OPTOMETRY STATEMENT OF WORK
1. WORK TO BE PERFORMED: The contractor will provide optometric services. to include routine eye examinations, assessment of corrective eyewear needs, and prescription of corrective lenses, as needed to the inmates. Contractor will report outcome of the examinations to the Health Services Administrator or designee.
2. PLACE OF PERFORMANCE: Lay Medical Building, Health Services Department, Federal Prison Camp, 1012 Douglas Avenue, Yankton. South Dakota.
3. WORK SCHEDULE: The work schedule will be normally consist of eight sessions (one session equals one hour) one month, and 16 sessions (one session equals one hour) the second month. A typical schedule would be one day per month, 7:30 a.m. to 4:00 p.m., and two days the next month.
7:30 a.m. to 4:00pm. This schedule is subject to some variation as the needs of the institution vary. A schedule that is mutually acceptable to the Government and the contractor can be determined after award of the contract. The contract period is anticipated to be from Oct 1, 2021 thru Sep 30, 2026. Under no circumstances will the contractor work more than 30 hours per week. Contract award is anticipated to be Oct 1. 2021, pending Irie availability of funds. Should funds not be immediately available, contract award for the fiscal year will be postponed until such time as funds become available.
4. MATERIALS AND SUPPLIES: All materials and supplies that the contractor may routinely need will be supplied by the Government. Any records maintained by the contractor under this contract will remain the property of the Government.
5. SCOPE OF SERVICE The contractor is required to
(a) Conduct routine eye examinations.
(b) Organize all diagnostic testing material/equipment and administer as appropriate.
(c) Interpret test results and prescribe corrective lenses as appropriate.
(d) Report outcome to Health Services Administrator or designee.
6. SPECIAL QUALIFICATIONS REQUIRED:
(a) Must have a degree in optometry from an accredited school of optometry.
(b) Must have, or be eligible for, a license to practice optometry in the State of South Dakota.
(c) Shall maintain during the term of this contract liability insurance issued by a responsible insurance carrier of not less than $1 million per occurrence.
7. INMATE MANAGEMENT: The provisions of this contract require frequent and unsupervised contact with inmates. The contractor will not be responsible for the management of inmates other than to ensure that those inmates involved in activities within the contractor's scope of work shall abide by the rules in effect to ensure their safety and well being. Any infraction to the rules and regulations on the part of an inmate will be reported to the Health Services Administrator or a Correctional Supervisor.
8. INSTITUTION SECURITY: The contractor agrees to adhere to all regulations prescribed by the institution for safety, custody, and conduct of the inmates. The contractor shall attend a four hour orientation training program before assuming his/her duties. Reimbursement to the contractor for Irie time spent in institution security orientation or other periodic security related training will be at the contract rate. prorated hourly. The following investigative procedures will be applied and appropriate forms completed before the contract is awarded:
(a) National Crime Information Center (NCIC) Check;
(b) FD-258 Fingerprint Check;
(c) Law Enforcement Agency Checks;
(d) Vouchering of employers over the past 5 years;
(e) Resume/Personal Qualifications;
(f) BP-A0660 Authority for Release of Information; and
(g) NACI check (if applicable).
A urinalysis is required for detection of marijuana and other drug usage (if test is positive the contractor will be disqualified from being awarded the contract).
9. EVALUATION CRITERIA: Attached.
10. OTHER INFORMATION:
(a) This service is a contractual arrangement and not a personnel appointment;
(b) Payment is based on the provision of an end product or the accomplishment of a specific result;
(c) This service does not constitute an employer/employee relationship; and
(d) The contractor will not be subject to Government supervision, except for security related matters; however, the contractor's performance will be closely monitored.
11. PER FAR 37.401:
(a) The contract is a nonpersonal health care services contract under which the contractor is an independent contractor;
(b) The Government may evaluate the quality of professional and administrative services provided, but retains no control over the medical, professional aspects of the services (e.g. professional judgments, diagnosis for specific medical treatment);
(c) The contractor shall indemnify the Government for any liability producing act or omission by the contractor, its employees and agents occurring during contract performance;
(d) The contractor shall maintain medical liability insurance in accordance with the attached clause 52.237-7;
(e) The contractor shall ensure its subcontracts for provisions of health care services contain the requirements of clause 52.237-7, including the maintenance of medical liability insurance.
