Subject

The Federal Bureau of Prisons Field Acquisition Office, Grand Prairie, Texas, intends to make an award for the provision of Dental Assistant, Dental Hygienist, Pharmacy Technician, and Radiologic Technologist services to the inmate population at the Metropolitan Detention Center (MDC), located in Guaynabo, Puerto Rico. General information regarding the Federal Bureau of Prisons (BOP), and its facilities can be located at www.bop.gov. The Government reserves the right to award without discussion and to make a single award. Therefore, each initial offer should contain the offeror’s best terms from a cost or price standpoint.

Place of Performance

Metropolitan Detention Center 652 Road 28 Guaynabo, Puerto Rico 00965

The services will be performed on-site in the Health Services Department at MDC Guaynabo, in a clinical setting. The contractor will be required to sign the contractor log indicating the date of visit to the institution along with the time of arrival and departure for security reasons.

Schedule

The contractor will normally work, Monday through Friday (except Federal holidays), normally between the hours of 7:30 a.m. and 4:00 p.m. with a thirty (30) minute unpaid lunch period, (if applicable). A session will equal one (1) hour. The Dental Assistant, Dental Hygienist, Pharmacy Technician, and Radiologic Technician are each expected to work an estimated 1,872 sessions per year. The facility is expected to require an average of 40 sessions per week. The schedule shall not exceed forty (40) hours per week. The Contracting Officer's Representative (COR) will be responsible for defining each weekly schedule. The schedule is subject to change based on the variable needs of the institution and the unpredictable nature of the correctional environment. Upon agreement, any changes to the schedule must be coordinated with the COR assigned under the contract. The contractor will be available for emergencies as necessary. A mutually agreed upon schedule will be determined following award of a contract. The contractor may be subject to schedule changes and must be flexible.

Due to the nature of the correctional setting, delays in performance of work may be experienced.

Such delays may include but are not limited to inclement weather, electrical or mechanical problems, or other security matters. When contract employees report to the institution but are unable to perform services due to such delays, the contractor will be paid for one (1) session.

Normally, any remaining sessions for that day will be suspended and the contractor will be free to leave and the contractor will not be paid for the remaining unperformed sessions. However, if the institution requests that the contractor remain at the institution for eventual performance, the contractor shall be paid for such sessions. In the event of any absence, the contractor shall provide replacement coverage. If the contractor fails to provide coverage, the Government may seek contractual remedy.

Description of Duties

Dental Assistant:

All duties shall be performed in accordance with standard and methods generally accepted within the field of Dental Assistant Service and compliance with all Federal and State policy and regulation. The contractor shall adhere to all standards and regulation associated with the Accreditation Association for Ambulatory Health Care (AAAHC) and American Correctional Association Foundation/Core standards for Adult Local Detention Facilities (ACA). The COR shall monitor the Contractor’s performance. The contractor must have the ability to work with an inmate population from a variety of cultural, regional, racial, religious, and ethnic backgrounds.

Services will be unsupervised, but will be monitored by an assigned contract monitor in accordance with Bureau of Prisons policy. The contractor will abide by MDC Guaynabo's infection control policies. The contractor must have knowledge of the current Occupational Safety Hazard Administration (OSHA) guidelines and infection control techniques affecting the practice of dentistry; pharmacy, and Radiology. The contractor shall (but is not limited to):

• Maintain a system of dental records on inmates in accordance with Federal Acquisition Regulation (FAR) 52.224-1, Privacy Act Notification, and 52.224.2, Privacy Act. All records maintained by the contractor under this contract will remain the property of the institution.

• Provide chair-side assistance to the dentist in restorative, endodontics, oral surgery, periodontics, prosthodontics, and routine examinations.

• Perform clerical duties.

• Maintain the dry goods, dental instruments, materials and inventories.

• Restock and sterilize dental instruments and equipment.

• Process all biomedical hazardous waste including special handling of needles, syringes, and instruments.

• Operate dental equipment, instruments, and materials utilized a dental clinic.

• Control hazardous materials inventories.

• Provide verbal and written hygiene instructions.

• Assist the Dentist in providing oral health educational programs to the inmates.

• Provide and assist the dentist in providing prevention services.

• Clean, polish and/or oil the equipment and supplies according to the manufacturer’s or biomedical recommendations.

• Clean and sterilize instruments, using autoclave techniques

• Prepare sterile tray and packs

• Maintain a number of recurring reports, including those concerning material and equipment.

• Store and maintain adequate supplies in the dental clinic.

• Greet, receive, and prepare the patient for treatment.

• Lay out instruments and materials needed for treatment.

• Obtain a current patient health history, record cursory examination of patient's oral cavity.

• Take radiograph such as bite wings, periapical, and panoramic film.

• Complete processing of exposed radiograph and mark them with identifying information.

• Must use proper Personal Protective Equipment (PPE), which consists of (but is not limited

to) the use of lead aprons and shield as protection against radiation.

• The contractor will be responsible for understanding and operating dental equipment, instruments and materials currently utilized at MDC Guaynabo Dental Clinic.

Dental Hygienist:

• Processing all biomedical hazardous waste including special handling of needles, syringes, and instruments.

• Controlling hazardous materials inventories.

• Performing oral prophylaxis and periodontal care service.

• Assisting the Dentist in providing dental care procedures.

• Cleaning, polishing, and/or oiling equipment and supplies according to manufacturer's or biomedical recommendations.

• Cleaning and sterilizing instruments using autoclave techniques.

• Preparing sterile trays etc.

• Maintaining a number of recurring reports and inventories including those concerning materials and equipment.

• Properly storing and maintaining adequate supplies in the dental clinic.

• Greeting, receiving and preparing inmate patients for treatment.

• Laying out instruments and materials needed for treatment.

• Obtaining a current patient health history.

• Assisting the Dentist in providing oral health educational program to the inmates

• Recording cursory examination of patient's oral cavity.

• Taking radiograph such as bite wings periapical, and panoramic films.

• Completing processing of exposed radiographs and marking them with identifying information.

• Using lead aprons and shields as protection again radiation.

• Providing oral and written hygiene instructions.

• Removing supragingival and subgingival calculus and stains

• Applying fluoride treatment, and provide coronal polishing of teeth (rubber cup prophylaxis).

The MDC Dental Hygienist shall be required to maintain a system of dental records on inmates in accordance with Federal Acquisition Regulation (FAR) 52.224-1, Privacy Act Notification, and 52.224.2, Privacy Act. All records maintained by the contractor under this contract will remain the property of the Metropolitan Detention Center.

Pharmacy Technician:

• Provide the non-personal services of a Pharmacy Technician which shall include the following tasks to prepare Request for Purchase (RP) of pharmaceuticals for Pharmacist's review and approval.

• Submit the RP to the HSA or designee for final approval and fund controlling.

• Receive pharmaceuticals and supplies into stock from the warehouse and conduct appropriate accounting process.

• Receive and fill prescription orders from Pharmacy stock items, as submitted.

• Read individual prescriptions and determine type, strength, and dosage of drug dispensed.

• Correlation of instructions on the order and Pharmacy policy limitation must be known.

• Set-up prescriptions by obtaining correct stock containers, counting, and placing medications in smaller containers.

• Dose and dispense of medications during scheduled medication lines.

• Print out daily medication profiles for the physicians.

• Enter patient profile (i.e. new inmate arrivals, date of birth, allergies, and corrections as needed).

• Maintain a clean and organized Pharmacy area.

• Assist the Pharmacist in monthly inspections/inventories (i.e. ward, syringe inventory, drug inventories, etc.).

The Pharmacist checks all contents and label prior to dispensing, process prescriptions manually, or via pharmacy computer, patient profile are reviewed for change in therapy, allergies, drug in-teractions and timeliness of renewals. Any questions and/or changes of problems are referred to the Pharmacist.

Radiologic Technologist:

• Process all biomedical hazardous waste including special handling of needles, syringes, and instruments.

• Provide the non-personal services of a Radiologic Technologist (X-Ray Technologist) which shall include performing radiographic examinations, as requested by the health care provider staff on inmates entrusted to the care and custody of the Federal Bureau of Prisons who are housed at MDC Guaynabo.

• Perform X-rays of chest, extremities, abdomen, skull, sinus exams, spine films, views of major joints and soft tissue views.

• Package and ship x-ray examination films to the contract radiology service for interpretation.

• Assure the return of all examination reports and films.

• File authenticated reviewed x-ray reports in the patient's medical record and x-ray file jackets.

• Create x-ray file folders and file x-rays (developed film) in the film library.

• Perform x-ray processor quality control and monitor other performance indicators as required.

• Administer the exchange program of radiologic dosimetry badges and submit all reports of dosimetry to the consultant radiologist for review.

• Maintain the Radiology Library and create x-ray file folders.

• Properly store and maintain adequate supplies in X-Ray room.

• Greet, receive, and prepare the patient for treatment.

The MDC Radiologic Technologist (X-Ray Technologist) shall be required to maintain a system of X-ray records on inmates in accordance with Federal Acquisition Regulation (FAR) 52.224-1, Privacy Act Notification, and 52.224.2, Privacy Act. All records maintained by the contractor under this contract will remain the property of the MDC.

Education and Experience

Dental Assistant:

The individual providing Dental Assistant services under this award must have graduated from an accredited school or program; be licensed as a Dental Assistant in the Commonwealth of Puerto Rico, and have appropriate insurance coverage certificate. The contractor must have a current Cardio-Pulmonary Resuscitation (CPR) certification, from the American Heart Association or American Red Cross.

Dental Hygienist:

The individual providing Dental Hygienist services under this award must have graduated from an accredited school or program; be licensed as a Dental Hygienist in the Commonwealth of Puerto Rico, and have appropriate insurance coverage certificate. The contractor must have a current Cardio-Pulmonary Resuscitation (CPR) certification, from the American Heart Association or American Red Cross.

Pharmacy Technician:

The individual providing Pharmacy Technician Services under this contract award must have graduated from an accredited school or program; be licensed as a Pharmacy Technician in the Commonwealth of Puerto Rico, and have appropriate insurance coverage certificate. The contractor must have a current Cardio-Pulmonary Resuscitation (CPR) certification, from the American Heart Association or American Red Cross.

Radiologist Technologist:

The individual/organization providing Radiologic Technologist (X-Ray Technologist) services under contract award must have graduated from an accredited school or program; be licensed as a Radiologic Technologist (X-Ray Technologist) in the Commonwealth of Puerto Rico, and have appropriate insurance coverage certificate. The contractor must have a current Cardio-Pulmonary Resuscitation (CPR) certification, from the American Heart Association or American Red Cross.

Offeror’s must specifically identify the individual that will be performing these services under any subsequent contract award. The offer must provide proof that their candidate meets all qualifications. The candidate's name and qualification must be submitted at the time the offer is submitted. All offeror’s must submit with their quote a resume, photocopies of documents evidencing education, training, and proof of insurances.

Supplies:

Due to security reasons all supplies and equipment the contractor may need to perform the duties of this contract will be furnished by the institution. The MDC will provide personal protective equipment such as gowns, gloves, surgical masks, and eye protection. If the contractor requires special equipment, he or she must make a request in writing to the Health Services Administrator and/or designee of the MDC. The contractor will be responsible for the accountability of any supplies and equipment utilized. No equipment will be removed from the institution, and the contractor will not bring any supplies or equipment into the institution.

Attire

The contract employee must wear appropriate and professional attire at all times. The contract employee is prohibited from wearing khaki or green clothing of any type as this is the color clothing the inmate population wears. Jeans (of any color), T-shirts, sandals, tank tops, and sweat pants/shirts are not considered appropriate professional attire.

Employee Health Requirements

The contractor will ensure the Dental Assistant is free of infectious diseases. Any individual performing as a Dental Hygienist under this contract who subsequently develops a medical condition which might reasonably be expected to place other workers, inmates, or the public at risk shall be immediately removed from activities requiring performance of invasive procedures and activities which might place others at risk. In any such case, the contractor shall be responsible, at no additional cost to the institution, for securing the services of any replacement personnel required to contract performance.

Restrictions on Contract Workers

The Contractor shall not supervise inmates, supervise Bureau of Prisons staff, participate in inmate accountability, requisition, sign for, or pick up supplies, formulate policy or procedures, or attend meetings, staff recalls or staff functions, unless otherwise required by the Health Services Administrator or designee.

Patient Management/Inmate Management

Provision of services will require frequent and unsupervised contact with inmates. The contract employee shall not be responsible for the management of any inmates, other than to ensure inmates involved within the contract’s scope of work abide by all rules, in effect, to ensure the safety and well-being of all participants. The contract employee will report any infractions of rules and regulations by any inmate to the Health Services Administrator or the COR. Subsequently, the contract employee will be required to provide documentation to the COR, or designee, of any such event. The contractor may, at any time, be removed from the institution at the discretion of the Warden. The contract employee is to constantly be aware of security and maintain a professional relationship with inmates at all times.

Patients shall be afforded impartial access to treatment or accommodations that are available or medically indicated, regardless of race, creed, sex, national origin, or religion. Staffing shall be commensurate with the expected needs of the patients and within the scope of services offered.

The contractor shall NOT provide information to the inmates(s). If the situation arises where inmate(s) are required to be provided information, the Clinical Director, Health Services Administrator (HSA), or COR shall provide any information the inmate(s) are to receive. The contractor is required to maintain an open line of communication with the Clinical Director, HSA, and COR.

Institution Security

The contractor agrees to adhere to all regulations prescribed by the institution in regards to the safety, custody, and conduct of inmates. The contractor will be required to attend a one-time orientation program, consisting of approximately four (4) hours, prior to performing any services under this contract. Both parties will mutually agree upon the date and time for this orientation, and reimbursement for attendance will be at base year’s hourly contract rate.

The contract employee will be required to sign in and out of the institution for accountability purposes. A badge will be issued and worn at all times for proper identification.

Method of Performance

The contractor has sole responsibility and discretion for choosing and implementing the manner and method of performing tasks outlined in this Statement of Work as long as the requirements are met in accordance with the policies described above. The institution’s clinical staff will ensure contract compliance, although not directly supervised, the COR shall review the services.

Contract monitoring reports will be prepared by the COR and maintained in the correct file.
