About this file
This is a Statement of Work (SOW) template for ARPA-H-SOL-26-164, the Simulation-augmented, Real-time Platform Adaptive Seamless Trials (SURPASS) program. The template provides standardized formatting and guidance for proposers to describe technical tasks, subtasks, and associated deliverables for potential Other Transaction Agreement awards. The document establishes a structured framework requiring proposers to define task objectives, responsible organizations, work locations, detailed descriptions of technical approaches, start and end dates (measured in months 1-60), and identification of any Animal Subject Research (ASR) or Human Subject Research (HSR) components.
The template requires proposers to specify measurable milestones with designations for payable milestone payments, along with deliverables such as data, reports, PowerPoint presentations, regulatory documents, or Excel models. The hierarchical structure accommodates multiple levels of detail, including tasks, subtasks, and sub-subtasks, with alignment requirements across the SOW and associated cost workbooks. All proprietary information must be excluded from submissions, and proposers must remove instructional text before final submission. ARPA-H, operating under the Department of Health and Human Services National Institutes of Health, will host an informational webinar on October 15, 2026, and a two-day Proposers' Day on November 6 and 9, 2026, with questions directed to SURPASS@ARPA-H.GOV.

Scope of Work Use this template to describe the planned technical tasks and subtasks. All text in blue font may be deleted. There is no page limit to this template.
Notes:
1. Do not include proprietary information.
2. Please identify if a task will include Animal Subject Research (ASR) or Human Subject Research (HSR).
3. Tasks, subtasks, milestones should align across SOW (this document) and Cost Workbook.
Please remove all instructions in blue when submitting the SOW.
Task #: Title Objective: general objective of the task, why is it needed? What does it determine?
Organization: name of organization responsible for completing the task Location: Where work is conducted Task description: Detailed description of the approach. This needs at least one paragraph, but usually more. This is similar to high-level Materials and Methods section in a publication. If Task # has subtsaks, then this description may be more general, with details provided in corresponding subtask descriptions below.
Task start date: Month (1 through 60) Task end date: Month (1 through 60) ASR: [yes/no, if yes, please describe] HSR: [yes/no, if yes, please describe] List of subtasks:
Subtask #.#: Title
Subtask #.#: Title
Milestones: If there are no subtasks in this task, then list all measurable observable milestones. If there are subtasks, then put “see subtasks” and list milestones under corresponding subtasks. Please number milestones with unique number-identifiers. Please indicate which milestones are payable, i.e., would have an associated milestone payment in a potential contract.
Deliverables: Describe deliverables, usually data, or reports in PowerPoint, or regulatory documents, or Excel models, etc.
Subtask #.#: Title (should match one of subtasks listed under Task #) Objective: subobjective Organization: name of organization responsible for completing the task Location: Where work is conducted Subtask description: Detailed description of the approach. This needs at least one paragraph, but usually more. This is similar to high-level Materials and Methods section in a publication.
Subtask start date: Month (1 through 60) Subtask end date: Month (1 through 60) ASR: [yes/no, if yes, please describe] HSR: [yes/no, if yes, please describe] List of sub-subtasks: (if applicable)
Sub-subtask #.#.#: Title
Sub-subtask #.#.#: Title
