STATEMENT OF WORK (SOW)
1. INTRODUCTION
a. The primary mission of the Defense Logistics Agency (DLA), Installation Management is to directly provide services for Disposition Services. DLA Disposition Services (DLA DS) function is to dispose of Department of Defense (DoD) excess and surplus personal property generated by the military services. This is accomplished through reutilization to other DoD activities, transfer to other Federal agencies, donation to authorized state and local organizations, sale to the public, or ultimate disposal through abandonment or destruction. DLA DS and Installation Management are headquartered in Battle Creek, Michigan, with a variety of field sites worldwide. The purpose of this contract is to acquire new industrial equipment at DLA DS Sites in the Continental United States (CONUS) and also our site located in Hawaii.
b. This procurement service is funded through DLA Installation Management (DODAAC SC4400). DLA Installation Management’s Point of Contact (POC) will certify the invoice submitted through the Wide Area Work Flow (WAWF) system; the on-site POC will verify the order is properly received and installed.
c. Upon receipt of a task order, the Contractor has 12 months to deliver the forklift. The Contracting Officer (KO) may extend the delivery period for extenuating circumstances. Ten days prior to the end of the period of performance period, a written request for extension must be received from the Contractor detailing the circumstances that delayed delivery. Such written notifications will include the subject Task Order number, reason(s) for the delay with supporting substantiated actionable documentation, as applicable, and the estimated date and time for completed performance.
d. New Equipment ONLY; no remanufactured or “gray market” items.
e. The forklift will have a DoD item unique identification (IUID) in compliance with DFARS 252.211-7003 (252.211-7003 Item Unique Identification and Valuation. | Acquisition.GOV).
2. CLIN 0001 4K ELECTRIC REACH TRUCK FORKLIFT SALIENT CHARACTERISTICS
a. When CLIN 0001 is ordered, the Contractor shall purchase and deliver one (1) 4K Electric Reach Truck Forklift with the following salient characteristics:

3. LIN 0002 4K ELECTRIC FORKLIFT STAND-UP WITH 48” TINES – SALIENT CHARACTERISTICS
a. When CLIN 0002 is ordered, the Contractor shall purchase and deliver one (1) 4K Electric Stand-Up forklift with an open cab and all of the characteristics in section 2.a with 48” tines.
4. CLIN 0003 4K ELECTRIC FORKLIFT SIT-DOWN WITH 36” TINES – SALIENT CHARACTERISTICS
a. When CLIN 0003 is ordered, the Contractor shall purchase and deliver one (1) 4K Electric Sit-Down forklift with the following salient characteristics:
1) 4,000 lb. lifting capacity
2) 36” Tines
3) 3-Stage high visibility mast
4) Maximum lifting height of 187”
5) Overall Lowered Mast Height 82”
6) Wheelbase not to exceed 56.3”
7) Hydraulic fork positioner
8) Hydraulic side shifter
9) 48-volt electrical system.
10) Visible amber LED strobe light – continuously activated, overhead guard mounted
11) Audible reverse activated 82-102 dB(A) self-adjusting alarm
12) LED Work Lights (Two front, one rear work light and two brake/tail backup lights)
13) Side Mirrors
14) Full suspension vinyl seat
15) Solid pneumatic rubber tires
16) Technical manuals, one digital and one hard copy
17) Fire Extinguisher
18) One (1) battery charger, 60 VAC, 3 phase, for the 48v forklift.
5. CLIN 0004 4K ELECTRIC FORKLIFT SIT-DOWN WITH 42” TINES – SALIENT CHARACTERISTICS
a. When CLIN 0004 is ordered, the Contractor shall purchase and deliver one (1) 4K Electric Sit-Down forklift with an open cab and all of the characteristics in section 4.a with 42” tines and a 36” carriage back rest.
6. MANUALS AND FAMILIARIZATION TRAINING
a. The Contractor shall provide one digital and one hard copy Operators Manual Guide, and Maintenance and Service Manual for each machine delivered.
b. The Contractor shall provide familiarization training upon delivery to the end user for each piece of equipment delivered. The Contractor shall ensure the training takes place within 14-business days of initial delivery. The training shall include but not be limited to:
1) Daily operator startup and shutdown procedures.
2) Initial system startup including identifying and filling of all fluids.
3) Daily and periodic preventative maintenance requirements and visual checks.
4) Operator Safety overview.
5) Operator Controls and indicators overview.
6) Operational familiarization. Maneuvering the forklift and all its controls.
7) Machine limitations and common operator errors that cause damage or premature wear on parts/systems.
8) Attachment and detachment of all tools, parts, and attachments delivered with the machine that can be used during operation.
c. The Contractor shall provide a video of the familiarization training.
7. EQUIPMENT DELIVERY AND WARRANTY
a. For the Contractor to successfully handoff the forklift and receive payment from the United States Government (USG), they shall:
1) Be present for delivery.
2) Unload the piece of equipment.
3) Assemble the piece of equipment (if required).
4) Conduct an inspection with USG personnel to verify that all items are present, and that the equipment contains the minimum listed salient characteristics.
b. The Contractor shall complete and sign the provided DD250, Material Inspection and Receiving Report, with USG personnel and attach the completed form in WAWF prior to acceptance for payment. A picture of the data plate with serial number shall be attached to the invoice prior to Government acceptance.
c. This equipment shall come with the manufacturer’s standard equipment warranty. The warranty date shall begin upon completion of familiarization training.
d. The delivered equipment must be set-up and operational within ten days after delivery. The Contractor shall provide at least 14 calendar days advance notice to the site POC before rendering services at the site.
e. The delivery to be performed under this contract will be performed during normal government working days, Monday through Friday. The Contractor shall not schedule delivery on a regular federal holiday (see below), designated by Presidential proclamation, or DLA DS closure, rescheduling shall be at the discretion of the POC.
f. The holiday is observed on the preceding Friday or the following Monday when the holiday occurs on Saturday or Sunday, respectively.

8. DESTINATIONS
a. The Contractor shall notify Michelle Wood, and Adam English at least 14-business days prior to delivery of the machine to its destination to ensure the site is prepared to accept the machine. The Contractor shall use dladsopsmhe@dla.mil and Adam.English@DLA.mil as the official communication link for all delivery functions.
b. The Contractor shall verify the destination POC and projected CLIN in Attachment 1 – Destinations. Prior to final delivery, the Contractor shall verify the shipping address and set up pre-installation meeting date, time, and location with the Site Manager. All destinations and CLINs are subject to change and will be provided on the associated Task Order.
c. Destinations associated with this SOW are within CONUS and Pearl Harbor, Hawaii.
9. SPILL RESPONSIBILITY
a. The Contractor is solely responsible for all spills or leaks during the performance of this contract, which occurs because of, or is contributed to by the actions of its agents, employees, or subcontractors. The Contractor shall clean up such spills or leaks to the satisfaction of the USG in a manner that complies with applicable Federal, state, and local laws and regulations. The clean-up shall be at no cost to the USG. The Contractor shall report all such spills or leaks, regardless of their quantity, to the POC and Contracting Officer (KO) immediately upon discovery. A written follow-up report shall be submitted to the POC and KO not later than twenty-four hours after the initial telephonic report. The written report shall be in narrative form, and as a minimum shall include the following:
1) Description of item spilled (including identity, quantity).
2) Whether amount spilled is EPA or state reportable, and if so, whether it was reported. Exact time and location of spill including a description of the area involved.
3) Containment procedures initiated.
4) Summary of any communications Contractor has with press or USG officials other than the KO.
5) Description of clean-up procedures employed or to be employed at the site including disposal location of spill residue.
6) Description of item spilled (including identity, quantity).
10. ENVIRONMENTAL MANAGEMENT SYSTEM
a. DLA DS implemented an Environmental Management System (EMS) in accordance with ISO 14001:2015(E), DOD, and DLA requirements at all appropriate DLA DS facilities. An EMS is an overall management system that includes organizational structure, planning activities, responsibilities, practices, procedures, processes, and resources for developing, implementing, achieving, reviewing, and maintaining the environmental policy of an organization. The DLA DS EMS is designed to ensure that all personnel, including contract personnel, whose work activities can cause real or potential significant environmental impact, are aware of how their work supports the EMS and are competent to perform their roles, responsibilities and authorities while supporting the DLA DS mission. The DLA DS EMS program is explained in the agency EMS Core Instruction. Site supplements may detail information required by EMS at a local level such as local, state and host environmental regulations as well as local emergency procedures and operating procedures unique to the location. Site supplements also contain information concerning the local interaction between the appropriate DLA DS tenant facility and the DOD/DLA host facility EMS program. Contractors and their personnel are required to be aware that each appropriate DLA DS facility has an EMS and to be aware of EMS policies to perform work and/or gain access to an installation. Each DLA DS facility maintains EMS awareness posters that identify the DLA DS Environmental Management Representative (EMR) and host facility EMR for specific guidance concerning an individual facility’s EMS program. Contractors may request current copies of EMS information identified in this Section by contacting the local COR.
11. INVOICING
0. The Contractor will invoice monthly through the DoD Wide Area Workflow (WAWF) system. Invoices must be submitted through WAWF in accordance with Defense Federal Acquisition Regulations Supplement (DFARS) 252.232-7003. Refer to https://wawf.eb.mil for additional information.
