery Summary. The contractor shall develop and implement procedures to identify, prevent, correct and ensure non-reoccurrence of defective services. The contractor shall provide a copy of the QPP to the contracting officer no later than the pre-performance conference and updated copies as changes occur. Records of all inspections conducted by the contractor and corrective action taken shall be made available to the government upon request.
4.4. Performance Evaluation Meetings. The CO shall require the Contractor to meet with the CO, contract administrator, Functional Commander, and other government personnel as often as deemed necessary. The Contractor may request a meeting with the CO quarterly and/or when deemed necessary. Meetings shall be documented in the contract file with written minutes signed by the contractor and the CO, or contract administrator. Should the Contractor not concur with the minutes, such non-concurrence shall be provided in writing to the CO within ten (10) calendar days of receipt of the minutes.
4.5. Disruption of Services. If routine services are disrupted through fault or negligence of the contractor for more than twenty-four (24) hours, the Government reserves the right to procure such from another source.
4.6. Performance of Service During Crisis Declared by the National Command Authority or Overseas Combatant Commander. According to DODI 3020.37, on occasion, services may be required to support an activation or exercise of contingency plans outside the normal duty hours, as described in Paragraph 4.3.1.
4.6.1. Contingency or emergency tasking for laundry services shall be limited to large increases in patient census due to accident and rescue operation, civil disturbances, natural disasters, and military peacetime and wartime contingency operations. The workload generated by these events is dependent on the number of patients admitted and the severity of their injuries. The Contracting Officer shall notify the contractor at the beginning and end of the contingency and/or emergency period. For the duration of the contingency/emergency period, the contractor shall continue to provide the required service within the times specified within this document.
4.6.2. Scheduled services are to be accomplished subject to emergency situations, which may require alteration of schedules. Contractor shall respond by phone within one hour and physically respond within eight (8) hours on emergency calls. Emergency situations shall be defined by the QAP. In the event of medical facility emergencies and/or disasters, the Contractor may be required to perform additional laundry services.
4.7. Safety: The contractor shall establish a safety program that establishes preventative measures and delineates safety goals and objectives. The contractor shall support government safety inspections, scheduled or unscheduled.
4.8. Transportation
4.8.1. Civil Damages. The government is not liable for civil damages resulting from actions of contractor employees while utilizing contractor owned or civilian privately owned vehicles (POV).
4.8.2. Licenses and Training. Contractor motor vehicles shall be operated and maintained in accordance with applicable regulations and laws when such motor vehicles are operated on government installations.
4.9. Holidays: When the holiday is on a Saturday, the preceding Friday will be the federal holiday. When the holiday is on a Sunday, the following Monday will be the federal holiday. Asterisk (*) indicated dates change each year. The Base Commander sometimes declares “family days” or other days for base personnel to take a holiday.

APPENDIX 1
DEFINITIONS:
Assortment: This refers to the variety of soiled linen turned over each day to the contractor for cleaning. It is essential that the mix of clean linen returned be comparable to the soiled linen being picked up.
Government Quality Assurance: Those actions taken by the Government to assure services meet the requirements of the PWS and contract provisions.
Contracting Officer (CO): The Contracting Officer is the person with the authority to enter into, administer, modify, and/or terminate contracts for the government. The Contracting Officer is responsible for ensuring the performance and compliance with the terms of the contract and safeguarding the interests of the government in its contractual relationship.
Program Manager (PM): The contractor individual responsible for overall performance of this contract appointed, in writing, by an officer of the company to the Contracting Officer. The Program Manager shall be available to meet with the Contracting Officer to discuss performance.
Service Delivery Summary: The service delivery summary defines the performance objectives and the performance thresholds for meeting those objectives.
Quality Assurance Personnel (QAP): The designated government representative who shall perform surveillance of the contractor’s performance and is responsible for collecting all standardized evaluation and data collection forms for his/her performance site.
Laundering: The process by which soiled fabric items are cleaned, starched, ironed, pressed, and treated. The term includes cleaning by mechanical action in appropriate temperature water with appropriate solutions of detergents and other additives. The appropriate products and procedures are to be determined by the contractor.
Clean: Finished articles are dry and free of visible soil, lint, and objectionable odors.
Spots and stains are removed without damaging the fabric. Tape and other adhesives shall be removed prior to or during the laundering process.
Unserviceable Articles: Those articles no longer usable for the purpose intended.
Medical Treatment Facility (MTF): Those facilities providing outpatient and/or inpatient health care services for authorized personnel.
Maximum Shrinkage Factor: The permissible items variance between the total items of soiled linen picked up on any given day and the total items of that same linen when it is cleaned and returned.

This checklist includes questions to ask to assess the infection control risks in your laundry practices. You can use this tool to review your laundry practices and minimize laundry-related infection control risks. Answers to all questions should ideally be Y for Yes (unless they aren’t applicable).
