2. Submit a quotation which address the Factors below.

3. Quotations are limited to 10 pages. Any Cover Page, Table of Contents or Executive

Summary, if included, must be clearly headered and do not count toward the page limit.

Technical appendices and other attachments are not permitted, including resumes. Past

Performance information shall be limited to one (1) page per record unless a copy of a

CPARS record(s) is provided. In this instance, the CPARS record (regardless of number of pages) will constitute 1 page.

4. Price shall be submitted in the attached Bid Schedule (Attachment 2).

5. Vendors are advised that FAR clause 52.217-8 Option to Extend Services is included in this solicitation and is intended to be included in the resultant award. However, the price for the effort associated with FAR 52.217-8 will not be included in the total awarded value at contract award.

6. The Government may choose to exercise the Option to Extend Services at the end of any performance period (base or option periods). If, at the end of the contract’s period of performance (the end of the base period or any option period) and within the time period established in the clause, the Government chooses to exercise this option, the pricing will be pursuant to the rates specified in the contract for the preceding performance period.

Evaluation and Award Basis Award will be made to the responsible vendor whose quotation conforming to the solicitation will be most advantageous to the Government, price and other factors considered. The following factors will be used to evaluate quotes:

1. Technical Approach (Pass/Fail)

This factor will be assessed on a pass/fail basis based on the vendor’s Technical Approach to:

• Provide a sound hiring and retention plan for local staffing; and

• Provide a sound quality control plan.

2. Past Performance

Provide at least three (3) similar projects completed within the last five (5) years (or is currently performing). For each relevant contract or reference, provide the following:

a. Agency/company and contact point

b. Period of Performance

c. Dollar value of contract

d. Contract number and type of work

e. A description of the project, relevance to the current project, and schedule of compliance will be reviewed.

The above information can be replaced with a record from the Contractor’s Performance

Assessment Reporting system (CPARS) if the record has been finalized. If applicable, provide a complete copy of the CPARS record(s). Each CPARS record will be considered one (1) page regardless of pages submitted.

3. Price

• Provide a completed Bid Schedule (Attachment 2). The total proposed price must include all labor, materials, supplies, equipment, travel, overhead, etc.

• Pricing will be evaluated for fairness and reasonableness to include assessment of skill mix, level of effort, and hourly rates.

• The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. To account for the option periods possible under FAR clause 52.217-8 Options to Extend Services (maximum of six months), the Government will evaluate the option by adding six months of the offeror’s Option Year 4 price to the offeror’s total price. This amount will be the total evaluated price.

• The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced.

• It is anticipated that price reasonableness will be established through competition, comparison with the IGE, and recent historical prices. Evaluation of options shall not obligate the Government to exercise the option(s).

The Government will evaluate quotations in accordance with the factors above. All quotations will be evaluated on a Pass/Fail basis under Factor 1 Technical Approach.

Quotations that “Fail" Factor 1 will not be further evaluated or considered for award.

Quotations that "Pass" Factor 1 will continue to be evaluated under Factor 2 Past Performance and Factor 3 Price. The Government will make a best value award using the trade-off method, where past performance is approximately equal to price.

The inside and outside of seats, covers and risers on each toilet will be scrubbed and washed clean with a disinfectant cleaner. To assist in keeping the inside of the risers clean, a pressure washer is recommended; however, a non-stick cooking vegetable spray or other approved product may be applied. The used cleaning fluid will not be dumped outside of the restroom building where it could kill vegetation or contaminate water sources; it may be dumped into the toilet vault or removed off-site.

• Clean and disinfect* restroom building and stalls: All surfaces, including walls, ceiling and floors will be kept clean of all dirt, cobwebs, and other substances by sweeping, spraying, or scrubbing with a disinfectant cleaner. The floors will be swept and mopped with disinfectant cleaner, and all standing water swept out (or otherwise removed) after cleaning, as much as possible.

• Stock toilet paper: All restroom stalls/vaults will be stocked with toilet paper during each cleaning visit. Any toilet paper rolls more than ½ empty will be replaced. Toilet paper dispensers will be filled to their maximum capacity.

11.0 CLEANING PRODUCTS & MATERIALS

Unless otherwise specified in the contract documents, standard commercial grade products, supplies, equipment, paper goods, and restroom supplies, of types and sizes to fit existing agency provided dispensers must be provided by the contractor. These products are subject to inspection and approval by the COR. The COR may require the contractor to submit, for approval, a list of proposed products and/or actual product samples of the supplies and equipment the contractor intends to use.

The contractor shall not use alternative restroom product dispensers in lieu of those already in place without the approval of the COR. The BLM reserves the right to modify or install additional paper product dispensers.

All cleaning materials used must be of a type that will not have an immediate or extended detrimental effect on any of the surfaces being cleaned. Water, detergent, and appropriate disinfectant will be used for all janitorial services. In the performance of this contract, the contractor will make maximum use of bio-based products that are United States Department of

Agriculture-designated items and products containing recovered materials that are EPA-designated items unless—

The product cannot be acquired—

• Competitively within a time frame providing for compliance with the contract performance schedule.

• (ii) Meeting contract performance requirements; or

• (iii) At a reasonable price.

Information about this requirement and these products is available at:

https://www.biopreferred.gov/BioPreferred/

12.0 NOTIFICATIONS OF SITE CLEANING DELAYED

Contractor shall provide an email notification when site cleaning visits experience delays.

a) All notifications must be by email. The contractor may call with information or questions, but must follow up with a brief, written record of skipped, delayed, or re-scheduled site cleaning visits. The BLM will confirm any schedule adjustments or amendments in writing via email.

b) Snow and/or impassable road conditions during inclement weather may prevent access to the recreation sites and/or specific restroom facilities. This is especially a concern during the winter months (December through March/April); and flexibility in re-scheduling restroom cleaning visits will be necessary. If weather conditions prevent a scheduled restroom cleaning visit, the contractor must notify the COR one day in advance or on the same business day as the originally scheduled restroom cleaning visit to reschedule. The restroom cleaning visit should be rescheduled as soon as practicable, weather and road conditions permitting. The contractor shall notify the COR by email on either the same business day or on the next business day after the rescheduled restroom cleaning duties have been completed.

c) BLM acknowledges that occasionally there will be other needs for flexibility in the approved annual cleaning schedule/calendar. Such reasons may include but are not limited to equipment and vehicle failures, staff shortages, and other unforeseen circumstances. The contractor shall attempt to adhere to the approved annual cleaning schedule/calendar as closely as possible. However, when reasons other than weather cause a delay to the approved annual cleaning schedule/calendar, the contractor must notify the COR one day in advance or on the same business day as the originally scheduled restroom cleaning to reschedule as soon as practicable. The contractor shall attempt to make up the restroom cleaning visit by Friday of the same week. If this is not possible, the contractor shall reschedule as soon as possible and preferably prior to the next high-use recreation day (generally holidays and weekends).

The contractor shall notify the COR on either the same business day or on the next business day after the rescheduled restroom cleaning duties have been completed.

13.0 INSPECTIONS

Contract work will be inspected randomly by BLM employees to ensure satisfactory completion of work. BLM employees include and are not limited to: COR, outdoor recreation planners, park rangers, law enforcement rangers, seasonal staff, or any BLM employees assigned to conduct restroom facility inspections.

Restroom cleaning visits will be evaluated on whether they are performed according to the cleaning duties specified in sections 9.0-10.0, judged based on observations made during restroom cleaning inspections and/or information recorded on completed inspection report forms, and reported as deficient or unperformed, as defined below:

a) Deficient: Work that is only partially completed, or unsatisfactory or inadequate work that does not meet the standards specified in sections 9.0-10.0.

b) Unperformed: None of the required restroom cleaning duties were performed at the scheduled recreation site. This is a scheduled restroom cleaning visit that was not performed and not rescheduled (with approval from the COR) prior to the next regularly scheduled cleaning visit.

14.0 DEFICIENT OR UNPERFORMED WORK

If the BLM finds a site cleaning visit to be deficient or unperformed, the COR will notify the contractor immediately. The contractor shall have one to two business days in which to complete or re-perform the scheduled work. In certain situations, or time periods, an extension may be granted by the COR.

At the initial documentation of a deficient or unperformed scheduled restroom cleaning visit, the

BLM generally will not assess a penalty but invite the contractor to return to the recreation site to complete or re-perform the work. The subsequent restroom cleaning visit to complete or re-perform the documented deficient or unperformed work cannot be invoiced and will not result in any additional payments. In other words, the Contractor may not bill the BLM twice for a repeat restroom cleaning visit that was performed to correct deficient work.

After completing or re-performing the work, the Contractor shall notify the COR on the same day the work is completed or as soon as practicable. If the contractor does not re-perform the work within the specified timeframe, Contractor shall forfeit the billing of the restroom cleaning visit.

15.0 DELIVERY

The contractor shall provide the following:

a) Restroom cleaning, as specified in sections 9.0-11.0. This includes all labor, supervision, tools, equipment, supplies, and transportation necessary to clean and sanitize recreation sites (inside and outside), stock toilet paper, and remove trash from inside and outside restroom buildings.

b) Annual restroom cleaning schedule/calendar, covering a 12-month period starting after the contract award date, as specified in section 7.0.

All project documents (annual restroom cleaning schedule/calendar, monthly schedule/calendars, and cleaning delayed notifications) will be delivered to the BLM COR. Periodic restroom cleaning inspections will be completed by BLM staff.

16.0 INVOICING/BILLING

As work is completed for this contract, the contractor shall submit invoices, bills, or statements monthly. Invoices must include an itemization of cleaning duties completed, dates of cleaning visits, total charges, and date of billing. To ensure prompt payment and proper contract administration, the contractor is encouraged to submit bills the first week of each month.

17.0 GOVERNMENT-FURNISHED PROPERTY

BLM will furnish keys for facilities and toilet paper dispensers.

18.0 SAFETY

BLM acknowledges that physical and biological (e.g., bacteria, viruses) may be present at these sites; the contractor must ensure the safety of their employees and provide adequate personal protective equipment that meets industry standards.

19.0 REPORTING SITE DAMAGE AND REPAIRS

Contractor shall report and provide photos of any damages and/or needed repairs to the recreation site facilities (inside and outside). Repairs will be performed at the BLM’s discretion by an agent of the BLM’s choosing. Repair costs will be paid by the BLM. If available for such repairs, the contractor will contact the COR and provide an estimate of repair costs. However, the contractor is not permitted to make repairs without direct authorization from the contracting officer.

The contractor is responsible for any damage occurring to recreation site facilities, structures, developments, roads, areas, or land associated with work duties under this contract and will restore the damaged item or area to its original condition. This restoration will include any action needed to restore the damaged item or area, including and not limited to painting, refinishing, repair, reconstruction, replacement, or rehabilitation.

QUALITY ASSURANCE SURVEILLANCE PLAN

(QASP)

1 INTRODUCTION

This quality assurance surveillance plan (QASP) is pursuant to the requirements listed in the

Statement of Work (SOW) entitled St. George Field Office Recreation Site Janitorial. This plan sets forth the procedures and guidelines the United States Department of the Interior, Bureau of

Land Management, Color Country District, will use in ensuring the required performance standards or services levels are achieved by the contractor.

1.1 Purpose

1.1.1 The purpose of the QASP is to describe the systematic methods used to monitor performance and to identify the required documentation and the resources to be employed. The QASP provides a means for evaluating whether the contractor is meeting the performance standards/quality levels identified in the SOW and the contractor’s quality control plan (QCP), and to ensure that the

Government pays only for the level of services received.

1.1.2 This QASP defines the roles and responsibilities of all members of the contract, identifies the performance objectives, defines the methodologies used to monitor and evaluate the contractor’s performance, describes quality assurance documentation requirements, and describes the analysis of quality assurance monitoring results.

1.2 Performance Management Approach

1.2.1 The SOW structures the acquisition around “what” service or quality level is required, as opposed to “how” the contractor should perform the work (i.e., results, not compliance). This QASP will define the performance management approach taken by BLM to monitor and manage the contractor’s performance to ensure the expected outcomes or performance objectives communicated in the SOW are achieved. Performance management rests on developing a capability to review and analyze information generated through performance assessment. The ability to make decisions based on the analysis of performance data is the cornerstone of performance management;

this analysis yields information that indicates whether expected outcomes for the project are being achieved by the contractor.

1.2.2 Performance management represents a significant shift from the more traditional quality assurance (QA) concepts in several ways. Performance management focuses on assessing whether outcomes are being achieved and to what extent. This approach migrates away from scrutiny of compliance with the processes and practices used to achieve the outcome. A performance-based approach enables the contractor to play a large role in how the work is performed, as long as the proposed processes are within the stated constraints. The only exceptions to process reviews are those required by law (federal, state, and local) and compelling business situations, such as safety and health. A “results” focus provides the contractor flexibility to continuously improve and innovate over the course of the contract as long as the critical outcomes expected are being achieved and/or the desired performance levels are being met.

1.3 Performance Management Strategy

1.3.1 The contractor is responsible for the quality of all work performed. The contractor measures that quality through the contractor’s own quality control (QC) program. QC is work output, not workers, and therefore includes all work performed under this contract regardless of whether the work is performed by contractor employees or by subcontractors. The contractor’s quality control program (QCP) will set forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in the SOW. The contractor will develop and implement a performance management system with processes to assess and report its performance to the designated

Government representative. The contractor’s QCP will set forth the staffing and procedures for self-inspecting the quality, timeliness, responsiveness, customer satisfaction, and other performance requirements in the SOW. This QASP enables the Government to take advantage of the contractor’s QCP.

1.3.2 The Government representative(s) will monitor performance and review performance reports furnished by the contractor to determine how the contractor is performing against communicated performance objectives. The contractor will be responsible for making required changes in processes and practices to ensure performance is managed effectively.

2 ROLES AND RESPONSIBILITIES

2.1 The Contracting Officer

The contracting officer (CO) is responsible for monitoring contract compliance, contract administration, and cost control and for resolving any differences between the observations documented by the Contracting

Officer’s Representative, and the contractor. The CO will designate one COR as the Government authority for performance management. Additional representatives serving as project inspectors may be appointed as well as the contractor’s performance, and must be identified and designated by the CO.

2.2 The Contracting Officer’s Representative

The COR is designated in writing by the CO to act as his or her authorized representative to assist in administering a contract. COR limitations are contained in the written appointment letter. The COR is responsible for technical administration of the project and ensures proper Government surveillance of the contractor’s performance. The COR is not empowered to make any contractual commitments or to authorize any contractual changes on the Government’s behalf. Any changes that the contractor deems may affect contract price, terms, or conditions shall be referred to the CO for action. The COR, along with the Project Inspector, will have the responsibility for completing QA monitoring forms used to document the inspection and evaluation of the contractor’s work performance. Government surveillance may occur under the inspection of services clause for any service relating to the contract. The COR will track and communicate with the CO findings, trends, and any other contract support to the CO.

3 IDENTIFICATION OF REQUIRED PERFORMANCE STANDARDS /

QUALITY LEVELS

The required performance standards and/or quality levels are included in the SOW. If the contractor meets the required service or performance level, it will be paid the monthly amount agreed on in the contract.

Failure to meet the required service or performance level will result in a deduction from the monthly

St. George Field Office Recreation Site Janitorial

4 | P a g e amount.

4 METHODOLOGIES TO MONITOR PERFORMANCE

4.1 Surveillance Techniques

To minimize the performance management burden, simplified surveillance methods shall be used by the

Government to evaluate contractor performance when appropriate. The primary method of surveillance is:

➢ Random monitoring, which shall be performed by the COR or designated BLM employee (outdoor recreation planners, park rangers, law enforcement rangers, or seasonal staff).

5 QUALITY ASSURANCE DOCUMENTATION

5.1 The Performance Management Feedback Loop

The performance management feedback loop begins with the communication of expected outcomes.

Performance standards are expressed in the SOW and are assessed using the performance monitoring techniques detailed therein.

5.2 Monitoring Inspection Forms

Government QA surveillance will be reported using inspection report forms. The form, when completed, will document the Government’s assessment of the contractor’s satisfactory or unsatisfactory performance under the contract to ensure that the required results are being achieved.

5.2.1 The CO and COR will retain a copy of all completed QA surveillance forms.

6 ANALYSIS OF QUALITY ASSURANCE ASSESSMENT

6.1 Determining Performance

6.1.1 Government shall use the monitoring methods cited to determine whether the performance standards have been met. If the contractor has not met the minimum requirements, it may be asked to develop a corrective action plan to show how and by what date it intends to bring performance up to the required levels. Failure to meet performance standards may result in a deduction from the monthly payment.

6.2 Reporting

6.2.1 Inspection report forms may be shared with the Contractor. Forms with negative findings will be shared within one week, as practicable, with the Contractor to review and make remediations. The

Contractor is responsible to inform the CO or COR of when remediation has been completed for review by the COR. Completed Inspection report forms will become part of the QA documentation within the contract file. It will enable the government to demonstrate whether the contractor is meeting the stated objectives and/or performance standards, including cost/technical/scheduling objectives.

6.3 Reviews and Resolution

6.3.1 The CO and COR may require the contractor’s project manager, or a designated alternate, to meet, in person or via other means, to discuss performance evaluation. The frequency will be as necessary, or per the contractor’s request, and could include in-depth reviews with the contractor, including appropriate self-assessments by the contractor. The agenda of the reviews may include:

➢ Inspection performance and trend analysis

➢ Issues and concerns of both parties

➢ Projected outlook for upcoming months and progress against expected trends, including a corrective action plan analysis

➢ Recommendations for improved efficiency and/or effectiveness

6.3.2 The CO and/or COR must coordinate and communicate with the contractor to resolve issues and concerns regarding marginal or unacceptable performance.

6.3.3 The COR, CO, and contractor should jointly formulate tactical and long-term courses of action.

Decisions regarding changes to metrics, thresholds, or service levels should be clearly documented.

Changes to service levels, procedures, and metrics will be incorporated as a contract modification at the convenience of the CO.
