About this file

This is a Solicitation Survey form (Solicitation No. W912WJ26QA140) that vendors must complete to be compliant with the solicitation requirements. Failure to provide a completed survey will result in the quote being deemed non-responsive.

The survey requires vendors to provide company identification information including name, SAM CAGE Code, address, and point of contact details, along with business type classification. All information submitted must be that of the prime vendor, not subcontractors, and must be substantiable by SAM registration. Vendors must document their years of experience in the relevant services and federal contracting, including prime and subcontractor experience. Question 4 requires vendors to disclose any failures to complete awarded work, with verification that vendors checking "No" but having associated terminations will be removed from consideration. Question 5 addresses key vendor personnel availability, including minimum and maximum employee counts, payroll status, and anticipated staffing needs for the project.

The survey requires detailed employee experience information for all personnel performing work and mandates submission of specific certifications or documented plans for obtaining them, including First Aid/CPR, Fall Protection Competent Person, Rope Access, applicable trade licenses, Crane Operator/Qualified Riggers credentials, and Boat Operator certifications. Subcontractor information must include name, point of contact, and location, with teaming arrangement documentation attached; vendors stating information will be provided later will be deemed non-responsive. Question 10 requires three contract references completed within the past three years with similar scope, size, and complexity, preferably government contracts, including full contact information for points of reference and detailed project scope descriptions. All statements must be certified by an authorized official as complete and correct.
