Statement of Work Fall Restraint
East (Wash Rack) Side of Hangar 90700
1. SCOPE
The contractor shall provide all management, tools, supplies, equipment, and professional labor necessary to install and perform required tests on a fall arresting system in Hangar / Building 90700 on Hurlburt Field, FL.
2. BASIC SERVICES
2.1. The contractor shall provide professional services to install and weight test a fall arrestor system in Hangar 90700. The contractor is responsible for providing and connecting a 140 foot monorail system and connecting it to an existing hangar structure on the east side of the Hangar Bay.
2.2. The contractor shall be responsible for all personnel, equipment, tools, materials, supervision, storage, assembly, transportation, incidentals and all other items and services necessary to dismantle, remove, and dispose of all existing hardware and to provide and install systems for new equipment.
2.3. The Monorail system shall be a Gorbel Ceiling Mounted Monorail System, PN
TTMS500, or Equivalent.
2.3.1. The monorail system shall contain sloped hangers to account for ceiling considerations.
2.3.2. The monorail system to be installed shall be connected (clamped) to the hangar structure, approximately 140 foot coverage, centered above the C-130 wing when the aircraft is parked in the hangar using the ISO parking location.
2.3.3. The contractor shall provide six (6) trolley connections to attach to the monorail to allow six (6) maintenance technicians the ability to walk on top of the C-130 wing while performing inspections, maintenance and repair.
2.3.4. Each trolley will require a 45-50 foot fall restraint reel assembly with retract/pull down cord to allow technicians to retrieve the connection from the trolley to connect to restraint harness.
2.4. The system shall include a Gorbel Self Retracting Lifeline, PN 55” Cable SRL, or Equivalent.
2.4.1. The Self Retracting Lifeline shall have a 55 foot cable length, single leg, and steel snap hook connector.
2.5. The vendor shall manage ordering, shipment, delivery and installation of all equipment from manufacturer to final acceptance by the designated Government Acceptor.
2.6. Sizes are approximated, after contract award the contractor will be responsible for verifying measurements before materials are ordered to ensure the sizes are correct. Any variances in sizes that will affect price shall be brought to the Contracting Officer’s attention prior to ordering materials.
3. TEST AND INSPECTION
3.1. The contractor shall accomplish an operational check of the fall arrest system to ensure proper operation. The contractor shall provide the Government a copy of the operational check documentation.
3.2. After installation and assembly, the contractor shall inspect and load test to the 5,000 lbs
AFOSHA requirement per AFMAN 91-203 13.4.6.2 for fall restraint. The Government shall be present for inspection and load test.
3.2.1. AFMAN 91-203 13.4.6.2. states “Personal fall arrest systems equipment shall have the manufacturer‘s name or logo; part number and model designation; capacity rating; warning to follow the manufacturer’s instructions included with the equipment at time of shipment from the manufacturer stamped on the equipment or permanently attached tag and shall be marked to indicate compliance with American National Standards Institute Z359.1. (T-0)”
3.3. The contractor shall provide the Government a copy of the inspection and load test of the equipment installed.
4. WARRANTY
4.1. The system shall have a minimum one (1) year warranty on installation and workmanship and manufacturer’s commercial warranty on all items installed. The warranty period will start on the day the Government accepts the entire project as complete. During this warranty period, the contractor shall provide on-site technical support to repair, services, add, and/or replace any system related equipment and resolve any technical issues that may arise.
5. GOVERNMENT FURNISHED PROPERTY AND SERVICES
5.1. There will be no Government Furnished Property or Services provided for this effort.
6. HOURS OF OPERATION
6.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: MON-FRI 0700 to 1600, except for federal holidays.
The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the Government.
6.2. Recognized Holidays. The contractor is not required to provide service on the following days: New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.
7. BASE/INSTALLATION SPECIFICS
7.1. All contractors shall submit a complete list of personnel in accordance with installation policy, including deliverymen, who will be expected to work on the installation. This list will contain Privacy Act information as required as well as the expectation that each individual will be able to present a current and valid state identification, to meet local Air Force security requirements. This information shall be sent to the CO at least 10 days in advance prior to the start of installation.
7.2. The contractor shall not employ persons for work on this contract if such employee is identified to the contractor by the CO as a potential threat to the health, safety, security, general well-being or operational mission of the installation and its population. Contractor personnel shall present a neat appearance and be easily recognized as contractor employees.
7.3. All commercial trucks must enter through the Commercial Gate on Cody Ave off of Independence Road, Fort Walton Beach, FL for inspection. Prior to going through the Commercial Gate, contractor will have to obtain a visitors pass from the Visitors Center located off of HWY 98. In accordance with the Installation 1st SOSFS Visitor Control Center;
base entry procedures must adhere to delivery vehicle process requirements mandated by the 1st SOSFS. Vehicles are subject to inspection. To obtain a base pass from the Visitors Center, the driver will require:
A current Vehicle Registration A current Proof of Insurance A valid and current Driver's License for the driver A valid and current Driver's License or other State Identification for any passenger(s) in the vehicle.
A Base Pass Letter from the CO with the contract number, and individuals requiring base access. The base pass request must be submitted to the CO at least 10 days prior to scheduled delivery and include the names and driver’s license number of each individual requiring base access. The request shall also include the anticipated installation timeframe for this project.
7.4. Gate Procedures:
Once at Cody Ave. Gate, be prepared to exit the vehicle and allow for vehicle inspection. At this time you may ask for directions to your final destination.
If the final destination requires the driver to leave Cody Ave. Gate, and re-enter the base through an alternate gate, the pass received will be sufficient documentation for entry into both gates, within a pre-established time directed by the Security Forces representative.
Arrival at the appropriate gate after the designated allotted time will result in non-admittance to the installation.
This procedure will occur each time a commercial truck requests access to the base.
7.5. Current Bill of Lading (BOL) to get delivery driver and the vehicle on base. BOL must include the AF installation name, date of anticipated delivery, and specific destination (including building number, street address, and point of contact information).
7.6. Personnel should have the following items when entering an installation:
Current Registration for all vehicles Current Proof of Insurance for all vehicles Valid driver’s license for each driver Valid driver’s license or other state identification for each passenger Depending on the current Force Protection Condition, a government representative may be required to sponsor those people in person at Pass & ID/Visitors Center.
7.7. Prohibitions: Firearms & Illicit drugs may not be brought on base under any circumstances.
Individuals with criminal records are not allowed on base. Contractor personnel are prohibited from sponsoring guests onto the installation.
8. OTHER INFORMATION This requirement is necessary due to C-130 Maintenance Advisory 1C-130MA-20140718001, changing the fall restraint requirement when aircraft maintenance is being performed when a possible fall could occur past four (4) feet.
