Statement of Work HPP Flooring
1. DESCRIPTION OF SERVICES. The contractor shall provide all management, tools, supplies, equipment, and labor necessary to provide and install 920 square foot of rubber flooring and 1,222 square foot of non-fill turf in Building 91025, Hurlburt Field, FL. The rubber flooring will be installed in a Physical Therapy room that is climate controlled. The non-fill turf will be installed in a Garage Bay.
1.1. BASIC SERVICES.
1.1.1. The contractor shall remove the existing carpet product and dispose of at an off base location.
1.1.2. The Government will remove all items and furniture from the room prior to the scheduled installation date. If we need the contractor to move items from the area, list what items are in the area that will need to be moved.
1.1.3. After carpet removal, the contractor shall grind, treat and prep the subfloor to prepare for rubber flooring installation. Treatment shall comply with manufacturer’s specifications for the product being installed.
1.1.4. Products must meet the salient characteristics detailed below:
Rubber Flooring:
- Rubber flooring thickness of 3/8” to 3/4" with antimicrobial protection
- Backing must allow liquids to immediately leave the surface
- Color: Dark Grey or Black
Non-Fill Turf:
- Turf grass blades shall be made with non-absorbent nylon fibers
- 5/16” foam backing for turf 1/16” variance authorized
- Total turf height including foam backing of 3/4”, 1/8” variance authorized
- Turf must have non-toxic antimicrobial protection
- Turf must be approved for NCAA athletic competition or US Army ACFT
- Minimum of 5 year manufacturer warrant
- 1 year workmanship warranty
- Color: Green
1.1.3 Flooring shall be installed in accordance with the manufacturer’s specifications, ADA federal regulations and in accordance with user/installation manuals as to not void any warranties.
2. GOVERNMENT FURNISHED PROPERTY AND SERVICES. There will be no Government Furnished Property or Services provided for this effort.
2.1. Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: Mon-Fri 7:00AM to 4:00PM, except for federal holidays.
The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government.
2.2. Recognized Holidays. The contractor is not required to provide service on the following days:
New Year’s Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.
2.3 The installation shall not last more than 7 calendar days from start of the project. The contractor will not be permitted to finish any work after the 7 day period and the Contracting Officer will terminate the contract for cause for any work not acceptably accomplished during this period. The contractor shall uncrate/unpack all new items and perform required assembly in accordance with the manufacturer’s instructions. Contractor shall remove and dispose of packing material at an off base location. The Contractor is responsible for securing the disposal site.
Additionally, the Government does not have a dumpster available for use for this project. If an onsite dumpster is required, the contractor is responsible for providing.
2.4 The contractor shall have a designated Project Manager that will remain onsite for the duration of the contract and ensure work is being completed correctly and in an acceptable manner. The Project Manager’s name and contact information shall be provided to the government representatives onsite prior to contract installation.
3. SPECIAL QUALIFICATIONS. The contractor shall be an authorized and/ or certified as a distributor/reseller of the project being offered as well as any subcontractors.
4. BASE ACCESS
4.1 Contractor, Employee Base Pass and Identification, Special Clearances, and Vehicle Passes. The contractor shall obtain base identification and vehicle passes, if required, for all Contractor personnel who make frequent visits to or perform work on the Air Force installation(s) cited in the contract. Contractor employees are required to wear or prominently display installation identification badges or contractor-furnished contractor identification badges while visiting or performing work on the installation. The contractor shall submit the employee listing the Contracting Officer no less than 3 days prior to the performance start date and at least one day prior as changes occur. Employees with active warrants or felonies will not be granted access to the base. Additionally, drugs of any nature are not authorized on federal installations. If found with any drug, persons will not be granted access to the base and will be banned from accessing in the future. This includes Medical Marijuana where a Medical ID is present.
4.2 The Contractor Employee List. This form shall list every employee requiring base access. Every time employees change, it shall be updated to reflect additions and/or deletions. Identify those individuals you wish to have escort authority by including the word “escort” or “E” next to their names. Limit the number of individuals designated as escorts to a reasonable number (i.e. one or two). For work locations use building numbers or specify as base wide if applicable. Once completed, email the list to the Contracting Office representative in EXCEL format. The Contracting Officer will then prepare, attach a cover letter, and forward the Employee List to the Visitor Control Center.
4.3 Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification.
If an employee will be driving on Hurlburt Field they are required to provide a valid driver’s license, vehicle registration, and proof of insurance. NOTE: The section of Commercial gate Control Center administrating this function is closed after 1800 (6:00 p.m.), therefore, employees must arrive prior to this time in order for their documentation to be processed.
4.4 During performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the work site.
4.5 FIREARMS & ILLICIT DRUGS MAY NOT BE BROUGHT ON BASE UNDER ANY
CIRCUMSTANCES. Individuals with criminal records are not allowed on base. Contractor employees are prohibited from possessing weapons, firearms or ammunition on themselves or within their contractor-owned or privately owned vehicle while on Hurlburt Field, FL.
4.6 DRIVING LICENSE/ON-BASE REQUIREMENTS. The contractor and employees shall comply with base traffic regulations at all sites. The contractor shall ensure employees have a current and valid state/country driver’s license for the type of vehicle being driven before allowing the employee to operate a personal vehicle on Hurlburt Field, Florida.
