Statement of Need (SON) Computerized Dynamic Posturography/Interactive Virtual Reality (CDP/IVR) System Department of Veterans Affairs

1. Purpose The purpose of this Statement of Need (SON) is to outline the procurement, delivery, installation, training, and support for a CDP/IVR System, a state-of-the-art computerized dynamic posturography and interactive virtual reality rehabilitation system at Columbia VAMC. The system will provide comprehensive balance assessment and therapeutic capabilities to enhance rehabilitation services provided to Veterans.

2. Scope The Contractor shall provide all necessary equipment, software, hardware, consumables, onsite installation, clinician training, warranty, and ongoing support needed to fully implement the Bertec CDP/IVR System or equal at building 31 at 6439 Garners Ferry Road, Columbia, SC 29209.

3. Salient Characteristics All CDP/IVR systems proposed must meet or exceed the following salient characteristics:

Physical and Technical Specifications Maximum installation ceiling height: 102 inches Maximum patient weight capacity: 450 pounds Accommodates patients up to 6 feet 7 inches tall Platform height: 4 inches for easy access Allows clinician to be on platform with patient during assessment/training for enhanced comfort and safety No visual obstructions, head/neck restrictions, or physical barriers to patient motion except adjustable safety harness Diagnostics and Rehabilitation

· 2 degree-of-freedom, smooth-moving force plate with both anterior/posterior translations and rotations

· Fully immersive, real-time adjustable virtual environment (visual surround, no physical or visual limitations)

· Dual-balance dynamic force plates with customizable movement profiles for training, including dual-task activities

· Real-time, objective data and user-defined adjustable parameters for thousands of training combinations

· Assessment Capabilities

· Minimum eight assessments with 3rd-party research-validated normative data:

· Sensory Organization Test (SOT)

· Motor Control Test (MCT)

· Adaptation Test (ADT)

· Head Shake SOT (HS-SOT; optional)

· Limits of Stability (LOS)

· Unilateral Stance (US)

· Weight Bearing Squat

· Rhythmic Weight Shift

· Additional off-platform assessments (when optional static plate is connected): mCTSIB, Instrumented BESS, COBALT, Sit to Stand, Step Up and Over, Tandem Walk, Walk Across, Step Quick Turn, Forward Lunge Virtual Environment & Therapeutic Modules

· More than 75 training modules and over 130 immersive VR environments

· Provides extensive real-time and adjustable parameters, supporting a patient-tailored "crawl, walk, run" approach

· 7 Optical flow virtual environments with real-time options, including:

· Optokinetics (speed, direction, density adjustable)

· Grocery (velocity, motion profile, scene density/complexity)

· Castle, Airplane, Driving, Park, Boardwalk (adjustable by multiple parameters and distraction/cognitive task integration)

· Medical software supports Perturbations training in a virtual setting (adjustable speed, distance, rotational, horizontal translational movements)

· Enables protocol series saving for future patient use

· Platform motion profiles fully customizable

· Incorporates dual task rehabilitation activities not available in any other CDP product Performance Platform vertical force sensitivity: avg. 0.02 kg Average COP accuracy: 0.25 mm (COPx, COPy) Hardware/Software Features:

· Dedicated high-end VR-ready computer

· Touchscreen monitor, keyboard, mouse, wireless remote for clinician

· Height-adjustable workstation on wheels; accessory tray; color printer

· Accessories: 1 rocker board, 1 foam pad

· 2 emergency stops for safety: system-mounted and portable

· Integrated safety harness structure

· Wireless HS-SOT headband (adjustable for any head size)

· Fully Immersive Sway Referencing Visual Surround and scenes that encompass full patient visual field

· Allows patient visibility for assessments

· Option to connect static platform for protocols outside the VR environment Service, Warranty, & Support

· 1-year warranty covering parts and labor

· Dedicated regional service teams

· Optional preventative maintenance package available

· Technical support during VA business hours

· Required service/maintenance visit response within 72 hours

· Training

· Onsite clinical and technical staff training (minimum 4 hours, up to 10 personnel)

· Training in quick options: mobility, closed chain, weight-shifting, vision, perturbations

· User and operational manuals provided

4. Deliverables

· Fully installed and tested CDP/IVR system

· All hardware, accessories, and software as described

· Initial comprehensive onsite training for staff

· User manuals, warranty documentation, and technical support information

5. Period of Performance Project initiation: upon award Delivery/installation: preferable within 30 days Training: at time of installation Warranty/support: one year post-installation

6. Security and Privacy Contractor personnel shall comply with all applicable VA and federal security, privacy, and facility requirements when providing delivery, installation, and support services. The CDP/IVR system, including any data transmission/storage, must adhere to HIPAA and VA Sensitive Data guidelines.
