Solicitation - 36C26224R0118 Amendment 0002 - Response to Questions 1-15-25
Question #DocumentSectionPage #Questions SubmittedVA Response to Questions
1RFPB.2 Schedule of Services and Price7Will the Government provide the number of billable enrollees on the most current invoice and the average number of billable enrollees over the last 12 months?See 'Billable Enrollees' tab on this excel. Billable is from January 2024 through December 2024. January billable enrollees is not available yet.
2RFPB.2 Schedule of Services and Price7Will the Government provide the historical number of established/active patients for the past five years? The government can only provide billable enrollees and average over the last 12 months.
3RFPB.2 Schedule of Services and Price7-9The RFP is requesting contractors propose a separate price PMPM for PC-MHI, a different approach than in other VA CBOC solicitations. Would the VA consider a PC-PMPM that is inclusive of PC-MHI services, as this is generally considered an extension of primary care? We feel this approach offers the best value to the government and avoids two underlying concerns. The first concern, is that the VA historic data available for PC-MHI services for Alamogordo reflects that only 2.6% of Veterans at the site are currently engaged in PC-MHI, significantly less than the 15% VA is recommending contractors use for pricing. Using the 15% recommended by VA will result in lower pricing that current data indicates to be appropriate. For this reason, we would like to propose a single primary care price inclusive of PC-MHI and then take any specific enrollment goals VA established as part of the QASP. The second concern is the potential conflict of interest a self-referral with benefit creates, whereby the contractor is being incentivized to refer patients to itself and then directly benefiting from that referral. In most other settings, this is strictly prohibited. The separate PMPM for the PC-MHI services aligns with the goal of ensuring clarity and transparency in cost allocation that’s driven by the VA’s determination that PC-MHI services are distinct from primary care and may require unique resources, staffing, and management. This ensures these services are adequately resourced and their usage is accurately tracked. While PC-MHI may be considered an extension of primary care, the VA recognizes that these services often require specialized mental professional and infrastructure, which justifies the need for distinct pricing. The VA’s 15% recommendation reflects a standard benchmark for pricing based on historical data across similar sites. The VA aims to establish a consistent and fair pricing structure for all potential contractors. The VA remains committed to ensuring that all services provided through the CBOC are ethical and free from conflicts of interest. The VA will implement strict oversight measures, including detailed reporting and audits, to prevent any self-referral practices that may arise. Contractors will be required to comply with VA regulations prohibiting such practices.
