SCOPE OF WORK

Service Contract for SPD RO Unit PM

I. Purpose of Contract:

The purpose of this contract is to obtain a service contract for the Sterile Processing Department (SPD) RO (Reverse Osmosis) Unit PMs.

II. Objective:

The contractor will provide routine maintenance service for the RO Unit located in the SPD.

Period of the contract will be for five years (Base + 4 years).

III. Government Furnished Property, Facilities, and Services

The Red Lake Hospital will provide access to the Sterile Processing Department (Working hours Monday - Friday 7am-5pm).

IV. Contractor Furnished Equipment, Facilities, and Services

The Contractor shall provide service to the Red Lake Hospital located at 24760 Hospital Drive Red Lake, MN 56671. The contractor is responsible for servicing the RO Unit located in the SPD.

V. Statement of Work:

The contractor (Lindsey Crystal Pure Water) will provide preventative maintenance and repair services for RO equipment located in the Sterile Processing Department at the Red Lake Hospital. The equipment shall be routinely serviced (per manufacturers recommendations). Nustream Filtration model LB30 serial number 1353, Nustream Filtration model CGD14, serial number 1352.

VI. Technical Requirements:

Sterile Processing Department: Reverse Osmosis Equipment

1. Loop, Storage Tank and RO Disinfect (Quarterly)

2. Slimline Sediment Pre-Filter (Quarterly)

3. DI Tank Exchange (Quarterly)

4. 2.5X20 Endotoxin Filter (Bi-Yearly)

5. UV Bulb Exchange (Yearly)

6. 0.5 Micron Vent Filter (Yearly) Vendor – will provide service including parts, labor and travel to perform:

1. Scheduled preventative maintenance in accordance manufacturers recommended schedule

V. Additional Requirements:

Vendor shall inform BioMed personnel of findings before leaving the Service Unit (each visit):
