This is a Request for Quotation (RFQ) for roof repair and weatherization services issued by the Albuquerque Area Indian Health Service (IHS), a component of the Department of Health and Human Services. The solicitation number is 75H70726Q00069, with an offer due date of June 29, 2026 at 5:00 PM Mountain Daylight Time. The contracting officer is Stephanie Begay, reachable at (505) 256-6750 or Stephanie.Begay3@ihs.gov.

The acquisition is set-aside for Indian Small Business Economic Enterprises (ISBEE) concerns under a cascading structure, with a secondary tier available to other small business concerns if insufficient competition exists. The NAICS code is 238160 (Roof Repair) with a small business size standard of $19.0 million. Services are required for the Santa Fe Indian Health Center, 1700 Cerrillos Road, Santa Fe, NM 87505, with a one-year base period of performance (TBD/2026 – TBD/2027). This is a firm-fixed-price procurement for commercial services under FAR Part 12.

The primary scope involves comprehensive roof repair and weatherproofing of approximately 4,300 square feet of low-slope metal roofing on the Business Building, including roof edge repairs using foam closure strips, penetration and seam repairs with high-performance sealants, gutter and drainage work, and debris removal. Secondary scope includes visual roof condition assessments, limited spot repairs, and written condition reports with repair recommendations for three additional buildings: the OEHE Building (2,100 sq ft), Dental Building (3,700 sq ft), and Maintenance Shop (2,100 sq ft). The contractor must verify field measurements, provide qualified personnel, coordinate with the government representative, and warrant workmanship for one year from final acceptance. Excluded work includes structural roof deck replacement, full roof replacement or tear-off, electrical/plumbing/HVAC modifications, skylight replacement, mold remediation, and painting beyond the scope identified in the Statement of Work.

Quotations must be organized in three volumes: Volume 1 (Technical Qualifications) demonstrating minimum five years of roof repair experience and ability to perform all SOW deliverables; Volume 2 (Past Performance) documenting up to five recent and relevant contracts with customer information and points of contact; and Volume 3 (Price) with unit prices valid for 90 calendar days. Required documents include the signed SF-1449, IHS Buy Indian Act Indian Economic Enterprise Representation Form, and any signed amendments. Award will be made on a best-value basis with Technical Capability and Past Performance combined being more important than Price. Davis-Bacon Act wage determination NM20260048 (dated May 18, 2026) applies, and invoices must be submitted electronically via the Treasury Invoice Processing Platform (IPP) at https://www.ipp.gov with the contractor's Unique Entity Identifier (UEI) included. Contractors are responsible for all applicable federal, state, tribal, and local taxes, including New Mexico Gross Receipts Tax.

(A) Are □ are not □ presently debarred, suspended, proposed for debarment, or declared ineligible for the award of contracts by any Federal agency;

(B) Have □ have not □, within a three-year period preceding this offer, been convicted of or had a civil judgment rendered against them for: commission of fraud or a criminal offense in connection with obtaining, attempting to obtain, or performing a public (Federal, State, or local) contract or subcontract; violation of Federal or State antitrust statutes relating to the submission of offers; or commission of embezzlement, theft, forgery, bribery, falsification or destruction of records, making false statements, tax evasion, violating Federal criminal tax laws, or receiving stolen property (if offeror checks "have", the offeror shall also see 52.209-7, if included in this solicitation);

(1) The tax liability is finally determined. The liability is finally determined if it has been assessed. (c) A certification that any of the items in paragraph (a) of this provision exists will not necessarily result in withholding of an award under this solicitation. However, the Government will consider the certification in connection with a determination of the Offeror's responsibility. Failure of the Offeror to furnish a certification or provide such additional information as requested by the Contracting Officer may render the Offeror nonresponsible.

(d) This provision does not require establishment of a system of records in order to render, in good faith, the certification required by paragraph (a). The knowledge and information of an Offeror is not required to exceed that which is normally possessed by a prudent person in the ordinary course of business dealings.

(e) The certification in paragraph (a) of this provision is a material representation of fact upon which reliance was placed when making award. If the Government later determines that the Offeror knowingly rendered an erroneous certification, in addition to other remedies available to the Government, the Contracting Officer may terminate the contract resulting from this solicitation for default.

FAR 52.212-2 - Evaluation – Commercial Products and Commercial Services (Nov 2021)

(a) The Government will award a firm-fixed price purchase order (PO) resulting from this solicitation to the responsible quoter whose quote conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors (listed in order of descending importance) shall be used to evaluate quotes:

2. Past Performance

(c) A written notice of award or acceptance of a quote, mailed or otherwise furnished to the successful quoter within the time for acceptance specified in the quote, shall result in a binding contract without further action by either party. Before the quoter’s specified expiration time, the Government may accept a quote (or part of a quote), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. SECTION F - INSTRUCTIONS, CONDITIONS, AND NOTICE TO QUOTERS

EVALUATION (IHS SUPPLEMENTAL GUIDANCE)

(Supplementing FAR 52.212-2 and FAR 12.203)

Evaluation will be conducted in accordance with FAR Part 12 and FAR 52.212-2 using a comparative, best-value tradeoff evaluation.

Quotes shall be organized into three (3) volumes, plus required forms, as follows:

Volume 1 – Technical Qualifications Include the following:

• Quoter Name

• UEI Number

• Point of Contact (name, phone, email)

• A description demonstrating the quoter’s capability to perform all requirements in the Statement of Work (SOW)

• Copies of current licenses, certifications, and other credentials necessary to perform the required services

Evaluation Elements:

In evaluating Technical Qualifications, the Government will consider, at a minimum, the following:

• Demonstrated minimum five (5) years of experience in roof repair.

• Ability to perform all deliverables and tasks outlined in the Statement of Work (SOW)

• Ability to provide required reports that are accurate, complete, and legible in paper or electronic (PDF) format, as applicable.

The Government will assess strengths, weaknesses, and any risks associated with the quotation and determine the level of confidence in successful performance.

Volume 2 – Past Performance Past Performance will be evaluated based on the recency and relevance of prior contracts of similar scope and complexity.

• Recency: Contracts performed within the last 5 years

• Relevance: Similar scope and complexity

Provide up to five (5) recent and relevant contracts or jobs of similar scope. Include:

• Customer/organization

• Location

• Dates of performance

• Description of work

• Contract value (if available)

• POC name, phone, and email

In addition, IHS may evaluate:

• CPARS reports

• Other federal past performance records

• Subcontractor performance (IAW FAR 12.203(a)(2))

• Past performance information already known to the Government

A lack of past performance will not be considered negative. Past Performance confidence assessments will be considered in the best-value tradeoff in conjunction with Technical Qualifications and Price.

Project Title Roof Repair - SFSU

Volume 3 – Price Price will be evaluated for reasonableness, completeness, and balance using the unit prices submitted in the Pricing Table.

For evaluation purposes:

• The evaluated price will include the total price for the base period and all option periods.

• The Government may evaluate pricing for unbalanced pricing, including materially unbalanced unit prices or option period pricing.

• Prices shall be valid for at least 90 calendar days from the date of the quote submission.

The Government may consider whether the quoter’s pricing is consistent with the proposed technical approach and whether pricing presents a risk to successful performance.

The Government may reject any quotation with pricing that is incomplete, unbalanced, or inconsistent with the quotation’s technical approach.

Additional Required Documents

• Signed SF-1449 (first page only is acceptable).

• Signed IHS Buy Indian Act Indian Economic Enterprise Representation Form (IEE Form).

• Signed and acknowledged amendments, if any.

Award Decision Award will be made to the responsible quoter whose quotation conforming to the solicitation is determined to be most advantageous to the Government, price and other factors considered.

Technical Capability and Past Performance, when combined, are more important than Price. However, as quotations become more equal under the non-price factors, Price may become the determining factor for award.

The Government will perform a comparative evaluation of quotations and may select a higher-priced quotation if it is determined that the additional technical merit and/or past performance value warrants the additional cost.

The Government intends to award without discussions but reserves the right to conduct communications if determined to be in the Government’s best interest. The Government may conduct clarifications as necessary.
