STATEMENT OF WORK (SOW)

Project Number, Title: VGLZ202001 Demolish Bldg. 972 and Site Work 15 May 2020

1.0 GENERAL INSTALLATION REQUIRMENTS

1.1 The General Specifications and Selfridge Design Standards 10 Nov 2017 apply to this Statement of Work (SOW). The work shall be in strict compliance with those specifications and the requirements identified in this SOW and its attachments. Sections of this SOW may supersede the requirements of the specifications, in which case the more stringent/higher quality requirements shall apply. This SOW provides for selective upgrades and repair of exterior lights systems and electrical power systems components and includes, but is not limited to:

1.1.1 The Government will provide existing building drawings and site plans.

2.0 DESCRIPTION OF SERVICES

2.1 The purpose of this Statement of Work is to allow the use of measurable industry Based Standards in a competitive environment. The work metric will be based entirely on the Contractor’s ability to provide the Government with the most comprehensive, efficient, durable, and long-term solution, while achieving the highest possible level of quality industry standards.

3.0 SCOPE OF WORK

3.1 The Contractor shall provide all labor, materials, and supervision necessary to demolish and to dispose of facilities, various structures, roads, railroads, sidewalks, and utilities; the Contractor shall restore the site following demolition, re-grade and establish grass groundcover on the sites as described below. Demolition includes, but is not limited to, buildings listed in Table 1.01 below and as defined specifically on the information drawings provided. The Contractor shall be held responsible for all requirements described in the SOW, contract documents and all work, including that of his subcontractors, if any. All work shall be done in accordance with the contract documents. Failure to familiarize oneself with their requirements will not relieve the Contractor of his responsibility to comply with contract document requirements. The organization of the work, specifications into divisions, sections and articles, and the arrangement of drawings shall not control the Contractor into dividing the work amongst Subcontractors or in establishing the extent of work to be performed by any trade. All work performed under this SOW shall be conducted in accordance with all applicable State and Federal laws, regulations, and codes, ANG policies and regulations, General Specifications, and the SOW as outlined.

This table shows buildings that are part of VGLZ202001 project, with Sq. F.T. as approximate values. Total Sq. Ft. of the Demolition site will include these buildings and various ancillary facilities defined on the informational drawings. All areas should be field verified by contractor. Contractor shall remove all buildings foundations/slabs and associated sidewalks, parking lots, drives, and curbs as shown on plans; additionally, various site amenities such as power poles, pad mounted transformers, trees, shrubs, bushes, and parking blocks are required to be removed.

4.0 DEMOLITION

4.1 Contractor shall provide for the complete demolition of the specified facility items, all horizontal items, and utilities items, as shown on plans and itemized on Table 1.01.

4.2 Demolition of the buildings shall be by trade acceptable demolition methods. The use of explosives or fire is not acceptable on SANG Base. Contractor is responsible to implement protective dust control measures during demolition operations, and must keep all debris secured on jobsite.

4.3 The demolition method and equipment use shall be approved/disapproved by the Contracting Officer prior to demolition.

4.4 The Contractor shall provide for the complete removal of all material, fixtures and equipment that is contained within demolished facilities.

4.4.1 All materials, such as, but not limited to lumber, brick, broken concrete, pavements, masonry, glass, old roofing, siding, sheet metal, fixtures and equipment shall be removed and legally disposed of off base as the work progresses in a state-approved landfill, at no additional cost to the Government. The contractor must make all efforts to recycle material such as scrap metal.

4.4.2 Dispose all non-salvageable debris in Type II landfill.

4.4.3 The Contractor must report to the Government the amounts and destinations of all materials recycled and/or disposed of no less than quarterly

4.4.4 All materials shall be disposed of as specified in Hazardous Materials Removal. Transportation and packaging of all materials removed from site must be in accordance with 49 CFR 100-177, as well as state and local requirements.

4.5 The Contractor shall properly remove and terminate existing utilities and electrical services, domestic water and sewers will be properly abandoned, removed and terminated for all demolished facilities. The Contractor shall properly plug sanitary building-services with concrete. Domestic water supply lines to building and hydrant are to be terminated with mega lugs and thrust blocks at the mains. Hydrant is to be removed complete. Electrical utility services conductor shall be removed back to second closest existing utility pole and cap / terminated per NEC code requirements. Storm water catch basins are to be removed complete. Intersecting pipes are to be properly capped with concrete and abandoned in place.

4.6 The Contractor shall provide for the complete demolition of the building site. This includes, but is not limited to the removal of all designated horizontal site development, such as pad mounted transformers, foundations/slabs, associated sidewalks, railroads, parking lots, fences, power poles, trees, shrubs, bushes, debris piles, and parking blocks / bollards are required to be removed.

4.6.1 Concrete:

4.6.1.1 Any/all concrete including reinforced concrete or block foundation and footings shall be removed completely to a minimum depth of 60”.

4.6.1.2 Concrete and asphalt shall be broken using equipment normally accepted for this use.

4.6.1.3 Remove all building/facility sites' concrete and asphalt. Such removal includes, but is not limited to foundations, stairs, slabs, ingress/egress walks and adjoining drives and parking lots, unless noted otherwise on drawings and specifications.

4.6.1.4 This contract also includes the removal of all roadways indicated on the drawings, as well as ancillary pavements associated with bus stops, dumpster pads, storage facilities, etc.

4.6.1.5 The Contractor is solely responsible to verify the extent of removal needed to expose any utility terminations prior to bid.

4.6.2 Trees and Shrubs:

4.6.2.1 Trees and shrubbery containing active bird nests for birds protected under the Migratory Bird Act cannot be removed. Removal of such vegetation will need to be postponed until all fledglings have left the nest, at such time the vegetation can be removed.

4.6.2.2 Per the United States Fish and Wildlife Service (USFWS), Selfridge ANGB should avoid conducting tree removal activities during the interval starting 15 April and ending 15 August to minimize the potential to take any migratory birds.

4.6.2.3 All cut trees, shrubbery, and wood debris from activities must be disposed of or recycled within the current quarantine area specified by the MDA and USDA.

4.6.3 Railroads:

4.6.3.1 Railroads shall be removed complete to include, but not limited to tracks, control implements, wooden ties, and ballast.

4.6.3.2 The Contractor must make all efforts to recycle track rails and control implements materials as scrap metal.

4.6.3.3 Ballast shall be removed to a depth of 6” below adjacent grade areas. Ballast shall be considered contaminated media and handled appropriately.

4.6.3.4 All materials, such as, but not limited to lumber, brick, broken concrete, pavements, masonry, glass, old roofing, siding, sheet metal, fixtures and equipment shall be removed and legally disposed of off base as the work progresses in a state-approved landfill, at no additional cost to the Government. The Contractor must make all efforts to recycle material such as scrap metal.

4.6.3.5 Dispose all non-salvageable debris in Type II landfill.

4.6.3.8 Following the removal of the track, the Contractor shall patch the portion of Luke Rd where the railroad track was removed. Pavement markings will be repainted as required to provide continuous markings along the road.

4.6.3.9 Asphalt patch compositions is as follows:

4.6.3.9.1 Compacted subgrade, four passes

4.6.3.9.2 6” of 3/4”crushed stone / Road Base. Compacted to 98%

4.6.3.9.3 Bituminous paving, binding course, 2” thick

4.6.3.9.4 Bituminous paving, wear course, 2” thick

5.0 SITE RESORATION

5.1 The Contractor shall re-grade and establish a flourishing grass groundcover on the sites as described below to achieve site stabilization.

5.1.1 The Contractor shall restore the site surrounding the removed railroad tracks. Contractor to complete all final grading to insure adequate drainage is accomplished for the site area disturbed. Refer to section 32 92 00 – Turf and Grasses – Non Airfield, in the general specifications during vegetative seeding.

5.2 The site will not be considered stabilized until the Macomb County Soil Erosion permit for the project is closed. At the completion of the demolition and removal operation, the Contractor shall fill the area to -4’’ below anticipated Final Grade.

5.3 Suitable Sub Grade material may be made available by the Government, from an on-base location that will be determined after awarding of contract.

5.4 Any barrow location will need to be identified on the soil erosion plan developed by the Contractor, and the Contractor shall also be responsible for barrow location site grading and stabilization.

5.5 The Contractor will be responsible for adding an additional 4’’ of Certified Top soil to establish final grade using the below guidelines.

5.6 Certified Topsoil is defined by high organic material, low salts, high nutrient levels, and free of debris or any environmental contamination that exceeds Residential Part 201 and Part 213 Cleanup Criteria and Risk-based Screening Levels.

5.8 Contractor to provide Topsoil Analysis/Testing: Performed by County or State soil testing service, or approved certified independent soils testing laboratory. Contractor to provide 3 copies of testing results to Government for approval prior to placement. Test results must include sample date and reference the collection location.

5.9 Placed topsoil shall be stabilized immediately by:

5.9.1 Hydro-seeding with the approved seed mixture and/or mulch matting.

5.9.2 Installing perimeter silt fence or other controls as approved on Soil Erosion Permit.

5.10 All Topsoil shall be screened prior to installation, and will be free from objects larger than 3/4 inches maximum dimension including hard clods of heavy clay, shale, rocks, decomposed shale or other subsoil, noxious weed parts (roots, seeds, or shoots), grass, refuse, stumps, roots, brush, other foreign matter, hazardous or toxic substances, and deleterious material that may be harmful to plant growth or may hinder grading, planting, or maintenance. Leftover debris from the screening process will be disposed off Base Property by the Contractor.

5.11 Contractor to complete all final grading to insure adequate drainage is accomplished for the site area disturbed.

5.12 Entophytic Fescue is required to be used in all Airfield areas. MDOT Class A seed mix is acceptable for use in all other locations. An as built figure showing locations where Entophytic Fescues are established shall be provided to the Government upon completion of the contract by the Contractor.

5.13 The Contractor is also responsible for insuring irrigation measures are deployed, maintained, and operated on a scheduled basis to insure new seed growth is established in a timely manner, specifically from 15 MAY to 15 SEP.

ADDITIONAL PROJECT REQUIREMENTS:

6.0 MAINTAINING TRAFFIC

6.1 Vehicle and pedestrian traffic will be required along the roadways adjacent to demolition sites at all times. Protection to persons, vehicles and property shall be provided through the progress of the work by the use of, but not limited to, barricades placed 10 feet on center. No excavations are permitted to remain open or exposed overnight without closing off the areas and establishing a safe perimeter with barricades and/or snow fencing. Lighted barricades will be required around heavy traffic areas, as specified in Engineering Design Specification. Lighted barricades will be required around demolition sites that adjoin parking lots, roadways, sidewalks and pedestrian walkways.

7.0 SCHEDULING OF WORK

7.1 On Base work hours for the contractor will typically be Monday thru Friday, between the hours of 7:00 a.m. and 5:00 p.m., excluding Saturdays, Sundays and Federal Holidays. If the contractor desires to work during periods other than above, he must notify the Contracting Officer three days in advance of his intention to work during other periods to allow assignment of additional inspection forces. When the Contracting Officer determines that they are reasonably available, he may authorize the Contractor to perform work during periods other than normal duty hours/days; however, if Inspectors are required to perform in excess of their normal duty hours/days solely for the benefit of the Contractor, the actual cost of the inspection, at overtime rates, will be charged to the Contractor and will be deducted from the final payment of the contract amount. The Contractor shall inform the 127th Contracting Office at least 5 days in advance of ANY site visit to insure base personnel are availability to prepare for Contractor visit, evaluation, or repair work.

8.0 STORAGE

8.1 The Government will provide no secure storage space. The Government will not be responsible for property belonging to, or under the present control of the Contractor. The Contractor is to protect his materials. An unsecured, open staging area will be designated by the Contracting Officer for storage of construction equipment and materials during the period covered by this contract. Any on-site storage of Petroleum Oil and Lubricants (POL) to include, but not limited to, gasoline, diesel, motor oil, etc., must be stored in secondary containment if the storage containers are 55 gallons or greater.

9.0 USE OF UTILITIES

9.1 The Contractor may use a reasonable amount of water and electricity for this construction operation without charge. With prior coordination with the Contracting Officer and Base Fire Department, the Contractor shall be given access to water hydrants to obtain water for construction activities. The Contractor shall provide and install a double back flow preventer with meter to all water hydrants made available to them for project use. Any expense incurred to gain access to these utilities (temporary tap-ins, etc.) shall be the responsibility of the Contractor and all utilities shall be returned to their original configurations at the end of the contract. No alterations to the existing utilities shall be accomplished without the written permission of the Contracting Officer. Do not use this electricity or any gas connection provided by this contract for temporary heating without the approval of the Contracting Officer.

10.0 SOIL EROSION

10.1 Throughout all phases of contract execution, the Contractor shall use best practices with regard to soil erosion.

10.1.1 The Contractor shall develop the site-specific soil erosion plans and submit them to the Government and Macomb County Soil erosion office for approval. This erosion plan shall covering all activities under this project.

10.1.2 Prior to submitting the soil erosion plan to Macomb County, it will need to be submitted to the Government for review and approval regarding proposed grades. The Contractor will be responsible for submitting the soils erosion plan to Macomb County and obtaining approval of the plan from Macomb County. The project plans provided only include basic information. The Contractor will be responsible for developing separate plan sheets or modifying existing plan sheets to comply with Macomb County Soil Erosion plan requirements, guidance documentation concerning soil erosion plan requirements can be obtained from Macomb County Soil Erosion Office.

10.1.3 The follow list includes some general items missing on the plan, which the Contractor will be required to add. Please note this is not an all-inclusive list:

10.1.3.1 North Arrow is not provided on all plan sheets.

10.1.3.2 Wetland Boundaries are not identified on the plans (GIS data for existing wetland boundaries has been provided by the installation). No excavation or grading work can occur in wetland under this project. Wetland areas adjacent to project areas shall be protected by the Contractor.

10.1.3.3 Storm water catch basins are not identified on the project plans.

10.1.3.4 Contour data is not provided on the plans (LIDAR data for the installation has been provided and is accepted by Macomb County for use as existing contours).

10.1.3.5 Ditches/swales in proximity to the project site are not identified on the plans.

10.1.3.6 Proposed grades must be shown on the plan. This would be required for any areas that grading will be revised. (The plans specify this will be accomplished at the west munitions location). Grading plans developed by the Contractor should be submitted to the Government, as soon as possible during design for approval.

10.1.3.7 Areas disturbed under the project are not shown on the plans.

10.1.3.8 Various other plan requirements to include soil erosion maintenance notes, controls, details, etc. are not on the plans. The Contractor shall refer to Macomb County Soil Erosion Plan requirements and incorporate all missing data into the site specific plan developed by the Contractor. Gathering and collecting any necessary data to develop the soil erosion plan would be the responsibility of the Contractor.

10.1.4 Once approval of the soil erosion plan is obtained, the Contractor will be required to obtain the soil erosion permits and incur costs of applicable State and Local permits necessary to start work activities.

10.1.5 The Contractor shall furnish all fees, labor, materials, equipment and supervision necessary for Best Management Practices (BMPs) of erosion controls and compliance with applicable permits. All BMPs shall be developed in accordance with the Guidebook of Best Management Practices (BMP) for Michigan Watersheds. All BMPs will be installed and maintained in accordance with this book and any manufacturer instructions. Erosion controls shown on the plans are basic controls only. If a BMP is found to be inadequate for the site, the Contractor should notify the Government and the Macomb County Soil Erosion Office of the concern and corrections made on site to address the concern. The Government and/or the Macomb County Soil Erosion Office may require additional measures to address the concern.

10.1.5.1 The Government will develop the site-specific soil erosion plans and submit them to Macomb County Soil erosion office for approval. Once approval is obtained, the Contractor will be required to obtain the soil erosion permit and incur costs of applicable state and local permits necessary to start work activities.

10.1.5.2 The Contractor shall furnish all fees, labor, materials, equipment and supervision necessary for Best Management Practices (BMPs) of erosion controls and compliance with applicable permits. All BMPs shall be developed in accordance with the Guidebook of Best Management Practices (BMP) for Michigan Watersheds. All BMPs will be installed and maintained in accordance with this book and any manufacturer instructions. Erosion controls shown on the plans are basic controls only. If a BMP is found to be inadequate for the site, the Contractor should notify the government and the Macomb County Soil Erosion Office of the concern and corrections made on site to address the concern. The Government and/or the Macomb County Soil Erosion Office may require additional measures to address the concern.

11.0 ENVIRONMENTAL SOIL EROSION CONTROL SEQUENCE OF EVENTS

11.1 All requirements for soil erosion control will be in accordance with 40 CFR 122; Michigan Act 451 (1994) parts 91 and 31; and the Macomb County Soil Erosion and Control Ordinance. Although some significant requirements are listed in the following, it is the responsibility of the Contractor to be completely familiar with the code requirements and to comply with all requirements therewith.

11.2 All BMPs shall be developed in accordance with the Guidebook of Best Management Practices for Michigan Watersheds. All BMPs will be installed and maintained in accordance with this book and any manufacturer instructions. Erosion controls shown on the plans are basic controls only. The field engineer and or the Macomb County Soil Erosion Office may require additional measures.

11.3 All streams and drains, to include the main stream or trunk and all tributaries or branches of any creek or river, any watercourse or ditch, either open or closed, any covered drain or storm sewer or conduit and any structure or mechanical device that will introduce flow of water into said drain, will be protected from soil infiltration.

11.4 When earth changes or the limits of the demolition site are within 100 feet of a wetland, the wetland will be protected using the appropriate BMP(s) as near as practical to the demolition site.

11.5 All soil erosion and sedimentation control measures will be installed prior to or in conjunction with the start of excavation and are to remain in effect until areas are stabilized. After stabilization has occurred removal of all BMP's will be the responsibility of the Contractor.

11.6 The Contractor will instruct drivers to remove soil materials from tires on site. Streets surrounding the demolition site must be kept clean at all times.

11.7 To protect streets, a vegetative buffer strip (grass) at least 50' wide will be maintained between the street and exposed areas. The strip will act as a filter. Where buffers cannot be maintained, additional BMPs will be required.

11.8 Seeding or sodding of exposed buffer strip must be completed immediately after final grading. Whenever the completion date falls during a dormant season, the site will be straw mulched. Within 15 days of the beginning of the next growing season, grass will be installed. Disturb only the area needed for demolition.

11.9 The demolition site will be inspected by a Contractor employed Michigan Certified Storm Water Operator once per week and within 24 hours after precipitation events that result in a discharge from the site and ensure that any needed corrective actions are carried out. The Contractor’s soil erosion inspection logs must be maintained at Selfridge, up to date, and readily available for Government and regulatory inspections. A notification of where logs are being kept must be submitted by the Contractor to the Contracting Office. If the location changes any updates should be made immediately by the Contractor to the Contracting Officer. A copy of all inspections will be provided to the Contracting Officer by the Contractor immediately following close out of the soil erosion permit and kept for a period of three (3) years after the project is terminated.

11.10 The Contractor must submit the permit application to Macomb County and pay the permit fees. A copy of the approved soil erosion permit provided to the Contractor by the County must be provided to the Government before the Base Dig Permit will be signed, allowing excavation work to begin. The Contractor will also pay for State of Michigan NOC (Notice of Coverage) fees. County permits are in effect for a period of 6 months, 1 year, or 2 years, and they must be renewed by the Contractor until disturbed areas under the project are declared stabilized by Macomb County Erosion. Controls are only to be removed by the Contractor per verbal or written direction of Macomb County.

12.0 SITE SPECIFIC PERMIT & PAYMENT

12.1 Macomb County Soil Erosion and Sedimentation Control Permit is to be site specific in the base name with the Contractor as agent, (on-site responsible party), and payment of fee is to be the responsibility of Contractor. The NOC is to be site specific in the base name with the Contractor as agent, and payment of fee is to be the responsibility of Contractor. Permits will be renewed by the Contractor until disturbed areas under the project are declared stabilized by Macomb County.

12.2 Macomb County Soil Erosion and Sedimentation Control Permit are to be site specific in the base name with the Contractor as agent, (on-site responsible party), and payment of fee is to be the responsibility of Contractor.

13.0 ENVIRONMENTAL RECORDS OF COMPLIANCE

13.1 The Contractor is responsible for record keeping, log books and certification practices to ensure the work is in compliance with Federal, State and local code requirements.

13.2 MAINTENANCE NOTES

13.2.1 The demolition site(s) will be inspected by a CONTRACTOR EMPLOYED Michigan Certified Storm Water Operator, as defined in 3.04.10 once per week and within 24 hours after a precipitation event, or as needed. Soil Erosion Inspection Logs must be maintained by the contractor, readily available on site, and provided to the government upon close out of the soil erosion permit.

13.2.2 Any excess dirt or mud tracked onto public roads due to construction traffic or run-off passing other erosion/sediment control devices shall be cleaned immediately with a stiff bristle broom or square point shovel. If these methods do not control the dirt or mud tracking onto the public roads, Contractor must hire personnel and equipment to clean roads adequately.

13.2.3 Silt fence should be trenched-in, back-filled, and stapled or staked according to the manufacture’s specifications. Maintenance includes the removal of built-up sediment when the sediment accumulates to 1/3 the height of the fence. Contractor should take any steps necessary to repair fence if it fails, including removing, replacement, retrenching, or re-back-filling the fence. It would also be necessary to reinstall if any portions of the fencing are damaged in any way that prevents the silt fence from functioning as an adequate erosion/sediment control device.

13.2.4 Inlet filters should be inspected for buildup of silt and other debris. This is evident if geo-textile/stone structure is causing standing water. Inlet filters should also be inspected for tears resulting in holes letting sediment pass by the geo-textile filter and to insure filter meets the manufacture’s specifications for installation and placement. Maintenance would consist of removal of sediment with a stiff bristle broom or square point shovel. If inlet filter is beyond this level of repair it should be replaced immediately.

14.0 HAZARDOUS MATERIAL REMOVAL

14.1 LEAD MATERIAL/CONTAMINATION/LEAD BASE PAINT (LBP)

14.1.1 The Contractor is to dispose of items containing lead, or LBP, in buildings defined in Attachment 2, and including any additional demolition items discovered by Contractor to contain the same contaminants. Removal and handling procedures are to be in accordance with Federal, State and Local requirements, including

14.1.1.3 29 CFR 1910.1206 HAZWOPER Standards

14.1.2 Lead-based demolition items are expected to be removed in whole (e.g., windows, doors, wall, ceiling, etc.) or broken down only to sizes that are necessary for removal with typical large demolition machinery. Then the debris can be disposed of in a Type II municipal landfill. Landfill owners must be informed of such disposal and applicable permits obtained. If disposal facilities will require any testing prior to disposal this will be the responsibility of the Contractor. A copy of all analytical reports on waste streams required by disposal facilities must be provided to the government. If the disposal facility selected by the contractor will require profile sheets for disposal, they must be completed by the Contractor and provided to the Government for approval and signature.

14.2 CONTAMINATED SITES IN CONSTRUCTION AREA

14.2.1 Areas of concern (AOCs) within the project area have known media (soil and groundwater) contamination. A table listing contaminated sites associated with demolition facilities is provided in the table titled Listing of Contaminated Site Associated with Facilities to be demolished. A listing of documentation for reports pertaining to AOCs impacted under this project is provided in the table titled Document Listing of Areas of Concern Impacted. These documents are available in electronic format and have provided along with the specification as guidance and reference materials. The Contractor will need to comply with all state requirements for media relocation during work activities under Section 201120c of Part 201, Environmental Remediation, of the Natural Resources and Environmental protection Act, 1994 PA 451, as amended (Act 451), and other applicable state and federal regulations if contaminated media will be relocated to or from these AOCs. Soil relocation or grading in these areas will need to be approved by the government, as remedial action have not been implement for several of these sites (i.e. relocation or placement of soils on site could exacerbate future remedial actions for the sites). If contaminated ground water is present and the contractor anticipates that dewatering will be necessary. The Contractor must provide a dewatering plan for review and approval prior to accomplishing any dewatering within the boundaries of identified contaminated sites with media relocation restrictions. The Contractor is responsible for all costs associated with the proper management of contaminated soil and groundwater to include; containerization, characterization, transportation, and proper disposal, as necessary to accomplish project requirements. Activities should be plan to minimized impacts to and handling of contaminated media.

14.2.2 The Contractor is responsible for developing site specific health and safety plans addressing personnel exposures that could be encountered during demolition activities in compliance with all OSHA/MIOSHA regulations on these sites. The health and safety plan will need to address any controls to be implemented by the Contractor to prevent exposures to base personnel during the demolition activities. The Contractor will be required to notify base personnel and/or Subcontractors working on site regarding the health and safety plan, and have all personnel on site to include government personnel received a briefing on the health and safety plan prior to entering construction areas where contaminated soils are present. Documentation of such briefings will need to be maintained by the Contractor and provided to the Government at the end of the contract. Where levels of contamination in work activities could result in potential exposure above action levels, exclusion zones will be set up by the Contractor to prevent any non-qualified personnel from entering the area.

14.2.3 Monitoring wells are currently installed and additional monitoring wells are in the process of being installed at several AOCs within proximity to the demolition activities under this project. The contractor is responsible for locating all wells on their construction sites, and marking them so that they are not damaged during construction. Any monitoring wells damaged by the contractor will need to be properly abandoned and be replaced in kind by the contractor following coordination and approval through MDEQ, NGB, 127WG/CEV, and COR, at no additional cost to the Government.

14.3 LIGHTING BALLASTS AND PCB’s

14.3.1 Demolition facilities have lighting fixtures with ballasts. PCB’s are commonly found in lighting starter ballasts. Unless the ballasts are labeled, “Non-PCB” they should be considered to contain PCB’s and be disposed of as such in accordance with local, state and Federal regulations.

14.4 CLOSURE REQUIREMENTS FOR 90 DAY HAZARDOUS WASTE STORAGE FACILITIES

14.4.1 N/A

14.5 OTHER HAZARDOUS MATERIALS

14.5.1 Major, known hazardous materials/wastes have been defined where possible. The Contractor will be required to remove an underground Oil/Water Separator (OWS), located at the N/E corner of Bldg. 891, as part of the this Demolition Contract. Contractor to fully comply with the below Guidelines.

14.6 PACKAGING AND TRANSPORTATION

14.6.1 Packaging and transportation of all materials leaving the site must be done in accordance with 49 CFR and all equivalent State and Local requirements. If requirements are not the same, Contractor is to follow the most stringent requirements. All hazardous and non-hazardous waste manifests, to include asbestos bill of lading forms, must be prepared by the Contractor, and approved and signed by the Government.

14.7 CONFINED SPACE AND HAZWOPER TRAINING

14.7.1 The Contractor is required to practice all necessary OSHA procedures to insure workers are safe and HAZWOPER trained. The Contractor must follow 29 CFR 1910.146 and use Air Force Form 1024 when making an entry into confined spaces and during grubbing/digging of underground pipe or facilities. Comply with 40 CFR 261 for hazardous material. Tank demolition and grubbing/digging of underground pipe are possible confined space hazards.

15.0 WARRANTY:

15.1 The Contractor shall warranty the site work for a minimum 1 year period. The Contractor shall respond to site performance problems for 12 months after the date of project completion.

15.2 All as-built drawings shall be provided in PDF & AutoCad format.

16.0 QUALITY ASSURANCE:

16.1 The COR will assess the contractor’s performance through intermittent on-site inspections of the contractor’s quality control system. The Government may inspect each task as completed or increase the number of quality assurance inspections if deemed appropriate.

17.0 GOVERNMENT FURNISHED EQUIPMENT AND MATERIALS

17.1 There is no government-furnished equipment or material for the commodities buy, design or incidental installation for this project. Copies of lighting drawings plan shall be made available from Point Of Contact, at Base Civil Engineering Office. The Government does not guarantee the accuracy or adequacy of the existing as built drawings. It is the contractor’s responsibility to verify all drawings and existing as-built conditions. All Contractors observed discrepancies between drawings, specifications and existing conditions shall be referred to the Contracting Officer before affected work is performed. Failure to make such notification shall place responsibility upon the contractor to carry out the work in a manner acceptable to the Contracting Officer at no additional cost to the Government.

18.0 GENERAL INFORMATION

18.1 HOURS/DAYS OF OPERATION: The contractor shall coordinate work dates and times with COR. Typically, contractor working hours shall be from 0700 – 1700 hrs. Monday through Friday unless the installation has specific requirements as outlined in the contract documents. Work on Federal Holidays and base down days requires approval of the contracting officer. There will be no staff to allow for access or provide coordination on those days.

18.2 SECURITY: The Contractor and all employees shall comply with all installation security requirements imposed by SANGB installation commander at all times while on the premises as outlined in the contract documents. The Contractor shall provide an up-to-date roster of Contractor’s personnel, by employee name and position title, authorized to work on the base to the Contracting Officer/COR at the pre-performance conference meeting. The Contractor shall provide to the contracting officer/COR a Selfridge ANGB Pre-Authorization Access Request List on company letterhead with the names of all employees who will require base access. The access list will contain the following information: Employee’s full name, date of birth, driver’s license number and state of issue.

18.3 ESCORTS: All personnel must qualify for unescorted base access during the construction period. Some of this work is to be accomplished in secure areas and will require the Contractor to coordinate independent escorts with the POC to complete the task.

18.4 BADGES: The Contractor shall obtain and monitor assigned security badges (used by both prime contractor and subcontractor staff) used during the duration of this contract. All security badges or passes shall be returned to COR upon expiration of the badge, upon completion of the project, or when possession of the badge is no longer necessary (e.g., upon removal of contracted personnel from specific projects).

18.5 PERSONNEL ROSTER: The Contractor shall provide a personnel roster to the contracting officer consisting of names, social security numbers, and security clearance confirmation no later than 10 days after the contract award date. The personnel roster shall be updated as individuals are removed and added to roster.

18.6 PHYSICAL SECURITY: The Contractor shall be responsible for safeguarding all government property in the work area. At the close of each workday, the contractor shall secure facilities and equipment.

19.0 SAFETY:

19.1 All appropriate safety measures (as outlined in Occupational Safety and Health Administration (OSHA) and Air Force Occupational and Environmental Safety, Fire Protection, and Health (AFOSH) guidance) will be utilized and adhered to throughout this project.

19.2 Maintaining Work Area: The Contractor shall be responsible for immediately cleaning up any spills, debris, etc., which may occur during work. The Contractor shall be responsible for keeping the work area clean. The site conditions shall be returned to original conditions or better. All safety precautions, measures and procedures shall be in place. Installation materials and equipment shall not be stored in the site during non-working hours except when the using agency provided a written approval allowing storage of such materials and equipment on approved location on site.

20.0 PLACE OF PERFORMANCE:

20.1 Primary location of work performance is SANGB as indicated in project documents.

21.0 PERIOD OF PERFORMANCE:

21.1 Contract is for 4 months (120 days) from time of award. All work will be completed within that timeframe.

22.0 EMPLOYEES:

22.1 Contractor personnel shall either wear distinctive clothing bearing the name of the company or appropriate badges that contain the company name and employee name at all times while performing work on base.

22.2 The Contractor shall not employ any person who is an employee of the US Government if employing that person would create a conflict of interest.

22.3 The Contractor shall not employ any person who is an employee of the Department of the Air Force, either military or civilian, unless such person seeks and receives approval according to DODD 5500-7, Joint Ethics Regulation.

23.0 VISITS:

23.1 The Contractor shall attend a kick-off meeting and post award conference.

23.2 The Contractor also shall attend and support meetings and teleconferences to discuss technical or regulatory issues and project progress and status. The COR or Point of Contact will schedule and provide space for all required meetings.

24.0 NOTIFICATION REQUIREMENTS:

24.1 The Contractor is required to notify the Contracting Officer (KO) of critical issues that may affect the contract performance and/or human health and the environment. The types of issues that require notification include, but are not limited to health risks and changes in critical personnel. On critical issues, verbal notification should be made immediately, followed by written notification as soon as practical.

25.0 COORDINATION OF ACTIVITIES:

25.1 The Contractor shall coordinate worksite activities with all applicable personnel to ensure the protection of human health and the environment; the prevention of damage to property, utilities, materials, supplies, and equipment; and the avoidance of work interruptions. The Contractor shall provide physical security to work areas with security equipment and personnel.

26.0 CONTRACT ADMINISTRATION:

26.1 Contracting Officer (KO), the term used herein, does not include any representative not acting within the scope of his/her authority. Notwithstanding any of the provisions of this contract, the KO shall be the only individual authorized to in any way amend or modify the terms of this contract
