QUOTATION SUBMISSION:

Statement of Work The National Water Quality Lab (NWQL) uses two (2) Perkin Elmer Inductively Coupled Plasma – Optical Emission Spectra (ICP-OES) Optima 8300 DV instruments, with Cetac ASX-560 autosamplers, for trace metal and major cation analysis. Timely and reliable instrument service and maintenance is critical to this analysis as these instruments analyze thousands of samples per year. The NWQL is located in building 95 at the Denver Federal Center, located in Lakewood, CO.

Scope:

The Contractor shall provide full on-site service, maintenance and software support, along with travel for the Perkin Elmer and Cetac instruments listed below.

Work Requirements:

A. The Contractor shall diagnose equipment failure (including failure caused by OEM supplied software) and provide all parts, labor, testing equipment, travel, software and firmware upgrades required to maintain the equipment covered in this order to the original equipment manufacturers’ (OEM) published operating capabilities and specifications. All software and firmware provided by the Contractor shall be free of malicious code and spyware.

B. The Contractor shall perform preventive maintenance in accordance to the procedures and at the frequencies recommended by the OEM (at least annually) and is included as part of this service agreement. Preventive maintenance shall be conducted at the NWQL. All preventative maintenance calls shall be scheduled with the technical liaison or designated NWQL representative. Within 30 days of contract award, the equipment to be covered shall be inspected by the Contract Service Engineer and shall be maintained to the OEM specifications for operations and performance. The Contractor shall coordinate this inspection visit through the NWQL’s Technical Liaison.

C. All repairs and preventative maintenance, provided by the Contractor, shall be performed by a certified, factory trained technician and be scheduled with the concurrence of the technical liaison or other authorized NWQL employee. If the technician is not factory trained, he/she shall provide documentation of adequate training, which may include references from two other customers, and proof of at least one year of successful experience repairing the Perkin Elmer equipment in the contract. The documentation shall be provided upon request from the technical liaison or other authorized NWQL employee and prior to the commencement of any work.

D. Emergency repairs shall be completed within three (3) working days of verbal notification to the Contractor by the Technical Liaison or other authorized NWQL representative. Emergency repair is herein defined as “as system or component of a system that cannot carry out its intended function without repair”. An emergency repair will be so specified, at the initiation of the service call, by the initiator of the service call, at the sole discretion of the initiator of the service call.

E. The Contractor shall provide non-emergency repairs as required within five (5) working days of its verbal notification by the Technical Liaison or other authorized NWQL representative. Non-emergency repair is herein defined as “a system or component of a system that requires repair or service but can presently operate or carry out its intended function with the repair or service”. If the category of the repair is not specified by the initiator of the call, the repair shall be considered a non-emergency repair.

F. If the repair requires parts, the Contractor shall procure and arrange to have the new, OEM parts on site within forty-eight (48) hours of the initial service visit. This contract shall include the cost of all parts required for repair or service.

G. The Contractor shall be responsible for shipping expenses of all replacement parts and return shipment expenses for all non-expendable replacement parts.

H. When the service engineer and the technical liaison concur that the equipment requires offsite repair, the Contractor shall provide shipping expenses for both the shipment of the equipment to the repair facility and the return to the NWQL.

I. The Contractor shall provide a service report to the technical liaison or other authorized NWQL representative within three (3) business days of completing a service call.

J. In the event of a service repair that cannot be completed within five (5) business days of the original request, the Contractor shall submit to the Technical Liaison or other authorized NWQL representative- in person or electronically- a written detailed report. This report shall be received by the NWQL no later than the sixth (6th) business day of such request and shall include the nature of the repair, measures that are being pursued to complete the repair, and an estimate of the time period required to complete the repair. The Contractor shall include a contingency plan for repairs that cannot be completed within five

(5) working days. This contingency plan shall provide additional procedures for obtaining expertise and/or resources when the equipment cannot be diagnosed and repaired within the set period of time.

K. The Contractor shall provide local or toll-free telephone numbers for unlimited telephone service and support, available 8 AM to 5 PM local time, Monday through Friday, excluding Federal holidays, as part of this order. These numbers shall be staffed to accommodate the aforementioned hours.
