The certification must be signed by an authorized official with printed name, title, and date. An optional section allows offerors to provide clarifying remarks regarding their performance approach or subcontracting strategy. The template serves to ensure compliance with federal subcontracting limitations and allows the government to verify appropriate utilization of small business subcontractors.

Compliance with FAR 52.219-14, Limitations on Subcontracting

Offeror Self-Performance & Subcontracting Disclosure Checklist Offerors shall complete all sections. Incomplete responses may render the quote ineligible for award.

1. Performance Approach

1. Will the Prime Contractor self-perform the required work?

☐ Yes. Prime will self-perform the required percentage of work with its own employees.

☐ No. Prime intends to subcontract a portion of the work.

If “Yes,” skip Sections 2–4 and proceed to Section 5.

2. Subcontracting Intent Identify whether subcontracting is planned:

☐ No — The Prime will not utilize subcontractors. (If “No,” return to Section 1A and correct your response.)
