NOTE: CPs may only approve non-certified anchorages that are capable of holding 5000 lbs. All other anchorages and horizontal lifelines must be certified by a QP.

f. End User: A person who has been trained and authorized by the employer on the use of assigned fall protection equipment in a typical fall hazard situation. They must have an understanding of workplace activities and follow the policy, procedures, and instructions of the Competent Person regarding the use of fall protection, rescue systems, and related equipment.

They shall:

i. Be trained by a CP in accordance with EM 385-1-1, Chapter 21-3. d, to include hands-on training and practical demonstrations;

(3.6 m) FF energy absorbing lanyard shall be used in accordance with manufacturer's instructions and recommendations. A 12 ft (3.6 m) FF energy absorbing lanyard does not refer to the lanyard length. Instead it refers to a FF that is greater than 6 ft. (1.8 m) up to 12 ft. which is created by the anchor point being located below the dorsal D-ring.

The maximum length of the lanyard used shall not exceed 6 ft. (See EM 385-1-1, Chapter 21-8. c.

4. Existing Conditions -

a. Gatehouse:

i. The Union Village Dam gatehouse is a two-story (2) masonry structure with a slate roof located on the Eastern end of the dam with access from Main St. in East Thetford, VT.

ii. Entrance to the building is via double hung metal doors measuring twelve feet five inches high by seven feet four and three-quarter inches (12' 5" x 7' 4 3/4") that open outward with a three foot by six-foot eight inch (3' x 6' 8") pass door that opens inward. Once inside the building there are two (2) levels, the operations floor and basement substructure. The operating floor houses the gate operators, and the basement substructure contains the gate well openings and furnace.

b. Chimney:

i. Exterior -

1) The existing brick chimney measures approximately two feet six inches long by two feet six inches wide by fifty-two feet six inches tall (2' 6" x 2' 6" x 52' 6"), with a bluestone rain cap constructed atop a concrete curb.

2) It consists of a four (4) sided structure with alternating courses keyed into the exterior masonry of the gatehouse.

3) It is structurally compromised, exhibits a significant vertical crack, and multiple step cracks in the masonry. This damage was caused by a previously installed, improperly sized, and uninsulated flue liner which led to excess condensation. The chimney wash (or crown) is also cracked, allowing for water infiltration.

4) Oil fill pipe, box, and vent pipe shall be protected from damage; if removed they shall be capped or plugged to prevent rain and debris from entering.

ii. Interior -

1) An existing, thimble on the operating floor is capped, but not tightly sealed. Rust has formed around the outside and does not create a tight seal with the brick allowing exhaust to enter the gatehouse.

2) Many of the glazed bricks around the thimble show signs of failure to include delamination due to excess moisture.

iii. Furnace:

1) Situated on the basement level of the gatehouse and accessible by two (2) sets of stairs, the AirCo BCL190 lowboy oil furnace (194,000 BTU, 1.65 GPH) serves as the primary heat source for both the basement and operations floor of the gatehouse.

5. Work Plan - The Contractor shall submit a written Work Plan describing the means, methods and equipment required to accomplish the work. This plan shall also address how the site will be restored following the completion of work.

6. Other Contracts - The Government may undertake or award other contracts for additional work not related to this contract, and the Contractor shall fully cooperate with other contractors and Government employees. The Contractor shall not commit or permit any act which will interfere with the performance of work by another Contractor or by Government employees.

7. Clean Up and Waste Disposal - The Contractor shall practice good housekeeping to maintain a safe job site. The Contractor shall keep the work area, including any designated storage areas free from accumulation of waste materials. Upon completing work in an area the Contractor shall remove any tools, equipment, and materials that are not the property of the Government. Any and all disturbed areas resulting from Contractor activities shall be restored by the Contractor to the satisfaction of the Technical Point of Contact. All waste material will be disposed of offsite in accordance with all federal, state, and local regulations.

8. Environmental Protection - Containers for excess and/ or waste materials, rubbish, etc. shall be provided by the Contractor at the site, and the site will be inspected/ cleaned on a daily basis.

Water, air, and land resources shall not be adversely impacted during the course of the work.

Storage, fueling and lubrication of equipment and motor vehicles shall be conducted in a manner that affords the maximum protection against spills and evaporation. Storage and usage of lubricants and daily fueling of machinery and equipment shall occur at least fifty feet (50') from any drainage inlets, water bodies (rivers, streams, and lakes), environmentally sensitive areas (wetlands and vernal pools). Contractor will take necessary steps to ensure all federal, state, and local environmental regulatory requirements are met.

The Contractor shall keep on site the means to quickly cover and protect all areas open to the elements during an unforeseen weather related event that could cause damage to the building and or any of its contents during all phases of this contract.

11. Personal Protective Equipment - It is the Contractor's responsibility to furnish and supply personnel with the proper personal protective equipment necessary for performing the work.

12. Government Resources - The Government will supply any electricity necessary for the project work from points of existing outlets when available. The Contractor shall carefully conserve the use of electricity provided. The Contractor shall supply their own means of water and communication (telephone).
