About this file
This document is a solicitation attachment for a Minnesota state contract opportunity containing responder declarations, pricing instructions, and additional information requirements. The attachment establishes mandatory certifications that all responding vendors must complete, including attestations regarding the accuracy and truthfulness of submitted information, authorization to contractually bind the responding organization, and non-collusion certifications. Respondents must also disclose any organizational conflicts of interest that could impair their ability to provide impartial assistance or objectivity in performing contract work, with immediate written disclosure required to the State's Chief Procurement Officer if conflicts are discovered post-award. The solicitation does not utilize a traditional price schedule document; instead, responders must enter bid prices directly into the SWIFT system at the unit line level, with the State reserving the right to reject responses containing unauthenticated alterations or erasures of pricing information.
Additional submission requirements include providing contractor contact information for both contract management and purchase order receipt, specifying any prompt payment discount terms offered from a defined list of options ranging from one to five percent discounts with payment terms between 10 and 30 days, or selecting net 30 or no discount options as applicable. Responders must also declare their delivery timeframe following receipt of order, with all information submitted subject to verification and potential disqualification or debarment proceedings if found to contain inaccurate or misleading content. The document indicates responders may contact the Vendor Assistance Help Desk at (651) 201-8100 for technical assistance with proposal submission in SWIFT.
