Notice to Proceed (NTP) as stated in Block 11 below w ill not be issued until the Contractor provides sufficient bonding to cover the w ork being performed. Bonds and other payment protections shall be provided w thin 10 calendar days of aw ard.

INSTRUCTIONS TO PROPOSERS
GENERAL INFORMATION
A.SITE VISIT:
A site visit will be held on Wednesday, 03 August 2022 at 9:00 am (Arizona Time). Only one site visit will be conducted and no requests for additional site visits or make-up site visits will be granted.
1. All vendors needing access to the military installation enter through the Van Deman Gate of Fort Huachuca, AZ, located at the corner of State Highway 90 Bypass and State Highway 90.
2. Base Access: Fort Huachuca is not an open base. Contractors must pass Criminal Background Checks and be vetted prior to being allowed access. Passes are available at the Van Deman Gate.
3. For more information call: Van Deman Gate (East Gate) & Visitor Control Center at 520-533- 3269 for any questions.

E. BASIS FOR CONRACT AWARD:
1. The Government will award a Firm Fixed-Price Contract that will be made on the basis of the lowest evaluated price of proposals meeting or exceeding the acceptability standards for non-cost factors of the Solicitation.
2. To assure timely and equitable evaluation of proposals, offerors must follow the instructions contained herein. Proposals shall be complete and include all information the offeror wants the Government to consider and evaluate and conform to all required terms and conditions.
PROPOSAL INSTRUCTIONS
F.PROPOSAL SUBMISSION:
PROPOSAL PREPARATION
Technical:
1. Technical Proposals shall address all the elements required in the attached SOO.
2. The contractor’s proposed schedule shall be submitted with the proposal. The schedule shall be in the form of a progress chart of suitable scale to indicate the percentage of work scheduled for completion at any given date during the period.
3. Technical Proposal and schedule submissions are limited to 15 pages.
4. Provide quantities for materials, and ensure that they coincide with your cost proposals.
5. Proposals shall be single spaced. Each paragraph shall be separated by at least one blank line. A standard, 12-point minimum font size applies. Arial or New Times Roman fonts are required. Tables and illustrations may use a reduced font size no less than 8-point and may be landscape.
Price:
1. A price for each CLIN shall be provided in your proposal. Pricing must be totaled in the proposal to show the total overall price as indicated in the price summary below.
2. An example price summary is provided below.

3.No price information shall be presented in any part of the proposal except the Price Proposal. Proposals shall be clear and concise, logically assembled (with all pages appropriately numbered) and referenced to applicable parts of the Performance Work Statement or Request for Proposal, as appropriate. To reduce proposal size, the Offeror will confine submission to essential matters sufficient to define the proposal, and provide an adequate basis for evaluation. File shall not contain classified data.
4.The contractor shall provide a labor break-out of all labor rates and loadings (all indirect rates, inclusive of labor indirect rates, G&A, and profit) that lead to the establishment of the firm fixed price. The contractor will provide a cost breakout for all major components and equipment.
5.Per FAR 36.204 Disclosure of the magnitude of construction projects the estimated price range for this requirement is $250,000 and $500,000.
Past Performance:
1. Past Performance related to Repair Bridge occurring in the past 5 years must be provided using the Past Performance Evaluation Questionnaire Form provided in the Contract Forms Section of this solicitation. Also provided as an Attachment to the solicitation.

4. Additional Sources of Past Performance Information for Evaluation which may be used by the contracting office to determine acceptability of past performance. Sources such as the following maybe used:
• Past performance information may be provided by the offeror, as solicited.
• Past performance information may be obtained from established systems such as the Contractor Performance Assessment Reporting System (CPARS), Federal Awardee Performance and Integrity Information System (FAPIIS), Electronic Subcontract Reporting System (eSRS), or other databases; and may be obtained from other sources available to the Government, such as the Defense Contract Management Agency; and interviews with Program Managers, Contracting Officers, Fee Determining Officials. Although the Government may elect to consider data from other sources, the burden of providing detailed, current, accurate and complete past performance information rests with the Offeror.

BASIS FOR AWARD
I. PROPOSAL EVALUATION:
CRITERIA/FACTORS:
1. The Government will award a firm fixed price contract. The Contract will be awarded on a Lowest Price Technically Acceptable (LPTA) basis using the following factors:

The above ratings will be used to determine if the proposal meets Technical requirements. All subfactors must be rated as acceptable for the overall technical rating to be determined acceptable.

c. FACTOR 3: Past Performance:
1. Experience provided by the contractor that is related to Repair Bridge in the past 5 years will be evaluated. Please limit to no more than 3 of the most relevant projects.
2. Past Performance information must be submitted using the Past Performance Questionnaire form provided in the Contracting Forms Section. Past Performance will be assigned an adjectival rating based on the criteria below:
Adjectival Rating Description Acceptable Based on the offeror’s performance record, the Government has a reasonable expectation that the offeror will successfully perform the required effort, or the offeror’s performance record is unknown (a neutral rating shall be considered “acceptable”).
Unacceptable Based on the offeror’s performance record, the Government does not have a reasonable expectation that the offeror will be able to successfully perform the required effort.
3. The contractor must provide sufficient Past Performance Information to be assessed an adjectival rating of Acceptable.
NOTE: In the case of an offeror without a record of relevant past performance or for whom information on past performance is not available or so sparse that no meaningful past performance rating can be reasonably assigned, the offeror may not be evaluated favorably or unfavorably on past performance (see FAR 15.305 (a)(2)(iv)). Therefore, the offeror shall be determined to have unknown (or "neutral") past performance. In the context of acceptability/unacceptability, a neutral rating shall be considered "acceptable."
OTHER DETAILS
1. This requirement, when awarded, will follow and incorporate the applicable FAR, DFARS, and other applicable Federal, State, and Local Statutes, Codes, or Regulations.
2. Government intends to evaluate and award without discussions; however, if deemed necessary, notification will be provided electronically or by phone.
3. Notice to Proceed: Notice to Proceed will not be issued until the Contractor provides sufficient bonding to cover the work being performed. Bonds and other payment protections shall be provided within 10 calendar days of award of a Contract.
4. DD FORM 1354: The contractor shall prepare a DD Form 1354 Transfer and Acceptance of Military Real Property near completion of the project and submit it to the Government prior to final inspection and acceptance. The contractor may obtain a DD Form 1354 Transfer and Acceptance of Military Real Property worksheet from the DPW COR.
5. Wage Determination: The applicable Wage Rate Requirement for this Contract is:
General Decision Number: AZ20220023 “Building” dated 06/10/2022.
6. The North American Industry Classification System (NAICS) code that applies to this requirement is 236220 with a size standard of $39.5M.
7. Payment for Materials Stored on Site: Payment in advance of installation for materials stored on site will not be considered under this Contract.
8. Provide a fully executed Bid Bond as required by FAR 52.228- 1 Bid Guarantee. Submit a scanned copy of the completed bid bond, Standard Form 24, electronically with the proposal.
Original bid bond must be received by this office within five calendar days after proposal due date. The Government will review the Bid Bond for legal sufficiency.
9. This requirement will be paid using Wide Area Work Flow (WAWF).
10. The proposal shall be valid for 90 calendar days.

52.204-8 ANNUAL REPRESENTATIONS AND CERTIFICATIONS (MAY 2022)
(a)(1) The North American Industry Classification System (NAICS) code for this acquisition is 236220.
(2) The small business size standard is 39.5M.
(3) The small business size standard for a concern that submits an offer, other than on a construction or service acquisition, but proposes to furnish an end item that it did not itself manufacture, process, or produce is 500 employees if the acquisition--
(i) Is set aside for small business and has a value above the simplified acquisition threshold;
(ii) Uses the HUBZone price evaluation preference regardless of dollar value, unless the offeror waives the price evaluation preference; or

52.204-21 BASIC SAFEGUARDING OF COVERED CONTRACTOR INFORMATION SYSTEMS (NOV 2021)
(a) Definitions. As used in this clause--
Covered contractor information system means an information system that is owned or operated by a contractor that processes, stores, or transmits Federal contract information.
Federal contract information means information, not intended for public release, that is provided by or generated for the Government under a contract to develop or deliver a product or service to the Government, but not including information provided by the Government to the public (such as on public websites) or simple transactional information, such as necessary to process payments.
Information means any communication or representation of knowledge such as facts, data, or opinions, in any medium or form, including textual, numerical, graphic, cartographic, narrative, or audiovisual (Committee on National Security Systems Instruction (CNSSI) 4009).
Information system means a discrete set of information resources organized for the collection, processing, maintenance, use, sharing, dissemination, or disposition of information (44 U.S.C. 3502).
Safeguarding means measures or controls that are prescribed to protect information systems.
(b) Safeguarding requirements and procedures.
(1) The Contractor shall apply the following basic safeguarding requirements and procedures to protect covered contractor information systems. Requirements and procedures for basic safeguarding of covered contractor information systems shall include, at a minimum, the following security controls:
(i) Limit information system access to authorized users, processes acting on behalf of authorized users, or devices (including other information systems).
(ii) Limit information system access to the types of transactions and functions that authorized users are permitted to execute.
(iii) Verify and control/limit connections to and use of external information systems.
(iv) Control information posted or processed on publicly accessible information systems.
(v) Identify information system users, processes acting on behalf of users, or devices.
(vi) Authenticate (or verify) the identities of those users, processes, or devices, as a prerequisite to allowing access to organizational information systems.
(vii) Sanitize or destroy information system media containing Federal Contract Information before disposal or release for reuse.
(viii) Limit physical access to organizational information systems, equipment, and the respective operating environments to authorized individuals.
(ix) Escort visitors and monitor visitor activity; maintain audit logs of physical access; and control and manage physical access devices.
(x) Monitor, control, and protect organizational communications (i.e., information transmitted or received by organizational information systems) at the external boundaries and key internal boundaries of the information systems.
(xi) Implement subnetworks for publicly accessible system components that are physically or logically separated from internal networks.
(xii) Identify, report, and correct information and information system flaws in a timely manner.
(xiii) Provide protection from malicious code at appropriate locations within organizational information systems.
(xiv) Update malicious code protection mechanisms when new releases are available.
(xv) Perform periodic scans of the information system and real-time scans of files from external sources as files are downloaded, opened, or executed.
(2) Other requirements. This clause does not relieve the Contractor of any other specific safeguarding requirements specified by Federal agencies and departments relating to covered contractor information systems generally or other Federal safeguarding requirements for controlled unclassified information (CUI) as established by Executive Order 13556.
(c) Subcontracts. The Contractor shall include the substance of this clause, including this paragraph (c), in subcontracts under this contract (including subcontracts for the acquisition of commercial products or commercial services, other than commercially available off-the-shelf items), in which the subcontractor may have Federal contract information residing in or transiting through its information system.

52.204-24 REPRESENTATION REGARDING CERTAIN TELECOMMUNICATIONS AND VIDEO
SURVEILLANCE SERVICES OR EQUIPMENT (NOV 2021)
The Offeror shall not complete the representation at paragraph (d)(1) of this provision if the Offeror has represented that it "does not provide covered telecommunications equipment or services as a part of its offered products or services to the Government in the performance of any contract, subcontract, or other contractual instrument" in paragraph (c)(1) in the provision at 52.204-26, Covered Telecommunications Equipment or Services-- Representation, or in paragraph (v)(2)(i) of the provision at 52.212-3, Offeror Representations and Certifications- Commercial Products and Commercial Services.
