STATEMENT OF OBJECTIVES

1.0 Project Background

1.01 Bridge 13599, located on Canelo Road Bridge between the Wren Arena and Site Maverick is starting to fail. Repairs are required to keep the bridge in service. The required repairs are detailed in this statement of objectives with photos and explanations.

2.0 Statement of Objectives

2.01 This is a design/build contract. The contractor shall provide design/build services, furnish all labor, material, equipment, supervision, transportation, permits, fees, licenses necessary to complete the required repairs on Bridge 13599. A non-specific statement of work for this repair project is provided in Section 4. The contractor shall prepare a more detailed scope of work outlining their Contract Execution Overview, to include their technical approach, and tentative schedule. The contractor shall field verify the area of work before submitting their SOW and proposal.

3.0 Proposal Requirements

3.01 Technical Approach. The contractor proposal technical approach section must address each of the line items listed in Section 4.0, Statement of Work. Language of this section must include a description of the steps and methods by which each item of the SOW will be completed. This description must adhere to the details outlined in pertinent UFGS specifications (see listing in back of this SOO), such that a clear understanding of the requirements can be demonstrated to a competent qualified person.

3.02 Materials/Quantities/Equipment. The Contractor Proposal Materials, Estimated Quantities, and Equipment section must provide a clear and detailed list of all of the building materials and estimated quantities, as well as the equipment to be used in the execution of the project. These items must be presented in a bulleted list fashion such that a competent qualified person can understand what materials, estimated quantities, and equipment will be used during the execution phase of the project.

3.02.01 Materials shall include items such as chemical or mechanical fasteners, hardware, devices, fixtures, valves, piping, sealants, structural members, electrical wiring and all other permanently installed materials at the project location.

3.02.02 Equipment shall consist of all tools, machinery, motorized or engine driven equipment, and all other non-permanent items which will not be left on site.

3.03 Labor Hours. The contractor proposal labor hours’ section must give a detailed breakdown of labor hours, to include an estimated number of hours assigned to each trade or supervisory position including engineers design hours, such that a clear understanding of the division of labor can be determined. In addition, the total number of hours estimated for the project shall be listed.

3.04 Tentative Schedule. The Contractor Proposal Tentative Schedule section must provide a detailed estimate of all activities to be conducted during the project period of performance. Scheduled actions must be listed in a logical order to demonstrate a clear timeline of the proposed project activities.

4.0 Statement of Work

4.01 The following list is not detailed in scope; the contractor shall provide a more specific scope of work.

Bridge 13599 is located on Ft. Huachuca.

Facility Carried: Canelo Road.

Feature Intersected: Slaughterhouse Wash.

Latitude: 31°34'01.88" N and Longitude: 110°23'44.92" W. (see Figure 1 to 6).

The Bridge 13599 is in critical condition.

BRIDGE DESCRIPTION & MATERIALS:

Bridge 13599 is a single-span reinforced concrete T-beam bridge with a total length of approximately 27’-10”.

Typical span length is taken as 27’-10”, face to face of abutment.

The beam is considered simply supported for live load. The elevation view sketch is shown in Figure 1. The section view sketch is shown in Figure 2.

4.02 Design

4.02.1 The contractor shall review this SOO and perform a complete design of the proposed repairs.

There is only one phase of design for this effort. The contractor shall not be required to submit incremental or preliminary designs. The contractor shall list/identify which standards and specifications are being applied to each Repair Bridge Group of the repair. A traffic control plan shall be submitted with the design.

4.02.2 The utilities in the area include a sanitary sewer lift station with gravity sewer, a 4-inch potable water line with ancillary water taps on the north side of the road/bridge area, a buried communications line, a buried natural gas line and overhead power to the lift station. The contractor shall obtain and keep valid the necessary dig permit. The fee for a dig permit is $132 as of the date of the SOO.

4.02.3 The contractor shall not start on-site work until all permits, plans and designs are approved.

4.03 Repair Bridge Group A (bridge)

The following is a list of the most significant findings:

- Shear and flexure cracking growth and exposed rebar on T-beams.

- Spall with exposed rebar in the deck underside.

- Crack growth in concrete T-beams and Railing.

- Girder cracks and degradation.

- Wing-walls have several cracks. See figure 15.

- Underside of deck have cracks with efflorescence. See figure 16.

- Beams have shear and flexure cracks. See figures17 & 18.

4.03.1 The contractor shall patch honeycombing with exposed rebar on the underside of deck and

T-beams at several locations. See figures 19 & 20.

4.03.2 The bridge has several damaged sections with crack widening and growth. The contractor shall remove the broken concrete and repair/replace the bituminous liner in accordance with ASTM849. See figures 12 thru 21.

4.03.3 The contractor shall demolish, replace and install new bridge rails. Bridge railing design shall be consistent with AASHTO LRFD Specifications Section 13. The length of the end treatments will need to be increased. The design engineer is encouraged to use ADOT Bridge Group structure detail drawings wherever appropriate. Bridge Group website maintains the latest versions of these standard drawings.

4.03.4 The contractor shall demolish, replace and install new bridge rails. Bridge railing design shall be consistent with AASHTO LRFD Specifications Section 13. The length of the end treatments will need to be increased. The design engineer is encouraged to use ADOT Bridge Group structure detail drawings wherever appropriate. Bridge Group website maintains the latest versions of these standard drawings.

4.03.5 The contractor shall replace rusted/charred/damaged wooden posts, wooden spacers and steel pipe with base plate. The steel pipe with base plate shall be replaced with the same size as existing. The timber for the posts and spacers will be rough sawn (unplaned or S4S with the nominal dimensions indicated). Any species or group of woods graded in accordance with the requirements for Timber and Posts of the Western Wood Products Association may be used. Timber will be No. 1 or better, and the stress grade will be 1,200 psi for a 6in by 8in post and block. The other sizes are noted but not used: 900 psi for an 8in by 8in post and block, and 900 psi for a 10in by 10in post and block. All timber will have a preservative treatment in accordance with the requirements of AASHTO M133. See figures 10, 12 &13.

4.04 Repair Bridge Group B (channel)

4.04.1 The following is a list of the most significant findings:

- Scour around abutments (degradation and aggradation). See figures 17 &18.

- Large scour hole with undermining in channel upstream from the structure.

- Erosion and undercutting of the downstream channel banks.

- Scour hole at downstream side of abutment 2. See figure 9.

- Scour hole and undermining occurring at upstream side of the channel. See figures 23 & 24.

- Erosion and undercutting of the downstream channel embankments. See figure 26.

4.04.2 The contractor shall remove the aggradation dirt sediment material from the channel under the bridge, approximately 900 square feet to a depth of approximately three feet to the natural soil. See figures 17 and 18.

4.04.3 The contractor shall re-grade and add rip-rap material for scour hole at downstream side of abutment 2, see figure 9; re-grade and add rip-rap for scour hole and undermining occurring at upstream side of the channel, see figures 23 and 24; re-grade and add rip-rap for erosion and undercutting of the downstream channel embankments, see figure 26.

4.04.4 The contractor shall regrade eroded areas near the bridge and the shoulders and edges.

4.05 Repair Bridge Group C (roadway)

The following is a list of the most significant findings:

- Longitudinal and transverse cracks on wearing surface. See figure 14.

4.05.01 The Contractor shall repair the asphalt surface of the bridge including the approaches and egresses. This work shall be performed on weekends to minimize road closures during the week.

4.05.02 The contractor shall replace the four hazard markers located at bridge approaches. See figures 3, 4, 10, 11 & 12. The contractor shall install two hazards markers on each approach to the bridge. One hazard marker will be installed at or near the end of each guardrail. H a z a r d Markers shall be in accordance with Arizona Department of Transportation Warning Signs and Object Markers OM3(R and L), Type 3 Object Marker (12” X 36”). See figure 22.

5.0 Schedule

5.01 Notice to proceed shall be issued no later than 10 calendar days after the award. The project shall be 100% complete and ready for final inspection no later than 150 working days after the NTP.

5.02 The contractor’s proposed schedule shall be submitted with the proposal. A revised schedule shall be submitted at the time of the preconstruction conference for approval. The schedule shall be in the form of a progress chart of suitable scale to indicate the percentage of work scheduled for completion at any given date during the period. The schedule shall take into consideration anticipated adverse weather that could preclude progress, long-lead equipment or materials critical to the completion of the project, review periods for submittal items requiring Government review and approval, air quality control permitting process and other events and milestones as necessary.

5.03 The schedule shall be updated, revised and submitted for approval each time in the event that work falls behind. The revision number shall be shown on any re-submittal. The intent is to show in the event that construction is behind just how the contractor intends to finish within the approved contract time period. It is the contractor’s responsibility to properly maintain the site during the life of the contract.

6.0 Occupancy

6.01 One lane of travel on the bridge shall be open 24/7/365. The traffic control plan shall include all pertinent details. The contractor shall provide traffic control signage including lighting and flaggers as necessary to maintain traffic flow.

7.0 Material, Standards, Codes, Regulations, and Workmanship (FAR 52.236-5, Material and Workmanship)

7.01 All materials and equipment shall be new unless otherwise specified. All work shall be completed in accordance with the plans and specifications, applicable industry standards, UFC, UFGS, NFPA, NEC, MUTCD, SDDCTEA, and AASHTO standards; federal, state and local codes, regulations of EPA, OSHA, if required; Network Enterprise Center (NEC) and SSVEC requirements; Fort Huachuca Energy Policy and Army Energy and Water Management Program. All government permits, certifications, lead paint and asbestos analyses, if required, etc. shall be obtained by the Contractor prior to commencement of work or prior to beginning construction and are the specific responsibility of the Contractor (Federal, State and local governments). Air quality control permitting efforts must be coordinated with DPW ENRD Air Quality Program prior to the start of any construction and prior to making any irrevocable decisions (i.e. purchasing equipment) related to equipment, activities or processes that have the potential to emit air pollutants such as combustion exhaust, dust, asbestos, evaporative emissions, refrigerant, etc. All certifications are the responsibility of the contractor (potable water health certifications, gas weld certifications, refrigerant technician certifications, asbestos removal certifications, etc.) and must be provided to the COR prior to final payment.

7.02 The contractor shall obtain specific approval for all materials and equipment that vary from the standards established herein. All work shall be performed in a skillful and workmanlike manner to current applicable industry standards and in accordance with manufacturer instructions. Work requiring certified craftsmen shall be performed by certified personnel. Proof of such certification, training, or other qualifications shall be made available to the Government upon request. If the manufacturer of any product used in accomplishing this project has published an installation and/or operation manual, such manual shall be followed in its entirety. If any organization such as, but not limited to, APA or ANSI publish any construction guide, it will be followed as applicable to this project. If the manufacturer’s manual deviates from any established code, regulation, local requirement or this scope of work, such deviation shall be brought to the attention of the government representative before award, for written resolution. A revised scope of work will be issued to specify such resolution and incorporated in any resulting award.

7.03 Commissioning of major systems (none noted) is required, unless otherwise noted.

7.04 The contractor shall comply with Federal directives by recycling at least 50% of Construction waste.

The contractor shall provide RFI in unable to meet this element.

7.05 The contractor shall comply with all Federal, State and Army Environmental requirements. Ensure Contractor responsibilities and operating limitations are noted in proposal [i.e. Storm Water Pollution Prevention Plan (SWPPP) shall be initiated and paid for by the contractor, contractor must conduct and record dust control for all dust-generating activities, contractor cannot use photo-chemically-reactive solvents in the application of architectural coating, etc.]. Contractor shall keep all required records as stated in the following (and any others as required by Federal/State authorities) and submit to the government.

7.06 Air, water, asbestos, lead based paint and hazardous waste, among others, need to be covered in any construction, demolition or renovation contract; thus, the contractor is required to maintain all record-keeping and submit it to the appropriate government entity at the appropriate time, with a copy sent to the DPW COR. The contractor shall have a competent person as recognized by OSHA to allow the contractor to properly deal with the asbestos containing material and lead paint as part of the repairs. The contractor shall be made aware that lead containing paint may be encountered.

Precautions shall be made to contain any lead dust created during repair work. Contractor shall prepare and execute an EPA/OSHA compliant Lead in Construction worker protection program in accordance with 29 CFR 1926.20 as applicable. No testing for asbestos containing material or lead containing paint has been accomplished due to the work being ‘structural’ in nature.

7.07 Environmental work statement clause for all exterior work (SWPPP)

7.07.01 Through the Arizona Pollutant Discharge Elimination System (AZPDES) permitting program, the Arizona Department of Environmental Quality (ADEQ) regulates the discharge of storm water from construction sites throughout the state including those on military installations.

7.07.01.01 “Construction” refers to actions that result in a disturbance of the land, including clearing, grading, excavating, and other similar activities. It also includes “construction-related activities,” areas that support the construction project such as stockpiles, borrow areas, concrete truck washouts, fueling areas, material storage areas and equipment storage areas.

7.07.01.02 Stockpiles, borrow pits, etc. need to be weed-free areas. Contractors shall use certified weed-free fill or contractors must sterilize the fill before bringing it onto the installation.

7.07.02 The AZPDES Storm water Program requires operators of construction sites one acre or larger (including smaller sites that are part of a larger common plan of development) to obtain authorization to discharge storm water under an AZPDES construction storm water permit. The Contractor is responsible for determining whether its construction site will exceed this minimum threshold with consideration of the definition provided above and if so, obtaining coverage under an AZPDES construction storm water permit.

7.07.03 Obtaining coverage under an AZPDES permit entails preparing and implementing a Storm Water Pollution Prevention Plan (SWPPP) in accordance with ADEQ requirements and submitting a Notice of Intent (NOI) for coverage under the applicable AZPDES general permit. Once ADEQ receives the NOI, ADEQ then issues an authorization certificate to the Contractor. If a contractor has a subcontractor working together on the same project, then the contractor and subcontractor(s) will have to submit a NOI for the same job and separate authorization certificates will be issued to each party.

7.07.04 If the Contractor determines that the project will not exceed the minimum threshold and does not submit an NOI and it is later determined that the threshold is exceeded, the Contractor will be required to stop all work until they have completed the above process. If the appropriate documentation is not provided to the Government prior to the start of construction or once the threshold is exceeded, the Contracting Officer will be notified of the failure to comply with a contract requirement, the ADEQ Enforcement Section will be notified, and other appropriate action will be taken to resolve the matter. The Contractor and any sub-contractor(s) will be liable for any fines levied by ADEQ.

7.07.05 The government highly recommends that contractors visit the ADEQ website at https://www.azdeq.gov, under water programs and type in “Construction Permits/SWPPP” in the search box for all relevant information on obtaining coverage.

7.07.06 It is the responsibility of the Contractor to develop a SWPPP that meets applicable requirements; the Government will not review or approve the Contractor SWPPP for compliance with applicable laws. The Contractor and any sub-contractor(s) shall provide a copy of the ADEQ Notice of Intent (NOI) and a copy of the SWPPP to the Government employee performing construction oversight prior to any construction or construction-related activities as defined above. Contractor may request a courtesy inspection by the Government of their site as it conforms to their SWPPP.

8.0 Clean Up (FAR 52.236-12, Cleaning Up)

8.01 The contractor shall at all times keep the work area, including storage areas, free from accumulation of waste materials. All trash will be removed from the site on a daily basis. Upon completion of the project, all equipment, materials and other items associated with the project shall be removed from the project site and the site shall be left clean. Any and all waste products created during performance of the work shall be removed from the project site, packaged in an environmentally approved manner and disposed of OFF-POST at an environmentally approved disposal site.

9.0 Utilities

9.01 Where available, onsite utilities will be provided to the contractor at their expense and will require a Utility Sales Agreement with Fort Huachuca. These utilities shall be used only in the performance and support of the project.

9.02 Any temporary connection to the utility system will be coordinated with the contracting officer’s representative prior to hook-up. The contractor shall complete the temporary connection and all temporary electrical work in accordance with applicable codes and standards. Temporary electrical wiring and devices shall comply with the current National Electrical Code. Temporary wiring and devices shall be removed at project completion. All temporary electrical work shall be at the contractor’s expense.

10.0 Storage Areas/Laydown Yards

10.01 The contractor and the Contracting Officer’s Representative will agree on temporary storage areas and lay down yards in the general location of the project. Request for proposed storage area shall be submitted with the proposal documents. The contractor shall be responsible for the safety, security and maintenance associated with storage areas. Storage areas shall be kept clean and in a neat and organized appearance at all times. If the storage area is not maintained, the Contracting Officer may direct the contractor to store materials off the installation. Storage areas and lay down yards shall be returned to the existing stable state, which may re- quire some rehabilitation of the land, like re-seeding with native grass mixtures.

11.0 DD Form 1354 Worksheet Preparation

11.01 The contractor shall prepare a DD Form 1354 Information Worksheet near completion of the project and submit it to the Government prior to final inspection and acceptance. The contractor may obtain a DD Form 1354 Information worksheet from the DOC CS or the DPW COR.

12.0 Controlled Areas

12.01 The contractor will not need to include time for escorts as this is not a controlled area.

13.0 Waste Disposal and Borrow Pits

13.01 The contractor shall comply with the Fort Huachuca Waste Disposal/Borrow Pit Plan and submit a project specific Environmental Protection/Borrow Pit Plan.

14.0 Contractor Quality Control (CQC)

14.01 The contractor is responsible for quality control and shall establish and maintain an effective quality control system and shall include the following:

14.01.01 The Quality control system shall consist of plans, procedures, and organization necessary to produce an end product that complies with the specified requirements of this project. The system shall cover all construction activities, both on site and off site, and shall be coordinated with the proposed construction schedule.

14.01.02 The contractor shall submit to the COR for review and approval a Contractor Quality Control Plan proposed to implement the specified requirements. The plan shall identify personnel, procedures, controls, instructions, tests, records, and forms to be used.

Construction will be permitted to begin only after acceptance of the CQC plan. The plan shall include, as a minimum, the following to cover all construction operations and features.

14.01.02.01 A description of the CQC organization and lines of authority (e.g., Organization Chart).

14.01.02.02 Names, qualifications, duties, responsibilities, and authorities of CQC personnel.

14.01.02.03 Procedures for scheduling, reviewing, certifying, and managing submittals.

14.01.02.04 Procedures for conducting CQC inspections to include routine inspections, follow-up inspections on deficient work, pre-final inspections, and final inspections.

14.01.02.05 Procedures for tracking construction deficiencies from the time of identification through acceptable corrective action (e.g., Deficiency Tracking Log).

14.01.02.06 Documentation and reporting procedures, including proposed reporting format.

14.01.03 The contractor shall maintain current records providing factual evidence that the required QC activities and/or tests have been performed. These records shall be furnished to the Government daily and within 24 hours after the date covered by the report (except when no work is performed; then one report shall be submitted for every seven days of no work). The reports shall include the work of sub-contractors and sup- pliers and shall be on an acceptable form that includes, as a minimum, the following information:

14.01.03.01 Contractor/subcontractor and their area of responsibility.

14.01.03.02 Work performed each day, giving location, description, and by whom.

14.01.03.03 Weather conditions encountered.

14.01.03.04 Any delays encountered.

14.01.03.05 Test and/or control activities performed with results and references to specifications/drawings.

14.01.03.06 Materials received at the site with statement as to quantity and quality, acceptability, and storage.

14.01.03.07 Submittals and deliverables reviewed.

14.01.03.08 Job safety evaluations stating what was checked, results, and instructions or corrective actions taken.

14.01.03.09 Instructions given or received and conflicts in plans and or specifications.

14.01.03.10 Contractor’s verification statement that equipment and materials incorporated into the work and the workmanship comply with the specified requirements.

14.01.03.11 Air Quality Control – the contractor shall comply with all requirements of the Fort Huachuca Air Quality Control Permit and other Clean Air Act regulations by submitting all required records, including but not limited to the following, as applicable, in accordance with applicable preconstruction/construction/post-construction timelines, to the DPW ENRD Air Quality Program:

14.01.03.11.01 Detailed equipment specifications and activity/process descriptions

14.01.03.11.02 Documentation of equipment that was installed in accordance with manufacturer instruction

14.01.03.11.03 Specifications of removed equipment

14.01.03.11.04 Dust control logs

14.01.03.11.05 Fuel certifications

14.01.03.11.06 Report of run times occurring during equipment installation (i.e. generator hour meter readings)

14.01.03.11.07 EPA engine certifications

14.01.03.11.08 Records of Asbestos NESHAP Notification for Renovation and Demolition Activities forms filed with the appropriate state/county entity and all supporting documentation

14.01.03.11.09 Miscellaneous chemical usage

15.0 Inspection

15.01 All work is subject to inspection and testing by the Government at all places and at all reasonable times before acceptance, in accordance with FAR 52.246-12, Inspection of Construction.

Government inspections and tests do not relieve the contractor of the responsibility for providing adequate quality control measures, nor does it constitute or imply Government acceptance of the work. The presence or absence of a Government inspector does not relieve the contractor of complying with the specified requirements, nor is the Government inspector authorized to change any term or condition of the specification without the Contracting Officer’s written authorization.

16.0 Accident Prevention

16.01 The contractor shall comply with 29 CFR part 1926, 29 CFR part 1910, all state and local occupational safety and health requirements, and the requirements of EM 385-1-1, U.S. Army Corps of Engineers Safety and Health Requirements Manual.

16.02 The contractor is responsible for accident prevention and safety and shall establish and maintain an effective system.

16.03 The plan shall be developed by a “qualified person” (EM 385-1-1, A-1, 1.a.) and be signed by a

SOO: HUAC_FEE16030-6J Bridge 13599 Repair Modification: 0 representative of the prime contractor’s “corporate office authorized to obligate the Company”. The plan shall meet the minimum basic outline requirements contained in EM 385-1-1, Appendix A.

The plan shall be job-specific and shall include work to be performed by subcontractors and measures to be taken by the contractor to control hazards associated with materials, services, or equipment provided by suppliers. The accident prevention plan shall include procedures for frequent safety inspections, conducted by competent persons, of the work sites, material, and equipment to ensure compliance with the plan and other referenced standards. Unless otherwise approved, Quality Control personnel shall conduct and document daily safety inspections.

17.0 Submittals

17.01 The contractor shall provide information and/or submit samples of materials that the contractor contemplates incorporating in the work. All submittals shall be at the contractor’s expense.

17.02 The following submittal items are required for this project:

17.02.01 Submittals required prior to start of construction (normally at the PRECON) shall include:

17.02.01.03 Shop drawings

17.02.01.04 Site Survey prior to construction in dxf format

17.02.01.05 As built site survey after construction in dxf format

17.02.01.07 Catalog cuts (equipment and materials)

17.02.01.08 Finish material samples and/or product data\

17.02.01.09 Sub-contractor personnel list (POC with phone number)

17.02.01.10 Quality Control, Accident Prevention, Disposal/Borrow Pit Plans

17.02.01.11 Permits and certifications (Dig Permit, SWPPP/NOIs, 4.01 permits, Hot Work, etc.)

17.02.02 Submittals required during the construction period shall include:

17.02.02.01 Monthly reports to COR

17.02.02.02 Air Quality Control Reports and other Environmental submittals as appropriate (construction water used, Dust control records, etc.)

17.02.02.03 TAB reports – certified by NEBB Technician

17.02.02.04 Draft DD Form 1354 Worksheet

17.02.03 Submittals required near the end of the project but before final inspection:

17.02.03.03 Certified Commissioning Reports

17.02.03.06 Training for shop and design personnel

17.03 If any of the equipment/material submittals are disapproved, the contractor shall resubmit the disapproved submittals for review and comment within five (5) working days of notification of their disapproval. This process shall continue until all equipment/materials are approved by the government.

17.04 If contractor uses or installs materials or equipment without approval, he does so at the risk of subsequent rejection, and replacement at his cost.

17.05 Final Inspection and Acceptance

17.06 Final inspection and acceptance shall be conducted after the pre-final inspection is completed by the CQC representative and the pre-final inspection punch list items are completed by the contractor. The CQC representative compiles the pre-final punch list immediately after the work is completed. The Government’s PM/COR/QA representative will sign for completion documents after the final inspection and its punch list items are corrected; and upon receipt of acceptable As- Built Drawings and any delayed work (seeding prior to rainy season, Air Conditioning Test, Adjustment and Balancing in summer months, etc.).

18.0 Warranty Requirements

18.01 All work shall be covered under the one-year warranty of construction (FAR 52.246-21, Warranty of Construction).

18.02 In addition to the one-year warranty of construction, the contractor shall obtain all warranties that would normally be given in commercial practice and, upon final acceptance of the work (or before), provide a list to the Government of each warranted equipment item, feature of construction, or system. The list shall indicate the following:

18.02.01 Name of item.

18.02.02 Model and Serial Number.

18.02.03 Installed location.

18.02.04 Name and phone numbers of manufacturers or suppliers.

18.02.05 Names, addresses, and phone numbers of sources of spare parts.

18.02.06 Warranties and terms of warranty. Items with warranties extending beyond the one- year warranty shall be indicated with separate warranty expiration dates.

18.02.07 Start date and duration of warranty period.

18.02.08 Summary of maintenance procedures required to continue the warranty in force with cross-reference to specific pertinent operation and maintenance manuals as applicable.

18.03 The list shall include the status of delivery of all Certificates of Warranty for extended warranty items, to include roofs, HVAC balancing, pumps, motors, transformers, and for all commissioned systems such as fire protection and alarm systems, sprinkler systems, lightning protection systems, etc.

18.04 At approximately four and nine months after acceptance the Government may conduct warranty inspections. The inspections will be scheduled by the Government COR/Project Man- ager and

SOO: HUAC_FEE16030-6J Bridge 13599 Repair Modification: 0 conducted with the user, and, if possible, contractor personnel attending. A list of deficiencies existing at the time will be compiled and furnished to the Contractor for correction.
