Sources Sought Questionnaire Comprehensive Medical Services Federal Correctional Complex (FCC) Beaumont Sources Sought Notice 15BFAO25N00000006
The Federal Bureau of Prisons is conducting market research pursuant to Federal Acquisition Regulation Part 10. A single solicitation is anticipated to be issued for Managed Healthcare Services for adults in custody (AICs) at the Federal Correctional Complex located in Beaumont, TX (hereinafter referred to as the “institution”). Services to be provided will encompass all levels of medical care to the AICs, including the on-site operation of medical facilities at FCC Beaumont and inpatient/outpatient physician services. The institution currently houses male AICs. Additional information about the inmate population at the institution can be found at www.bop.gov.
This Sources Sought Questionnaire is for information purposes only. The described services may or may not be included in the subsequent solicitation.
Presently, this institution is anticipating estimated quantities for the base and four-one year option periods as follows:
Managed Healthcare Services:
Mandays: 1,898,000 per year (approximately)
Pricing Methodologies
Manday Rate. This rate will be the price per inmate, per day based upon the actual count of the Federal Correctional Complex Beaumont at approximately 12:00 a.m., including all inmates located in community hospitals.
Catastrophic Limit and Medicare Reimbursement Rate:
Catastrophic Case. A catastrophic case is defined as one of the following:
1. A single hospitalization
2. Multiple hospitalizations within 24-hours of each preceding hospitalization for the same or directly related treatment.
3. Admission to a Long-Term Care Facility.
4. A recurring medical need, for example, community-based dialysis.
Catastrophic Limits and Catastrophic Limit Pricing for community-based treatment for inmates will be calculated from benchmarks utilizing Medicare reimbursement methodologies. For Catastrophic Limit Pricing, Offerors will be allowed to propose a variance from the benchmark Medicare rate in the form of a discount from or a premium to Medicare rates established by the Centers for Medicare and Medicaid services. The discount or premium to Medicare Part A or B shall only apply for reimbursements above the catastrophic limit within the resulting contract.
The rates established in the resulting contract shall not be construed as participation in the Medicare program; contract rates will merely be equated to Medicare rates of reimbursement without reductions for deductibles, co-payments, or co-insurance. When appropriate, outlier payments calculated in accordance with Medicare reimbursement methodologies shall be made in recognition of extremely costly stays. Outlier payments shall be calculated based upon the defined benchmark and will consist of the operating portion only. This structuring of the pricing methodology is not intended to be restrictive of any Offeror; Offerors need only to offer that percentage discount from or premium to the Medicare benchmark rate which will reflect the desired level of payment for the category of services rendered. If during contract performance, it is determined that a necessary contract deliverable is not covered by Medicare reimbursement methodologies, a separate rate shall be negotiated for such deliverable(s).
To calculate when the proposed catastrophic limit is reached, the Contractor shall re-price its billings for community-based medical care for an inmate at the applicable Medicare Benchmark rates identified in the following paragraphs with no discounts or premiums applied. When the total of the re-priced billings reaches the catastrophic limitation specified in the contract, the Contractor may begin submitting invoices for services beyond the catastrophic limitation. The Contractor shall provide a detailed analysis of the re-priced claims totaling the catastrophic limitation along with claim forms for each treatment encounter (i.e., properly coded Centers for Medicare and Medicaid Services (CMS) 1500 forms and Universal Billing (UB) 92 forms).
Invoices for services exceeding the catastrophic limitation shall be priced at the catastrophic limit pricing identified in the contact and shall be supported with properly coded claim forms for each treatment encounter (i.e., CMS-1500 and UB-92 forms). All claim forms shall be subject to the Government's review for appropriateness and accuracy.
Medicare Part A. The Medicare benchmark to be utilized for acute services covered by Medicare Part A shall be the most current Basic MS-DRG payment (i.e., Operating Federal Rate) established for CBSA 13140, Beaumont, TX. The Basic MS-DRG calculation shall not include any provider-specific adjustments allowed under actual Medicare participation. (Refer to Attachment 6, Computation of Basic MS-DRG Payments, of this solicitation.
Long-Term Care Facility Services: The Medicare benchmark to be utilized for long-term care hospital services by Medicare Part A shall be the most current LTC MS-DRG payment amount (i.e., Federal Rate) established for CBSA 13140, Beaumont, TX.

The Medicare benchmark to be utilized for physician professional services covered by Medicare Part B shall be those rates established in the most current Medicare Part B Fee Schedule for Payment Locality 20, Beaumont, Texas.
