This is a competitive combined synopsis/solicitation for commercial services issued by the National Institutes of Health (NIH), National Heart, Lung and Blood Institute (NHLBI). The solicitation number is 75N98026Q01111, issued on August 27, 2026, with responses due by September 10, 2026 at 3:00 PM EDT. This is an unrestricted acquisition classified under NAICS code 561910 (Administrative and Support Services) with a size standard of $19.5 million.

The requirement is for Medical Data Labeling Services, specifically expert manual annotation of 235 contrast-enhanced cardiac CT imaging studies to create a comprehensive cardiovascular segmentation dataset. The contractor must annotate each study for 20+ anatomical structures including ventricular blood pools, myocardium, atrial structures, aorta, pulmonary vessels, coronary arteries, valves, and annular structures. Deliverables include a pilot batch of five annotated studies for NHLBI review and approval, followed by completion of the remaining 230 studies within approximately 12 weeks. All annotations must undergo quality review by a board-certified radiologist with cardiovascular CT experience and be delivered in NHLBI-specified formats. The contractor must maintain confidentiality, process data only within NHLBI-authorized systems, and refrain from repurposing or disclosing government-furnished de-identified imaging data. The contract is firm-fixed price with award to the responsible offeror offering best value based on price and technical capability. Invoices shall be submitted to the Department of Treasury's Invoice Processing Platform upon successful delivery and acceptance. Statement of Work (SOW)

1. Project Title

Cardiac CT Anatomical Segmentation Dataset Development

2. Introduction and Background

The Cardiovascular Intervention Laboratory within the Division of Intramural Research at the National Heart, Lung, and Blood Institute (NHLBI) develops and evaluates advanced imaging technologies to improve diagnosis, procedural planning, image-guided interventions, and treatment of cardiovascular disease. Research efforts within the program focus on the development of new methods for cardiovascular image reconstruction, anatomical visualization, image-guided interventions, and artificial intelligence-based image analysis.

A critical limitation affecting multiple ongoing research efforts is the lack of comprehensive, high-quality reference annotations for cardiovascular computed tomography (CT) imaging data. Modern artificial intelligence methods require expertly annotated datasets both for algorithm development and for rigorous evaluation of performance. While public cardiac CT datasets exist, they generally provide limited anatomical coverage, are derived from highly curated research cohorts, and do not adequately represent the diversity and complexity of clinical cardiovascular imaging encountered in NHLBI research studies. Existing datasets often omit clinically important structures such as valve anatomy, coronary venous structures, annular structures, and detailed vascular anatomy. Furthermore, publicly available annotations frequently do not include studies containing imaging artifacts, implanted cardiovascular devices, complex congenital or acquired anatomy, or other features commonly encountered in real-world clinical practice.

3. Statement of Need and Objectives

The Cardiovascular Intervention Laboratory requires a comprehensive set of expert annotations across a broad range of cardiovascular structures to create a reference dataset that reflects clinically relevant patient populations and imaging conditions. This requirement is driven by multiple active research projects that have progressed to stages where further development and validation depend upon availability of expert-annotated cardiac CT data. In particular, ongoing projects focused on reconstruction of three-dimensional cardiovascular anatomy from fluoroscopic and interventional imaging data require annotation of anatomical structures not available in public segmentation tools, as well as reference annotations to establish quantitative performance in NHLBI datasets.

Because generation of these annotations requires specialized expertise in cardiovascular anatomy, medical image segmentation, quality assurance, and radiologist review, we seek contractor support to perform large-scale annotation of cardiac CT imaging studies.

Specifically, the objective of this effort is to obtain expert manual annotation of 235 contrast-enhanced cardiac CT studies to create a comprehensive cardiovascular segmentation dataset. The resulting dataset will be used to:

• Develop robust cardiac CT segmentation algorithms that generalize to real-world clinical data, which will support internal data analysis.

• Establish training data and ground-truth evaluation data for fluoroscopy-based synthetic CT reconstruction methods, an active area of ongoing research.

• Establish training data and ground-truth evaluation data for interventional cardiovascular CT

(iCVCT) reconstruction methods, an active area of ongoing research with external collaborators.

• Create training, validation, and testing datasets for future cardiovascular artificial intelligence research conducted by NHLBI.

4. Product Specifications/Features/Salient Characteristics

The contractor shall provide the following deliverables:

• Pilot annotation consisting of five (5) annotated cardiac CT studies for NHLBI review, iteration, and approval before full-scale production.

• Final annotation consisting of segmentation masks for 235 cardiac CT studies, including all required cardiovascular anatomical structures defined in the NHLBI annotation ontology. The 235 final scans includes final labels on the 5 pilot scans.

All deliverables shall be electronically transmitted to NHLBI and shall become the property of the United States Government upon acceptance.

Deliverables shall adhere to the following requirements:

• Each CT study shall be annotated for the following anatomical structures: left ventricular blood pool, right ventricular blood pool, left ventricular myocardium (including interventricular septum), right ventricular myocardium (free wall only), left atrium blood pool, right atrium blood pool, aorta, aortic root, left atrial appendage, pulmonary veins, pulmonary artery including left and right pulmonary arteries and second-order branches, inferior vena cava, superior vena cava, coronary arterial tree, left ventricular papillary muscles and trabeculation, right ventricular moderator band, papillary muscles, and trabeculation, coronary sinus with anterior interventricular vein, mitral valve leaflets (anterior and posterior), mitral annulus ring, aortic annulus ring, tricuspid annulus ring.

• Segmentation shall be done according to an agreed-upon labelling ontology.

• The contractor shall utilize NHLBI-provided segmentation masks where available as initialization and refine those annotations as necessary. Structures without initial masks shall be labelled manually.

• All final annotations shall undergo documented quality review by a board-certified radiologist with demonstrated experience in cardiovascular CT imaging. The contractor shall resolve identified annotation deficiencies before submission for final NHLBI acceptance.

• Final segmentation masks and associated annotation files shall be delivered in an NHLBI-specified format.

5. General Contractor Responsibilities

The contractor shall:

• Provide trained medical image annotators with relevant annotation experience.

• Provide project management support and routine communication regarding project status.

• Follow NHLBI-provided annotation guidelines and ontology definitions.

• Participate in iterative review cycles and correct annotations identified by NHLBI during quality review.

• Deliver all annotations within the agreed project timeline.

• Maintain confidentiality and protect all government-provided data.

6. Delivery or Performance Location

Work shall be performed remotely at contractor facilities using de-identified imaging data supplied by NHLBI. Annotation activities may be conducted through an NHLBI-approved third-party annotation platform. Final deliverables shall be electronically transmitted to NHLBI.

7. Delivery Date

The contractor shall deliver an initial pilot batch of five annotated studies for NHLBI review. Following approval of the pilot batch, the contractor shall complete annotation and delivery of the remaining studies within approximately twelve (12) weeks.

8. Inspection and Acceptance

NHLBI will review submitted annotations for completeness, consistency, and adherence to annotation guidelines. Acceptance criteria include:

• Successful completion of all required anatomical labels on 235 scans.

• Conformance with NHLBI-provided annotation specifications.

• Completion of radiologist quality review.

• Resolution of any deficiencies identified during NHLBI review.
