Statement of Work

Installation of Audiometric Screening Booth

1. Description Of Services. The contractor shall provide all supplies, tools, labor and equipment necessary to install seven single audiometric booths, to also include disassembly, transportation and disposable of the current single 6-station Audio booth for the following section: 94th Aeromedical Staging Squadron (Bldg. 550, Room 1256), Dobbins Air Reserve Base.

1.1 Objective. Replace single six-station booth with seven single audiometric booths to improve process flow, efficiency and throughput, in addition to preparing for upcoming changes to service member physical health assessment standards.

Audiometric Booths

1.2 Audiometric booths should be equipped with the Following: Standard LED lighting, Light and Fan Switches, Casters, Fan Exhaust & Static inlet recessed wall Silencers, Standard Acoustic patient viewing window, Jack panel, pass-thru grommet, Phone jacks, and Camlock Construction.

1.2.1 Standard Fan Silencer system capable of 10 to 12 full air exchanges per hour.

1.2.2 Standard Casters for ease of mobility (i.e. Two straight casters & two rotating casters) with option to swap out for isolators upon request.

1.2.3 Standard jack panel with minimum of one ¾” pass through grommet, two USB ports and four jack ports.

1.2.4 Camlock Construction for ease of Disassembly / Re-assembly

1.3 Audiometric booths exterior dimensions shall not exceed: 42” W x 40” D x 75”H

1.4 Audiometric booths interior dimensions shall not exceed: 38” W x 36” D x 68” H

HOURS OF OPERATION

2. Normal Hours of Operation. The contractor shall perform the services required under this contract during the following hours: MON-FRI 0800 to 1600, except for federal holidays. The contractor may work, with prior approval of the Contracting Officer, extended hours to ensure timely completion of work at no additional cost to the government. Recognized Holidays. The contractor is not required to provide service on the following days: New Year's Day, Martin Luther King Jr. Day, President’s Day, Memorial Day, Juneteenth, Independence Day, Labor Day, Columbus Day, Veterans Days, Thanksgiving Day and Christmas Day. If the holiday falls on Saturday, it is observed on Friday. If the holiday falls on a Sunday, it is observed on Monday.

3. SECURITY REQUIREMENTS.

3.1 Ten duty days prior to arrival at an installation, the Contractor will email the 94th ASTS POC listing the name, driver's license number and state of issue, date and place of birth for all individuals who will be accessing and working on the installation. The contractor will be responsible for obtaining written permission/signature from each individual for a background check to be completed. If a background check cannot be completed, entry onto the installation cannot be granted. Employees with active warrants or felonies will not be granted access to the base. Contact the 94th ASTS POC for further guidance.

3.2 Upon notification of the employee listing being forwarded to the Visitor Control Center (VCC), all contractor employees shall report to the VCC to complete the required documentation for base access. Contractor employees shall be required to provide two (2) forms of identification. If an employee will be driving on Dobbins ARB, they are required to provide a valid driver's license, vehicle registration, and proof of insurance. NOTE: The section of the Commercial Gate Control Center administrating this function is closed after 1600 (4:00 p.m.), therefore, employees must arrive prior to this time for their documentation to be processed.

3.3 During the performance of the contract, the contractor shall be responsible for obtaining required identification for newly assigned personnel and for prompt return of credentials and vehicle passes for any employee who no longer requires access to the site.

3.4 The Contractor and contractor employees shall comply with base traffic regulations. The contractor shall ensure employees have a current and valid state/country driver's license for the type of vehicle being driven before allowing the employee to operate a personal vehicle on Dobbins ARB, Georgia.

3.5 Weapons, Firearms, Ammunition. Contractor employees are prohibited from possessing weapons, firearms, and ammunition on themselves or within their contractor or privately owned vehicles while on base.

4. ACCEPTANCE

4.1 Acceptance of work completed will only occur after a visual inspection for proper placement.
