Statement of Work

REVVITY ENSIGHT MULTIMODE DETECTOR

GENERAL INFORMATION

1.0 Scope of Work: Contractor will provide factory trained engineers to perform one

(1) preventive maintenance service visit and unlimited emergency visits on the Revvity Ensight Multimode Detector, SN: 3400S1709131, located at 1920 Dayton Ave, Ames, Iowa.

Period of Coverage: Base + 4 Option Years contract to begin

12 months from Date of Award

2.0 Background: This machine was transferred from another laboratory and an OEM service check/repair was performed May 2024. This action is to re-establish a service agreement to ensure and maintain reliable operation in support of the NCAH research mission.

CONTRACTOR REQUIREMENTS

3.0 Technical Requirements/Tasks:

Service tasks include, but are not limited to:

1) Thorough cleaning of instrument, inside and out.

2) Operational baseline establishment

3) Realignment of filters/lasers if necessary.

4) Functionality and operations check.

5) Replacement of any faulty or worn parts.

Contractor will provide unlimited telephone support during normal business hours.

Contractor will provide unlimited emergency visits to the customer location within 3 business days of notification.

Repair parts and travel expenses are to be included unless otherwise stated in the approved Plan outline.

Only OEM factory trained service engineers will service the stated multimode detector using OEM replacement parts.

4.0 Government Furnished: Government-furnished support is limited to security access to the equipment in the NCAH facility, AC power (110VAC) and standard office lighting.

6.0 Travel: One (1) preventive maintenance service visit will be provided by the contractor. All travel shall be covered under this service agreement. All service visits will be coordinated with the laboratory at NCAH.

7.0 Contractor’s Key Personnel: Only factory trained technicians are to perform any service procedure on the Revvity Ensight Multimode Detector.

8.0 Security Requirements: A copy of an official government photo ID (State-issued driver’s license or ID card, passport, green card, etc.) must be provided no later than one week before arrival of contract service technician(s) except in the case of emergency service, were only the name of the technician provided via email or telephone will be necessary. Security access to the NCAH facility will be provided based on this identification requirement.

9.0 Place of Performance: 1920 Dayton Ave, Ames, Iowa 50010.
