Failure to do so may render the proposal unacceptable. On lump-sum line items, provide the total price only. SECTION C – Description/specifications/statement of work
This requirement consists of clear diesel and regular unleaded fuel to above ground storage tanks and fuel tanker truck located at the Mineral Headquarters, Southwest Entrance, and Manzanita Lake fueling sites. Location address and descriptions are listed below. Contractor shall ensure the delivery driver contacts the NPS prior to each delivery to confirm delivery location, fuel type, and quantity prior to delivery. Fuel tanks shall not be filled beyond 90 percent capacity per NFPA 30 22.11.4.5.

DELIVERY LOCATIONS
1. Mineral Headquarters Area - Estimated Quantity: 10,000 gallons unleaded & 1,500 gallons diesel Lassen Volcanic National Park 38050 Highway 36 East Mineral, CA 96063 40°20'51.7"N 121°36'38.4"W
The Mineral Headquarters Area is located Approximately 38 miles east of Red bluff on highway 36 (1/2 mile west of the town of Mineral). Follow the signs to the “Receiving” area after entering the compound.
Please note this location is located outside the park boundary.
2. Manzanita Lake Maintenance Area – Estimated Quantity: 5,500 gallons diesel Lassen Volcanic National Park 39489 Highway 44 Shingletown, CA 96088 40°32'26.9"N 121°34'36.8"W
The Manzanita Lake Maintenance Area is located on Highway 89 near the Northern entrance to Lassen Park. The Manzanita Lake Maintenance Area and fuel tanks are located approximately ¼ mile south of the Highway 89/Highway 44 junction down a service road on the right-hand side of the highway (if you get to the entrance booth you’ve gone too far).
3. Southwest Entrance Area – Estimated Quantity: 3,500 gallons diesel Lassen Volcanic National Park 21820 Lassen National Park Highway 89 Mineral, California 96063 40°26'11.9"N 121°31'59.3"W
The Southwest Entrance Area is located in the Southwest corner of the Lassen Park on Highway 89 approximately 5 Miles North of the Highway 89/36 Junction, and approximately 9 Miles East of the town of Mineral. The fuel truck will be located in the visitor center parking lot on the right-hand side of the road immediately after passing the Park Entrance Station.

Electronic Invoicing and Payment Requirements – Invoice Processing Platform (IPP) (Apr 2013)
Payment requests must be submitted electronically through the U. S. Department of the Treasury's Invoice Processing Platform System (IPP). "Payment request" means any request for contract financing payment or invoice payment by the Contractor. To constitute a proper invoice, the payment request must comply with the requirements identified in the applicable Prompt Payment clause included in the contract, or the clause 52.212-4 Contract Terms and Conditions - Commercial Items included in commercial item contracts. Under this contract, the following documents are required to be submitted as an attachment to the IPP invoice:
Vendor’s Itemized Invoice & Copy of OPIS (Chico) Daily Average Rack Average for Date of Delivery
Failure to attach the required documentation as specified above will result in the IPP invoice being rejected. The Contractor must use the IPP website to register access and use IPP for submitting requests for payment. The Contractor Government Business Point of Contact (as listed in SAM) will receive enrollment instructions via email from the Federal Reserve Bank of Boston (FRBB) prior to the contract award date, but no more than 3 - 5 business days of the contract award date. Contractor assistance with enrollment can be obtained by contacting the IPP Production Helpdesk via email ippgroup@bos.frb.org or phone (866) 973-3131. If the Contractor is unable to comply with the requirement to use IPP for submitting invoices for payment, the Contractor must submit a waiver request in writing to the Contracting Officer with its proposal or quotation.
Contractor Performance Assessment Reporting System (CPARS)
CPARS is a web-based system that allows for electronic processing of performance evaluation reports.
The reports are available for Government use in evaluating past performance as part of a source selection action in accordance with FAR 42.1500.
SECTION H – Special contract requirements
Work performed under this contract shall comply with applicable federal, state, and local safety and occupational health laws and regulations, including adherence to any local government health orders in effect to reduce the rate of transmission of Novel Coronavirus Disease 2019 (“COVID-19”). This includes, but is not limited to, Occupational Safety and Health Administration (OSHA) standards, 29 CFR, Part 1910, and Part 1926. Where the requirements of applicable laws, criteria, ordinances, regulations, and referenced documents vary the most stringent requirements shall apply.
The Contractor must comply with agency COVID-19 workplace safety requirements while in federal workplaces. (d) Comptroller General Examination of Record. The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, as defined in FAR 2.101, on the date of award of this contract, and does not contain the clause at 52.215-2, Audit and Records-Negotiation.
(1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor’s directly pertinent records involving transactions related to this contract.
(2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement.
Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved.
(3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law.

QUOTATION PACKAGE (What to Submit)
NOTE: To be eligible for award, offerors must have an active online SAM record at http://www.sam.gov at the time of award. SAM registration is completely free of charge.
Submit Standard Form 18 (SF-18), Request for Quotations, with blocks 13 through 16 on the first page completed. An official having the authority to contractually bind the offeror’s company must sign the SF-18 in accordance with the procedures prescribed in FAR 4.102;
Submit the price schedule from Section B with offered pricing, legal business name, DUNS number, point of contact name, email address and phone number;
If an amendment was issued, sign and submit Standard Form 30 to acknowledge it;
Submit either the filled-out reps/certs provision 52.212-3 (Attachment 1) manually or have completed Representations and Certifications in SAM.gov;
Unless you have previously certified in your SAM representation that you do not use prohibited telecom equipment, submit completed provisions 52.204-24(d)(1)&(2) and 52.204-26(c)(1)&(2) (Attachment 2). This is not needed if the Offeror has previously responded “does not” in their annual SAM representation at 52.204–26, Covered Telecommunications Equipment or Services— Representation, or in paragraph (v)(2)(ii) of 52.212–3, Offeror Representations and Certifications.
The Government will consider an offeror's noncompliance with these instructions to be indicative of the type of conduct that it may expect from the offeror during contract performance. Failure to provide the required information may be cause for rejection of the offer.

Responsibility Determination
A. It is the general policy of the Department of the Interior that contract shall be awarded only to contractors determined to be responsible in accordance with Part 9 of the Federal Acquisition Regulations (FAR). No contract shall be awarded to any person or firm unless the Contracting Officer first makes an affirmative determination that the prospective contractor is responsible within the meaning of FAR Part 9. Before making a determination of responsibility, the Contracting Officer shall have in their possession or obtain information sufficient to satisfy themselves that a prospective contractor currently meets the minimum FAR Part 9 standards.
B. In accordance with Section 9.104 of the Federal Acquisition Regulations, award of a contract will be subject to the Contracting Officer's determination that the offeror meets the following minimum standards for performing the work:
(1) Has adequate financial resources;
(2) Is able to meet the required delivery or performance dates;
(3) Has a satisfactory performance record;
(4) Has the organization, experience, accounting and operational controls, and technical skills, or the ability to obtain them;
(5) Has a satisfactory record of integrity and business ethics;
(6) Has the necessary production and technical equipment and facilities or the ability to obtain them;
(7) Is otherwise qualified and eligible to receive an award under applicable laws and regulations.
Section M – Evaluation Factors
52.212-2 Evaluation—Commercial Products and Commercial Services (Nov 2021)
1. The Government will award a contract resulting from this Solicitation to the responsible offeror whose offer conforming to the Solicitation will be most advantageous to the Government, on the basis of the lowest evaluated price of quotations meeting or exceeding the acceptability standards. All non-cost factors will be evaluated on a pass/fail basis. Best value is expected to result from the selection of the technically acceptable quotation with the lowest evaluated price.
2. A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer’s specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award.
