PERFORMANCE WORK STATEMENT (PWS)
FOR

1.0 GENERAL
1.1 Scope: This Performance Work Statement (PWS) defines the repair and certification requirements for an OHARNG M105 Tank, Liquid Storage (HIPPO) unit serial number 1138. Contractor will complete all structural repairs at the contractor’s location. The OHARNG will provide transportation of the M105 to and from the contractor’s location.
1.2 Background: The M105 HIPPO sustained structural damage to the metal frame, resulting in a reduction in structural integrity. Welding, repainting, and certification must be completed to return structural integrity to the HIPPO, to include painting and zero defect certification.
1.3 Place and Performance of Service and Repair: The contractor shall provide services off-site of any government facility, primarily the contractor’s sole location of operations. On Monday through Friday, with regular, predictable business hours that provide the government the ability to perform quality assurance inspections. The contractor shall at all times maintain an adequate work force for the uninterrupted performance of all tasks defined within this PWS to complete the service in a timely manner. When hiring personnel, the contractor shall keep in mind that the stability and continuity of the work force are essential.
1.4 General Information:
1.4.1 Requirements: Repair all structural defects/damage under AWS D1.1 certification standards, with all repairs certified by a Certified Weld Inspector to AWS QC1 standards. Repaired areas re-painted using MIL-SPEC and matching-color Chemical Agent Resistant Coating (CARC) paint. Magnetic Particle Inspection will document completion with zero defects.
1.4.2 Quality Control (QC): The contractor shall develop and maintain an effective QC Plan (QCP) to ensure service and repairs are performed in accordance with this PWS.
The contractor shall develop and implement procedures to identify, prevent, and ensure complete repair of defective components of the HIPPO (water tank). The contractor’s QCP is the means by which it assures itself that the work complies with the requirements of the contract. At a minimum, the contractor shall develop QC procedures that address the areas identified in Technical Exhibit 1, Performance Requirements Summary. After acceptance of the QCP, the contractor shall obtain the Contracting Officer’s (KO) acceptance in writing of any proposed changes to its
QCP.
1.4.3 Quality Assurance (QA): The Government shall evaluate the contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan (QASP). This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and acceptable quality level(s) (defect rate(s)).

EVALUATION OF OFFERS
EVALUATION CRITERIA AND BASIS FOR AWARD
The Government will award a firm fixed-price task order resulting from this solicitation to the responsible offer conforming to the solicitation will be the most advantageous to the Government, price and other factors considered. The following shall be used to evaluate offers:
Past Performance and Price
Offerors are reminded that the evaluation process is subjective despite the use of factors as outlined below.
Past Performance Evaluation: The Government's approach to evaluating past performance is to review past performance information provided by the Offeror to determine if the past performance is recent and relevant. To demonstrate recent past performance the Offeror shall provide documentation of past performance for the Army National Guard or any other Military organization within the last three (3) years including references, with names and contact information of the references who can verify such past performance. To demonstrate relevancy, offeror should provide documentation of performance on past or current contracts of a similar type, size, scope and complexity. In the case of an Offeror without a record of relevant past performance or for whom information on past performance is not available, the Offeror will be evaluated as neutral on Past Performance, providing the Offeror supplies a certified statement with their offer that no past performance information is available.
Neutral past performance is not negatively scored but may have a negative impact in the overall consideration for award. The Government may review any past performance information reported to Contractor Performance Assessment Reporting System (CPARS) and any data obtained from other sources available (i.e.
government database, past contract files, etc). The ratings for past performance will be as follows:
The Offeror is cautioned that while the Government will consider data from other sources, the burden of demonstrating satisfactory past performance rests with the Offeror. Limit Past Performance documentation to four pages.
Price Evaluation: All quotes submitted will be evaluated for Past Performance and Price. The Government will issue an award to the Offeror whose proposal is the most advantageous to the Government but not necessarily the lowest price. If all things are equal, the Offer which is most advantageous to the Government may also be the lowest price determined to be reasonable IAW FAR 12.209. Offeror's are encouraged to propose their best price including any available discounts. The Government will evaluate offers for award. If the Government determines that adequate price competition was not obtained and/or if the total evaluated price cannot be deemed fair and reasonable, the Government reserves the right to request other than certified cost or pricing data to evaluate such data utilizing the proposal analysis techniques delineated in FAR 15.404-1 as deemed appropriate.
The resulting contract from this solicitation will be a firm fixed price task order. The Government intends to make a single award for this opportunity.
The Government reserves the right to accept other than the lowest priced offer and reject any and all offers. The basis of award will be a conforming offer, the price or cost of which may or may not be the lowest.
The Government intends to make award of a conforming proposal without discussions.
