Statement of Work Supply and Deliver Chlorine and Sulfur Dioxide
To Water Wells and Wastewater Plant Hurlburt Field, FL
1. WORK INCLUDED: The Service Provider shall furnish all vehicles, containers, tools, equipment, parts, materials, supplies and labor for the loading, handling, transportation, and unloading of 150 pound Chlorine (up to 210 each total) and Sulfur Dioxide (up to 90 each total) cylinders. Chlorine gas and Sulfur Dioxide gas must be 100.00%pure. Facility accessibility is limited delivery vehicle should be no larger than a Freight Liner Business Class delivery truck. No Tractor Trailer Deliveries. Delivery vehicle must be able to deliver to the ground and to a loading dock.
2. JOB DESCRIPTION: The work to be performed includes the following principal features:
2.1 Service provider shall transport and unload chlorine cylinders to the following facilities 90601, 90355, 91146, and 91500.
2.2 Service provider shall transport and unload sulfur dioxide cylinders to 91500.
2.3 Service provider shall abide by all federal, state and local laws concerning the transportation and delivery of hazardous chemicals.
2.4 Service provider will provide cylinders and ensure hydrostatic test dates stay current.
2.5 Service provider shall ensure all cylinder valves are properly maintained IAW industry standards. In the event of a cylinder being unable to open it will be exchanged at no cost to the government.
2.6 Service provider should expect deliveries of up to 20 cylinders this includes chlorine and sulfur dioxide.
2.7 There will be a maximum of 33 deliveries to Hurlburt Field per year. This will include Water Operations and Wastewater Operations.
2.8 Service provider shall ensure all empties are picked up prior to delivery of full chlorine and sulfur dioxide at building 90053 and 91500.
3. PERFORMANCE TIME:
3.1 Service provider shall deliver quantity of full cylinders requested (chlorine, sulfur dioxide or both) within 10 business days from being notified.
3.2 The contract contains a base year as well as four (4) option years.
4. INSPECTION: Personnel of 1 SOCES/CEOIA and CEOIB shall service as the on-site representative of the Contracting Officer. Point of contact information will be provided at time of award.
5. SECURITY REQUIREMENTS:
5.1 The contractor shall comply at all times with base security requirements to include base pass requirements and personnel security clearance.

5.2 The contractor shall obtain base identification and vehicle passes, if required, for all contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor personnel are required to wear or prominently display installation identification badges or contractor-furnished, contractor identification badges while visiting or performing work on the installation.
5.3 The contractor will be required to complete a Contractor Employee Listing (to be provided by the government) in order for employees to gain access to the installation:
(i) The Contractor Employee List. This form must list every employee requiring access. Every time employees change, it must be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word “Escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations please use building numbers or areas (i.e. Bldg 90618 or Red Horse, AMMO Area, etc.). Once completed, send the list to the Contracting Office representative in EXCEL format using secure means such as hand carry, mail, or electronic mail. The Contracting Officer will then prepare and attach a cover letter, and forward the Employee List to the Visitor Control Center.
5.4 During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
5.5 When work under this contract requires unescorted entry to controlled or restricted areas, the contractor shall comply with AFI 31-101, Volume 1, The Air Force Installation Security Program, and AFI 31-501, Personnel Security Program Management, citing the appropriate paragraphs as applicable.
5.6 Upon completion or termination of the contract or expiration of the identification passes, the prime contractor shall ensure that all base identification passes issued to employees and sub-contractor employees are returned to the issuing office.
5.7 Failure to comply with these requirements may result in withholding of final payment.
5.8 Retrieving Identification Media. The contractor shall be responsible for retrieving all identification media including visitor passes from employees who depart for any reason before the contract expires. For example, termination for cause, retirement, etc… Visitor passes shall be retrieved and turned into 1SOSFS Pass and Registration/Visitor Center once the contractor and its employees leave the base for the last time.
5.9 Traffic Laws. The contractor and employees shall comply with base traffic regulations.
5.10 Weapons, Firearms, Ammunition. The contractor employees are prohibited from possessing weapons, firearms, or ammunition or within the contractor vehicle or privately owned vehicles while on the Government installation.
5.11 Contractor employees performing work in the controlled areas described as Open Secret Storage Area
(OSSA) must meet prescribed requirements IAW DoDM 5200.0 l , Volume 3, Enclosure 3, Para 2.a.( l).
"IDS Installation and Maintenance Personnel. Alarm installation and maintenance shall be accomplished by U.S. citizens who have been subjected to a trustworthiness determination (NACI) according to Reference (l)." The contractor shall provide information of individuals to government.
6. SMOKING, EATING AND DRINKING: The contractor shall permit smoking, eating and drinking only in designated areas.

7. DRIVING LICENSE/ON-BASE REQUIREMENTS: The contractor and employees shall comply with base traffic regulations at all sites. The contractor shall ensure employees have a current and valid state/country driver’s license for the type of vehicle being driven before allowing the employee to operate a personal vehicle on Hurlburt Field, Florida.
8. PROHIBITIONS: Firearms & illicit drugs may not be brought on base under any circumstances.
Individuals with criminal records and active warrants will not be allowed on base. Contractor personnel are prohibited from sponsoring guests onto the installation.
