PERFORMANCE WORK STATEMENT

PERFORMANCE WORK STATEMENT (PWS)

MILITARY HEALTH SYSTEMS SUPPORT (MHSS)

DWIGHT DAVID EISENHOWER ARMY MEDICAL CENTER (DDEAMC)

FORT GORDON, GA

Part 1

General Information

1. General: This is a non-personal services contract to provide Information Management and Information Technology (IM/IT) services to the Dwight David Eisenhower Army Medical Center (DDEAMC). The Government shall not exercise any supervision or control over the contracted service professionals performing the services herein. Such contracted service professionals shall be accountable solely to the Contractor who, in turn is responsible to the Government.

In an effort to provide a more flexible, adaptable, effective and integrated system to manage our facilities, the Defense Health Agency (DHA) is assuming administration and management responsibilities from the Army, Navy and Air Force for all military hospitals and clinics. DHA will initially oversee these facilities through a direct support relationship with the Military Medical Department intermediate management organizations. The DHA will relieve the Military Departments of this support during a transition period in which responsibility for specific health care and administrative functions are fully transferred from the Military Departments to the DHA. Currently, DDEAMC is transitioning under the Defense Health Agency (DHA) and areas of coverage are subject to change.

1.1 Description of Services/Introduction: The Contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, other items necessary to perform Military Health Systems Support as defined in this Performance Work Statement except for those items specified as Government furnished property and services. The Contractor shall perform to the standards in this contract.

This PWS defines the MHSS Management/Information Technology technical services support. It delineates the duties, responsibilities and deliverables for the selected Contractor personnel and further defines the skill sets and the educational background required for staffing of the positions created and sustained under this PWS.

1.2 Background: DDEAMC provides medical support to 3,000+ active duty military members, 2,000+ family members, and a substantial number of retirees at Ft. Gordon and its remote clinics. Additionally, healthcare support is provided and maintained for the surrounding Central Savannah River Area (CSRA), Charlie Norwood Veteran's Administration Medical Center (CNVAMC), Georgia Regents University (GRU) and Doctor's Hospital.

The Military Health Systems Support (MHSS) provides technical Information Management and Information Technology services support to DDEAMC and its environments. These services include the Customer Support Center (CSC), Web Development, Networking, Project Management and Medical Illustration. Changes to the task environment combined with increasing workload and technical complexity arising from the number of new automated health care system initiatives at DDEAMC and its subordinate medical treatment facilities (MTF) has necessitated modification to the existing staffing and support structure.

The IM department currently develops, integrates and maintains information technology and communication capabilities onsite in support of healthcare delivery systems within DDEAMC consisting forty-five (45) outlying clinics. This includes Rodriguez Army Health Clinic (RAHC) Fort Buchanan, Puerto Rico and Southern Command (SOUTHCOM), Miami, FL and its sub-unified command operationally assigned to Special Operations Command South (SOCSOUTH), Homestead Air Reserve Base, FL and Camp Shelby, Hattiesburg, MS.

RAHC develops, integrates and maintains information technology and communication capabilities with onsite personnel in support of healthcare delivery systems in Puerto Rico.

Remote support, collaboration and infrequent travel is provided at SOUTHCOM and its sub-command, SOCSOUTH, Miami, FL.

Remote support, collaboration and infrequent travel is provided to Camp Shelby, Hattiesburg, MS.

1.3 Objectives:

Support the DDEAMC Health Care mission by providing prompt, knowledgeable, consistent and customer focused IT support services.

Align the CSC contract with the Information Technology Infrastructure Library (ITIL) service model through best practices and will increase accuracy in service application and measurements of success.

Champion customer focus throughout IMD by providing a single point-of-contact service support strategy.

Measure customer satisfaction with overall quality of service and costs, when applicable.

1.4 Scope: This PWS establishes and describes the current major functions required to support Military Health Systems at DDEAMC. This effort includes duties in support of the CSC, Web Development, Networking, Project Management, Cyber Security and Medical Photography for DDEAMC and its supported locations.

1.5 Period of Performance: The period of performance shall be for one (1) twelve month Base Period and three (3) twelve (12) month option periods.

1.6 General Information

1.6.1 Quality Control: The Contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this PWS. The Contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The Contractor’s quality control program is the means by which he assures himself that his work complies with the requirement of the contract.

1.6.2 Quality Assurance: The Government shall evaluate the Contractor’s performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the Contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s).

1.6.3 Federal Government Holidays: Contractors employees shall adhere to an eight (8) hour workday with the Customer Support Center being staffed from 0600-1700 (5:00 p.m.), Monday-Friday, except during federal holidays (area will close). DDEAMC recognizes federal holidays listed below:

New Year’s Day 1st day of January Martin Luther King Jr.'s Birthday 3rd Monday of January Presidents Day 3rd Monday of February Memorial Day last Monday of May Independence Day 4th day of July Labor Day 1st Monday of September Columbus Day 2nd Monday of October Veterans Day 11th day of November Thanksgiving Day 4th Thursday of November Christmas Day 25th day of December

1.6.4 Hours of Operation: The Customer Support Center shall be open and conduct business, between the hours of 0600-1700 (5:00 p.m.), Monday through Thursday and 0600-1500 (3:00 p.m.) on Fridays, leaving two (2) hours weekly for administrative and training duties, except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings.

All other departments including Project Management, Cyber Security, Web Administration and Audio Visual shall be open and conduct business between the hours of 0730-1700, Monday through Thursday and 0730-1500 on Fridays, leaving two (2) hours weekly for administrative and training duties with same exceptions stated for Customer Support Center related to closures.

Southern Command (SOUTHCOM), Miami, FL and its sub-command is handled remotely during the normal CSC hours noted above.

RAHC shall be open and conduct business, between the hours of 0700-1600 (4:00 p.m.), Monday through Thursday and 0700-1400 (2:00 p.m.) on Fridays, leaving two (2) hours weekly for administrative and training duties, except Federal holidays or when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings.

For Firm Fixed Price contracts and other than Firm Fixed Price contracts, the Contractor shall not be reimbursed when the Government facility is closed for the above reasons. The Contractor shall at all times maintain an adequate workforce for the uninterrupted performance of all tasks defined within this PWS when the Government facility is not closed for the above reasons.

When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the workforce are essential. The Contractor will normally work an 8 hour day/40 hour work week. Additional hours may be required for emergency/contingency operations after normal working hours. In the event of an emergency, the Contractor shall provide an emergency response within 4 hours (either by returning to work, responding by telephone or computer) in order to avoid significant service disruption; to avoid placing employees or the public in unsafe situations; to protect and/or provide emergency services to property or equipment; or to respond to emergencies with patients and/or providers. Contractors are authorized to work from an Alternate duty/remote location during emergency situations within the Fort Gordon area of operations with the approval of the Contracting Officer Representative and Contracting Officer. Contractor shall continue to meet tasks, deliverables, and current level of support in accordance with contract. Government reserves the right to approve, disapprove, or cancel contract personnel working from an alternate duty/remote location. Working from a remote location will be at no additional expense to the government. The Government does not authorize the use of off-site rates to accomplish alternate duty/remote location work and will not be responsible for any associated costs. Personal or company issued equipment may not be used to access the government network infrastructure unless pre-approved; examples include using VPN or a corporate network. However, inability to access the government network infrastructure using authorized devices does not necessarily prohibit the contractor from performing off-site, whenever the work could be performed without such access.

1.6.5 Place of Performance: The work shall be performed under this contract at DDEAMC, Fort Gordon, GA, which consists of the hospital building and approximately forty-five (45) outlying buildings on Fort Gordon and RAHC, Fort Buchanan, Puerto Rico. Additionally, Contractor employees are required to provide support remotely with infrequent travel to SOCSOUTH, SOUTHCOM, Miami, FL and Camp Shelby, Hattiesburg, MS. Contractor employees may also be required to make infrequent trips to support DDEAMC customers at other Augusta, GA area locations, such as the Charlie Norwood Veteran’s Administration Medical Center (CNVAMC), Georgia Regents University (GRU) formerly Medical College of Georgia (MCG) and Doctor’s Hospital. Future requirements may add more local offsite RTF’s affiliated with Fort Gordon that require varied levels of support and may increase the frequency of travel off post in Augusta, GA. A Government vehicle shall be provided for travel between site buildings or to service calls off post.

In the event requirements necessitate scheduling an employee for routine duty at an offsite location, that facility shall be considered the work location and the employee shall be responsible for transportation. Work location for SOUTHCOM clinic employees shall include the SOUTHCOM clinic and associated support staff locations in the Miami, FL area which may include SOCSOUTH at Homestead Air Reserve Base and Camp Shelby, Hattiesburg, MS.

RAHC maintains IT and communication capabilities with onsite personnel in support of healthcare delivery and therefore does not require any travel.

1.6.6 Type of Contract: The Government will award as a hybrid which includes both Firm-Fixed-Price (FFP) and Cost type contract.

1.6.7 Security Requirements:

1.6.7.1 Contractor employees performing on this contract/task order must be a U.S. citizen. Contractor personnel (to include subcontractors) performing work under this contract must have a National Agency Check with Inquiries (NACI) at work performance start date and must maintain the level of security required for the life of the contract.

The Contractor is responsible for acquiring the clearances. The Contractor shall ensure that all assigned personnel understand applicable security policies and directives found in DoD 5220.22-M, National Industry Security Program Operating Manual (NISPOM); AR 380-5, Information Security Program; and all other applicable policies and regulations.
