SECTION 2: CONTRACTOR ARRANGEMENT

SECTION 3: SMALL BUSINESS FIRM’S PAST EXPERIENCE
NOTE: For purposes of responding to the questions in this section, FIRM refers to the small business entity performing as a prime sole contractor, team subcontractor, protégé member of a joint venture under a SBA mentor/protégé agreement, or the joint venture entity listed above under SECTION 2. CONTRACTS and TASK ORDERS provided in response to questions one through five below shall be for work performed on the contract/task order by the firm since May 2017. For questions where the FIRM does not have the experience requested, note as N/A.

5. IF AWARDED A CONTRACT, the firm □ WILL/ □ WILL NOT PAY MORE THAN 50% OF THE AMOUNT PAID BY THE GOVERNMENT FOR CONTRACT PERFORMANCE TO SUBCONTRACTORS THAT ARE NOT SIMILARLY SITUATED ENTITIES. Prime contractor should describe how they will comply with FAR clause 52.219-14, Limitations on Subcontracting.
6. Provide the total number of Full Time Equivalents (FTEs) currently employed by the firm. For a joint venture entity, provide total number of FTEs for each joint venture member. By separate attachment, provide a list of the number of employees by major categories, such as project managers, environmental engineers, environmental scientists, quality control, contract administration, administrative, craft/laborers, biologists, health and safety specialists, etc., that demonstrates the firm’s ability to perform environmental remedial actions and the management of a large IDIQ cost reimbursement contract.
